Dear John & Cathy Weaver:
An On-Site Water/Wastewater Permit, number SW000002, issued by this office for a
single-family system, expired on January 6, 2001. This permit was valid for 365 calendar
days.
If this was a well permit and the well has been drilled, a well log must be sent to this
office for documentation of the installation and to close the permit.
If this permit was for a wastewater disposal system, an original as-built inspection report
must be sent to this office for review, approval and documentation. This as-built
inspection report must be signed by the licensed Professional Engineer who inspected the
installation of the system. As-built inspection reports are required to be submitted within
30 days ofthe completion ofthe system.
If no system was installed under this permit, and you are still planning to install a well or
wastewater disposal system, a new permit must be obtained from this office. When
applying for a new permit, the fees are: $320.00 for a wastewater permit and $120.00 for
a well permit.

Ret': Septic System Upgrade for Lot 9, Block 2, Trails End Subdivision
(Bottomless Intermittent Sand Filter - ISF)
To whom it may concern:
The existing 4 bedroom house is served by a private well and septic system. The existing septic
system consists of a 1250 gallon septic tank and a bed type drainfield. The existing bed is
surcharged and must be upgraded. Comments regarding the proposed upgrade are summarized as
follows:
1. GENERAL: Several test holes were excavated on the south half of the property to determine
an area suitable for a septic system upgrade. Due to the high groundwater in the area, only one
test hole was found to be suitable for an on-site septic system. Given the limited area around the
test hole and the setbacks from the existing septic system, we are proposing that a 18' by 20'
Bottomless Intermittent Sand Filter be installed.
2. SOILS: On November 1, 1999, two test hole were excavated and groundwater monitoring
tubes installed. On November 16, 1999, t~vo additional test holes were excavated and
groundwater monitoring tubes installed. As can be seen on the attached soil logs, TH#2, TH#3,
and TH#4 were found to be unsuitable for a on-site septic system. We are proposing that a
bottomless I.S.F. system be installed within the 30 foot radius of TH#1. In TH#l, the soils below
the organics is a SP/ML material to a depth of 4.5 feet and than transition to a SP material to a
depth of 12.5 feet {bottom of test hole). Ground water was encountered at a depth of 6 feet at the
time of excavation and ten days later, water was found to be at 4'9"+/-. A percolation test was
performed between 2.0 feet to 2.5 feet, and determined the rate to be at <1 minute/inch.
· 3. DRAINFIELD DESIGN: Bottomless Intermittent Sand Filter (ISF)
a. Percolation Rate: <1 minutes/inch
b. Allowable Application Rate for ISF: 2 gallons/day/fi2
c. Number of Bedrooms: 4
d. Design Flow: 600 gallons per day
e. Minimum Absorption Area: 300 ft2
f. Effective Depth below pressure pipes: 2.5+ inches
g. Width: 18 feet
h. Length: 20 feet.
i. Effective absorption area = 360 tt2
j. Air Supply: Thomas Industries, Model 5070, "Anchorage Tank".
k. Air Supply Line: "Wasteflow" emitterline, 1/2 inch I.D, "Anchorage Tank".
1. Sand Material: In accordance with M.O.A. latest standards
m. Pea Gravel: 100% passing 3/8" sieve, less than 20% passing the 1/4" sieve, and
less than 1% passing the #8 sieve.
We are proposing to excavate down to a depth of 2.5 feet (maximum - remove all organics),
place a minimum of 6 inches of sand, install the air supply line, and cover it with 1.5 feet of
sand. On top of the sand, we will place 6 inches of 3/8 inch pea gravel, with the pressure laterals
midway in the layer. We will use a conventional lift station {Anchorage Tank), equipped with a
programmable timer so that flow can be intermittently dosed to the 1SF.
4. SURFACE WATERS: There is no surface water within a 100 foot of the the proposed
septic system upgrade.
$. TOPOGRAPtlY: As can be seen on the attached topography site plan, there are no slope
concerns.
6. MATERIALS AND CONSTRUCTION PRACTICES: The materials used, and the
construction practices will comply with DHHS' "Intermittent Sand Filter Design, Installation &
Maintenance Manual". The contractor should read this document prior to construction. Copies
are available at the Municipal Onsite Services office (Sth floor, 9th & L St.).
7. CLOSING: I am open to any suggestions from your department, which would be an
improvement to the proposed design. I am unaware of any adverse impacts this installation
would have on adjacent wells or septic systems. If you have any questions, please contact me at
