Following the as-built submittal of the referenced on-site facility you had three concerns which I would
like to clarify:
1) Soil Classification in zone 0 to 6feet below bottom of leach treneh.
During excavation of the leach trench a stump disposal pit was discovered. It was
removed by overexcavating the trench by 6 feet. The soils were consistent with what the
design engineer, Mr. Robert Pintner, P.E. had classified during his pre-construction
investigation test pit. In addition, no ground water was encountered during
overexcavation.
2) Seven day monitoring of water level at elevation of leach field.
The owner excavated a trench approximately 20 feet down gradient of the west edge of the
leach field to an elevation 9 feet below the bottom of the leach field. This trench was kept
open for at least a week for the deposit of organic debris from clearing the lot. No water
was observed to be present in the trench.
3) Owner installation certifwation.
Rick llammond, owner and installer of record, attended a seminar instructed by Mr. Dan
Roth of your office and should be on record as having done such.

The subject permit #SW990316 issued by this office for a single family well and/or on-
site wastewater system, is due to expire 365 days after it's issuance on September 3,
1999.
If this is a well permit and you have drilled the well, a well log must be sent to this office
for documentation of the installation and to close the permit.
If this is an on-site wastewater system and a licensed Professional Engineer has inspected
the installation, the original as-built inspection report must be sent to this office for
review, approval and documentation. All inspection reports must be submitted within 30
days of construction completion.
A new permit must be obtained from this office for a well and/or on-site wastewater
system NOT installed by the expiration date. However a new permit can be issued free of
charge for a second year if the application for the renewal is received on or before the
date of expiration of the original permit for which a fee was paid.
When applying for a new permit after the original permit has expired or for more than a
second year, the fees are: $320.00 for an on-site wastewater permit and $120.00 for a well
permit.

Residential Development &Lot 3A, Block 5, Robin Hill Subdivision, Unit 2 description of
probable impacts to adjacent properties.
The above referenced lot is the last lot to develop among the adjacent properties. Attached are
two proposed well and septic site plans: Sheet 1, Separation Distances Plan depicts the adjacent
properties and separation distances for wells, septic fields, and slopes; Sheet 2, Site Layout
depicts the proposed house, well, and septic system location. The septic is wholly contained in
the 70' x 155' septic reserve area as described on Plat 93-138. The adjacent properties and
probable impacts are described as:
Lots I and 10, Block 1, of Sun Valley Subdivision are located adjacent the south property
line &the subject property. Lot 10 is serviced by a community well located over 200 feet
from Lot 3A. The 100-foot radius well separation crosses into the middle north of Lot 10
as shown on Sheet 1. A secondary septic cannot be constructed within this area of LOt
10 due to the location of existing building to the property line and the 50-foot slope
separation west of the building. Therefore, the improvements shown on Sheet 2 do not
impact these lots.
Lot 4, Block 4, of Robin ltill Subdivision is located to the east of the subject property.
Separating these lots is a 60-foot Public Right of Way. The well is located greater than
100 feet &LOt 3A. Therefore, the improvements shown on Sheet 2 does not imp. act this
lot.

Lot 3B, Block 5, of Robin Hill Subdivision Unit 2, is located adjacent to the north
property line of Lot 3A. The 100-foot well separation does not cross into this lot.
Therefore, the improvements shown on Sheet 2 do not impact this lot.
Lots 6 through 9, Block 13, of Mountain Park Estates are located adjacent to the subject
property's west property line. The septic systems are generally located on the east side of
the lots and wells to the west. The improvements shown on Sheet 2 do not impact these
lots.
Based on the locations of existing improvements on potential development in the area, the
improvements requested for permit pose no probable impacts the adjacent parcels. If you have
