Gentlemen:
My firm was contacted to investigate a failed septic system at the above referenced lot.
The system was completely inundated with liquid. The fluid levels were approximately
three feet above the lateral pipe. I informed the owners that the systems were in failure.
The owners requested my firm investigate the possibility of installing a replacement and
upgrading the system to a five-bedroom system. A single test hole was excavated on
July 1, 2001. The soils report and a percolation test result is attached. Ground water
was monitored for seven days. No groundwater or bedrock was encountered in the test
hole.
The lot is approximately 0.92 acres in size. Lot 2 slopes to the north at approximately 3
to 5 percent. The proposed installation will be located in the northern portion of the lot.
The existing field will be abandoned. The septic tank will be abandoned during the
installation. A new 1,500-gallon septic tank will be installed outside the well radius.
The proposed location is greater than 100 feet away from the existing well serving this
property and 25 feet from the water service lines. The surrounding wells are located
greater than 100 feet from the proposed installation. The proposed installation will not
affect the future development of the surrounding or existing lots. See the attached
design.

Comments: This investigation was completed in compliance with MOA guidelines, regulations,
and best industry practices / methods. The assessment of the condition of the well and septic
applies only to the conditions as of the day tested. The flow and absorption rates may change
due to subsurface conditions that may not be observed from the surface, changes in land use,
local soil characteristics, groundwater levels that may fluctuate during the year, quality of
construction (workmanship & materials), the water usage of the family being served by the
system and maintenance. The operational life of all well and septic systems are subject to
these various and dynamic characteristics and are outside the control of the evaluator of the
