The Municipality of Anchorage Development Services Department (DSD) Issues Certificates of Health Authority
Approval (I IAA) based only upon the representations given In paragraph 5 by an Independent professional civil
engineer registered in the State of Alaska. Certificates of Health Authority Approval are required for the transfer
of flue (except between spouses) for properties served by a single family on-site wastewater disposal and/or
water supply system. DSD also issues HAAs upon request to homeowners. Ce~ficates of Health Authority
Approval are valid for 90 days from the date of issue for properties served by a private or Class C well and may
be reissued with new water sample results less than 30 days old. (Certificates may be reissued for a period of
up to one year with valid water samples.) Certificates are valid for one year for properties served by Class A or B
wells or a public water'system. The Municipality of Anchorage Is not responsible for errors or omissions In the
professional engineer's work.

Reft Waiver Request and Health Authority Approval for
Zantha Vista Subdivision Lot 2 Block B
The existing 4 bedroom house is served by a private well and septic system. We request you
grant a 95 feet separation distance waiver from the well on the referenced property to the septic
tank on the referenced property. The septic system was installed in October 1969, and the well
that currently serves the property was drilled on December 23, 1979.
The following items are justification for the waiver:
· The lot slopes gently from the east to the west, with the well on the uphill side, as shown on
the attached topographical map. If the septic tank were to overflow, the effluent ~
travel toward the well head.
,, The location of the septic system is in a very visible area so that if any effluent were to
surface, it would be noticed and the problem corrected.
· Any potential contamination by subsurface migration of wastewater from a leaking tank
would be mitigated by silty soils above the aquifer. As can be seen on the attached well log,
the aquifer lies below 49 ft of ALVM that would inhibit the migration of untreated
wastewater into the aquifer. Other well logs from surrounding properties show a mix of silt
and gravel.
· Recent water sample results indicated a nitrate level of 4.9 mg/l and no coliform bacteria.
These results are satisfactory, although the nitrate is somewhat high. The nitrate seems to be
naturally occurring, as other wells in the area have shown similar nitrate levels.
Based upon the aforementioned facts, it appears that there is minimal risk associated with the 95
feet separation distance waiver.

Reft Septic System Upgrade for Lot 2, Block B, Zantha Vista Subdivision
To whom it may concern:
The proposed 4 bedroom house is served by a private well and septic system. The existing septic
system consist of a 1250 gallon concrete septic tank and a deep trench type drainfield. The
existing drainfield will not pass an adequacy test and must be upgraded. A test hole was
excavated south of the existing septic system in the area of the proposed septic upgrade. The
proposed septic system will be designed around the 30 foot radius of this test hole. We are
proposing that the existing septic tank be excavated and the integrity be verified. If the tank is of
poor integrity, we propose that a new 1250 gallon septic tank be installed outside the 100 foot
well radius and also a dual deep trench type drainfield be installed. Comments regarding the
proposed design are summarized as follows:
1. SOILS: See the attached log which shows the soil classifications, groundwater monitoring,
and the percolation test results. It is our opinion that due to the overall appearance of the soils, an
application rate of 0.6 gallons/day/112 should be used.

3. SURFACE WATERS: There are no surface waters within 100 feet of the proposed
upgrade.
4. TOPOGRAPIIY: As can be seen on the attached topography site plan, the average
topography of this property is a 5 to I0 percent running from approximately southeast to
northwest; in short, there are no slope concerns. The trenches are to be installed parallel to slope
contours.
I am unaware of any adverse impacts this installation would have on adjacent wells or septic
