1. From the 2002 water sample, total coliform was 0, and total nitrates was 1.53 mg/L. A new
tank was installed in 2005.
2. Any overflowing sewage would have to travel around the house to get to the well head.
3. The location of the system is in a visible area and any surfacing effluent will be seen and
corrected.

Dear Jeff,
Per your request, we have re -verified the separation distance of 98 feet between the first
septic tank cleanout and the well located on the referenced lot. We are therefore
requesting that a waiver to 94 feet between the referenced well and septic tank be granted
at this time. This well and septic system are located on the opposite sides of the house
and have been in operation for over 37 years.
Per ADEC Separation Distance Waiver Guidelines for SCRO:
• Water Table (202' static well log)
• Soil Sorption (See well log-clay/sand/gravel)

The point total indicates that the well would almost sure to be free or no contamination
possible from any form of household sewage contamination. It appears that the moderate
nitrate levels (1.53 mg/L) of the most recent water analysis of March 26, 2002 are not just
isolated to the referenced well, but to the local area in general.

Gentlemen:
The owner has requested we proceed forward to obtain a septic permit to upgrade and
replace the existing septic tank for the subject lot. The existing septic tank will be
abandoned in place or removed in accordance with municipal standards.
There are no public or private wells within 200' of our proposed system location except
as noted. There is no surface water within 100' of the proposed system and there are no
known curtain drains within 50'. We do not expect there to be any adverse effect on
adjacent lots by the development of this system.

Approval (HAA) based only upon the representations given in paragraph 4 by an Independent professional
civil engineer registered in the State of Alaska. Certificates of Health Authority Approval are required for the
transfer of title (except between spouses) for properties served by a single-family on-site wastewater disposal
and/or water supply system. DSD also issues HAAs upon request to homeowners. Certificates of Health
Authority Approval are valid for 90 days from the date of issue for properties served by a private or Class C well
and may be reissued with new water sample results. (Certificates may be reissued for a period of up to one
year with valid water samples.) Certificates are valid for one year for properties served by Class A or B wells or
a public water system. The Municipality of Anchorage is not responsible for errors or omissions in the
professional engineer's work.

We are submitting a request for waivers from the separation
distances stated in Title 18, Alaska Administrative Code,
Chapter 80.020.
During an Health Authority Inspection on May 24, 1988 is was
discovered that the distance between the well on lot 6 and the
absorption field on lot 7, Block 3, was 95 feet, five feet less
than the required 100 ft.
The well was installed in 1971 and the septic system in 1972.
Therefore the well was in compliance at the time of
installation, but the septic system on lot 7 was in violation
of the requirements when it was installed.
The well is located up hill from the septic system. According
to the well log the well is 224 feet deep with static water
level at 202. Three clay layers were identified with a total
thickness of 172 feet.
By going through the DEC's procedure of analyzing the potential
for contamination I arrive at the following points:

DATE OF INSPECTION: May 24, 1988
TEST PROCEDURE: Well was pumped at a constant rate while the
drawdown was monitored with an acoustic probe. At the beginning
of the test water level was found at 199 feet below top of casing.
At a pumping rate of 6.5 gallons per minute the water level dropped
to 202 feet after 5 minutes of pumping and remained at that
level for the remainder of the test, 70 minutes. A total of 460
gallons were pumped. The well recovery was instantaneous .
TEST FOR E.COLI AND TOTAL NITROGEN: Water was tested for E.Coli
and total nitrates on May 25, 1988
E.Coli 0. Total Nitrates 0.65 mg/l.
Max. allowable Total Nitrates 10mg/1.
TEST RESULTS: This well meets the requirements of the
Municipality of Anchorage.
THIS WELL WILL PRODUCE MORE THAN 3 GALLONS PER MINUTE FOR MORE
THAN FOUR HOURS
The Municipal requirement for well flow is 150 gallons of water
per bedroom per day. This well exceed this requirement. The
assessment of the condition of the well applies only to the
conditions as of the day tested. The flow rate may change due to
subsurface conditions that may not be observed from the surface,
and changes in the land use and other factors that may impact the
aquifer feeding the well.

TEST PROCEDURE:
System was inspected and measured. Tank was
found with 3
feet of cover and a liquid depth of 49 inches.
Crib was 12 feet
deep with 91 inches of liquid.
460 gallons of clean water were added to the crib while the
water levels in the tank and the crib were monitored. Tank level
did not change. Water level in crib came up 17 inches. The
infiltration rate was monitored for 30 minutes. During this time
the waterlevel in the crib dropped 5 inches, indicating a rapid
infiltration.
TEST RESULT: This system meets the code requirements of
the Health and Social Services Department of the Municipality of
Anchorage.
NOTE The operational life of all septic systems depends on the
local soil conditions, groundwater levels that may fluctuate
during the year, and the water usage of the family being served
by the system. These conditions are outside the control of the
evaluator of this septic system. We can therefore not give any
estimate of how long this system will function satisfactory for
current or future occupants.
