Attached are the completed homeowner's agreement, final electrical inspection
sign-off and copies of the scale tickets Indicating the point of purchase for the

lot.
In addition, the air compressor and in line air gauge have been installed in the
garage serving the house. The air gauge currently reade 2.0 pounds per square

We have revised the septic system design for the subject lot to include a bottomless
sand filter in lieu of the shallow bed system previously proposed. The sand filter
requires much less surface area and will be more effective in treating the septic effluent
generated in the house. Please review the attached design revisions and modify the
permit for this lot to include the bottomless sand filter.

ON-SITE WASTEWATER DISPOSAL SYSTEM
CONSTRUCTION AND MATERIAL SPECIFICATIONS
SUBJECT: LOT 8, BLOCK 4, MOUNTAINSIDE VILLAGE SUBDIVISION
GENERAL:
1. The scope of this project includes the procurement and placement of a
new 1,500 gallon combination septic tank and lift station and pressure
distribution system. Work also includes the construction of a new 30'
long x 12' wide Bottomless Intermittent Sand Filter at the location
shown on the attached Site Plan. A minimum of 2' of coarse sand
meeting the specification from Anchorage Tank must be imported and
placed in the filter area. The 6" of pea gravel and distribution piping
must then be constructed atop the imported filter sand material in
confor~..ance .with specifications furnished by !,he .supplier. The.
distribution piping for the filter must be placed 3 below the original
ground surface at the area shown on the Site Plan to provide a
minimum of 4' separation to groundwater. All work must be completed
in strict conformance with the manufacturer's requirements for
installation of the intermittent sand filter.
Construction shall be in accordance with the approved site plan,
design drawings, Municipal Permit with any special provisions or
conditions, and all applicable State and Municipal Wastewater
Disposal Regulations.
The Contractor shall be responsible for obtaining all underground utility
locates and for the layout of the septic system and verification of the
location of all lot lines.
4. Unless specifically agreed otherwise, the contractor shall be
responsible for final grading areas subsequently depressed from soil
settling. Property owner shall be responsible for revegetation of
affected areas unless specifically agreed otherwise.
5. Contractors installing wastewater disposal systems must be certified
by the Municipal Department of Health and Human Services for
system installations. Owners installing their own systems must receive
pdor approval from D.H.H.S. before beginning system installation.
SEPTIC TANK/LIFT STATION INSTALLATION
1. The 1,500 gallon septic tank/lift station must be constructed by 'a
certified tank manufacturer. Construction shall include an 18" manhole
to provide access to the lift station.
2. The septic tank shall be sufficiently bedded to prevent settling or
shifting of the tank.
3. All standpipes on the septic tank shall extend a minimum of 12 inches
above final grade.
4. Tanks installed without 4' of cover shall have a minimum of 2" of direct
burial insulation.

5. A foundation cleanout shall be installed one to four feet from the
building foundation. Two cleanouts are required between the tank and
the drainfield.
6. Final grading over the tankJiift station shall be such that a positive
slope exists away from the septic tank.
ABSORPTION BED CONSTRUCTION:
The absorption bed shall be constructed to the dimensions shown on
the design. The bottom of the bed shall be within 2" of level.
Distribution piping must be placed level with perforations down atop a
level bed of drainfield rock. Rock should then be placed over the pipe
to provide a minimum of 2" of cover.
A silt barrier or geotextile fabric must be placed between the drainfield
rock and the natural soil backfill.

Monitor tubes must be 4" in diameter and installed at the locations
shown on the design. The portion below ground must be perforated.
Contractor shall verify the septic tank and drainfield are a minimum
100' away from any private water wells in the area, 150' from a Class
"C" Well or 200' from any community well.
Direct bury insulation must be placed over the distribution system if
less than 3' of backfill depth is available. Finish grade over the trench
must be mounded to prevent settlement or depressions.

Grade area surrounding the absorption trenches to drain away.
A minimum 2' of accepting soil is required below the drainfield rock for
a 5' wide trench. Contractor shall verify this condition prior to
placement of the rock. Ail pockets of unacceptable materials must be
removed and replaced.
MATERIAL SPECIFICATIONS:
The lift station must be constructed by a Municipally approved septic
tank manufacturer. An Orenco 20 OSI 05 HHF-5 pump is
recommended. Anchorage Tank is a recommended supplier for the
materials required for this project.
The following pipe materials are approved for use in septic system
installations in the Municipality of Anchorage:
Cast Iron (perforated and solid) ASTM D3034 or P.V.C. (perforated
and solid) ASTM F810 or H.D.P.E. (perforated, but not solid) and
ASTM D2662 or A.B.S. (perforated and so id).
2. Insulation shall be at least 2" thick extruded direct burial polystyrene
(Dow Chemical Co. Styrofoam HI or equal).

3. Septic tank inlets and outlets shall be fitted with watertight couplings
(Caulder, Femco, or equal).
4. A permeable geotextile fabric (Typar, Mirafi or equal) must be installed
between the final drain rock layer and the native soil layer,
5. All drain reck shall be .5" to 2.5" in diameter with less than 3% passing
the #200 sieve.
EQUIPMENT SPECIFICATIONS:
Air Supply Line shall be a Wasteflow Emitterline, %" inside diameter
from Anchorage Tank. Line shall be insulated with ~" foam pipe wrap
inside a 2" Schedule 40 PVC Jacket buried a minimum 3' below the
surface,
Air Compressor shall be a Thomas Industries Model 5070 as supplied
by Anchorage Tank. Compressor shall be located in the garage or
crawlspace.
A pressure gauge calibrated from 0 to 10 PSI shall be furnished and
installed in line with the air line. The gauge shall be mounted in an
area which provides easy viewing and is seen on a regular basis such
as the entrance to the home.
Material for Filter Sand must meet the following gradation:
50% or More Passing the No. 4 Sieve
2% or Less Passing the No. 100 Sieve
1% or Less Passing the No. 200 Sieve
INSPECTIONS:
A minim, urn of two inspections are required by Municipal Ordinance.
These inspections must be conducted under the supervision of a
prefessional engineer registered in the State of Alaska. The first
inspection must be conducted after the excavation of trenches, beds or
pits and before the installation of any gravel. A septic tank may be set in
place, but may not be backfilled.
The second inspection must be conducted after the placement of the
..geotextile fabric, gravel, distribution piping, standpipes, cleanouts and
insulation. No backfill should be in place at the time of inspection.
Contractor shall provide a copy of all field survey layout and construction
notes for use in preparing the certified as-built of the completed system.

Impacts to Adjacent Properties
Dear Onsite Services Engineer:.
The owner of Lot 8, Block 4, Mountainside Village Subdivision intends to place a new 4-
bedroom home on the lot. We are therefore applying for a permit to construct a new
well and sept c system on the lot to serve the proposed home. The attached Site Plan
and backup documentation identify the location a.nd configuration of the new well and
sept c system and the parameters used in the design. It also shows the location of the
test holes and the alternate septic site. Existing drainage patterns are shown and w
not be altered by the development of the lot.
Two test holes were placed at the prop..o, sed location for the new and alternate
absorption beds and revealed gravel w~th varying amounts of silt up to 25%.
Groundwater was noted in the test holes during excavation at 8' and 9' and remained at
those eve s durin~ the monitoring period. The soil percolated at a rate of 24 minutes
per inch. We are~herefore proposing !o place 2 absorption beds each 50' long by 15'
wide to treat the septic effluent. The p pe will be placed at 3 below the existing ground
surface and the total depth of the beds will be 3.5'. Some coarse sand may be required
to level the beds. A minimum of 3' of cover will be placed over the bed to prevent frost
penetration and possible freezing. A Zabel flow splitter will be placed to insure even
flow from the septic tank to the beds.
The ground surface on the lot slopes as shown on the attached Site Plan with 2% to 5%
grades from east to west near the northern portion of the lot. The slope becomes flatter
proceeding west and slopes to, the stream flowing through the property at the west
property line. The new beds w~ll be, constructed parallel t.o. the contours of the surface
~n conformance with Municipal requirements. Grading w~ll be accomplished to assure
su. rface drainage is aw. ay from the new beds. The existing drainage pattern on the lot
w~ll not be affected dunng development.

ON-SITE WASTEWATER DISPOSAL SYSTEM
CONSTRUCTION AND MATERIAL SPECIFICATIONS
SUBJECT: LOT 8, BLOCK 4, MOUNTAINSIDE VILLAGE SUBDIVISION
GENERAL:
The scope of this project includes the procurement and placement of a
new 1,250 gallon septic tank at the location shown on the site plan.
Work also includes the construction of 2 new 50' long X 15' wide
absorption beds at the location shown. The distribution line in the
beds will be placed at 3' below the existing ground surface. Coarse
sand may be required to level the beds. In addition, a Zabel Flow
Splitter Valve must be placed to insure even flow from the septic tank
to each bed. Total depth of the beds will be 3.5' below the existing
ground surface. All components of the septic system must be placed a
minimum of 100' from the well on the lot.
Construction shall be in accordance with the approved site plan,
design drawings, Municipal Permit with any special provisions or
conditions, and all applicable State and Municipal Wastewater
Disposal Regulations.
The Contractor shall be responsible for obtaining all underground utility
locates and for the layout of the septic system and verification of the
location of all lot lines.
Unless specifically agreed otherwise, the contractor shall be
responsible for final grading areas subsequently depressed from soil
settling. Property owner shall be responsible for revegetation of
affected areas unless specifically agreed otherwise.
Contractors installing wastewater disposal systems must be certified
by the Municipal Department of Health and Human Services for
system installations. Owners installing their own systems must receive
prior approval from D.H.H.S. before beginning system installation.
SEPTIC TANK INSTALLATION
1. A new 1,250 gallon septic tank must be be procured from an approved
source and installed at the location shown.
A septic tank is to be constructed by a certified septic tank
manufacturer. Construction shall include two 4" cleanouts for pumping
access.
The septic tank shall be sufficiently bedded to prevent settling or
shifting of the tank.
All standpipes on the septic tank shall extend a minimum of 12 inches
above final grade.
Tanks installed without 4' of cover shall have a minimum of 2" of direct
burial insulation.

6. A foundation cleanout shall be installed one to four feet from the
building foundation. Two cleanouts are required between the tank and
the drainfield.
7, Final grading over the tank shall be such that a positive slope exists
away from the septic tank.
DRAINFIELD CONSTRUCTION:
The drainfield shall be constructed to the dimensions shown on the
design. The bottom of the bed shall be within 2" of level.
Distribution piping must be placed level with perforations down ato.p a
level bed of drainfield rock. Rock should then be placed over the p~pe
to provide a minimum of 2" of cover.
A silt barrier or geotextile fabric must be placed between the drainfleld
rock and the natural soil backfill.
Monitor tubes must be 4" in diameter and installed at the locations
shown on the design. The portion below ground must be perforated,
Contractor shall vedfy the septic tank and drainfield are a minimum
100' away from any private water wells in the area, 150' from a Class
"C" Well or 200' from any community well.
Direct bury insulation must be placed over the distribution system if
less than 3' of backfill depth is available. Finish grade over the trench
must be mounded to prevent settlement or depressions.
Grade area surrounding the absorption beds to drain away.
A minimum 2' of accepting soil is required below the drainfield rock for
a 5' wide trench. Contractor shall verify this condition prior to
placement of the rock. All pockets of unacceptable materials must be
removed and replaced.
MATERIAL SPECIFICATIONS:
1. Septic tanks must be constructed by a municipally approved septic
tank manufacturer.
2. The following pipe materials are approved for use in septic system
installations in the Municipality of Anchorage:
Cast Iron (perforated and solid) ASTM D3034 or P.V.C. (perforated
and solid), ASTM F810 or H.D.P.E. (perforated, but not solid) and
ASTM D2662 or A.B.S. (perforated and solid).
3. Insulation shall be at least 2" thick extruded direct burial polystyrene
(Dow Chemical Co. Styrofoam HI or equal).
4. Septic tank inlets and outlets shall be fitted with watertight couplings
(Caulder, Femco, or equal).

5. A permeable geotextile fabric (Typar, Mirafi or equal) must be installed
between the final drain rock layer and the native soil layer,
6. All drain rock shall be .5" to 2.5" in diameter with less than 3% passing
the #200 sieve.
INSPECTIONS:
Municipal Ordinance requires a minimum of two inspections. These
inspections must be conducted under the supervision of a professional
engineer registered in the State of Alaska. The first inspection must be
conducted after the excavation of trenches, beds or pits and before the
installation of any gravel. A septic tank may be set in place, but may not
be backfilled.
The second inspection must be conducted after the placement of the
geotextile fabric, gravel, distribution piping, standpipes, cleanouts and
insulation. No backfill should be in place at the time of inspection.
Contractor shall provide a copy of all field survey layout and construction
notes for use in preparing the certified as-built of the completed system.

The Municipality of Anchorage Development Services Depadment (DSD) Issues Cedificates of Health Authority
Approval (HAA) based only upon the representations given in paragraph 5 by an independent professional civil
engineer registered in the State of Alaska. Certificates of Health Authority Approval are required for the transfer of
title (except be[ween spouses) for properties served by a sing!e fatuity on-site wastewater dispccal and/ct water
supply system. DSD also issues HAAs upon request to homeowners. Certificates of Health Authority Approval are
valid for 90 days from the date of issue for properties served by a pdvate or Class C well and may be reissued with
new water sample results less than 30 days old. (Certificates may be reissued for a period of up to one year with
valid water samples.) Certificates are valid for one year for properties served by Class A or B welIs or a public
