We propose to install 2 deep trenches and a HDPE 1000-gallon tank to serve the proposed 2-bedroom ADU residence. The design is based on the attached test hole conducted in 2023. Groundwater was not observed during monitoring or excavation. The ADU will be served by the existing well located on the lot to the south as noted on the design. The slopes are moderate at 0-5% at the proposed upgrade location. The lot and area are served by private water. The design will not impact any of the neighboring properties. Box 196650 Anchorage, Alaska 99519 Subject: Lot 156, NE4, T12N, R3W, S33 Septic Permit Gentlemen: My fimx was contacted to investigate the possibility of installing of a ne~v system at the above referenced property. Currently the lot is undeveloped, but a private well was" drilled on the property approximately 25 years ago. There is no well log available for this well on file ~vith your office. A single test hole was excavated on May 10, 1995. The soil report and percolation test results are attached. Ground ~vater was Checked on May 21, 2000, and was found to be dry to 16.5 feet. No bedrock was encountered in the test hole. The lot is approximately 2.5 acres in size. Lot 156 slopes to the west at a rate of approximately 1 to 2 percent, and drops off steeply (approximately 10%) along the western side of the property. The proposed installation will be located in the north east comer of the lot on a flat area. The proposed location will be greater than 100 feet away from the existing well serving this property and 25 feet from the water service lines. The surrounding systems are located greater than 100 feet from the proposed installation. The lots surrounding this lot are developed, but do not conflict ~vith the proposed installation. The proposed installation will not impact the future development of the surrounding or existing lots. See attached design. The new system will maintain over six feet vertical separation to the bedrock and over four feet vertically to the ground water. The owner is an experienced excavator, and may install the system himself. He may install both the primary and reserve drain-fields at the time of installation. STATEMENT OF INSPECTION BY ENGINEER As certified by my seal affixed hereto and as of the validation date shown below, I verify that my investigation, based on procedures outlined in the Certificate of On -Site Systems Approval Guidelines for this application, shows that the on-site water supply and/or wastewater disposal system is (are) safe, functional and adequate for the number of bedrooms and type of structure Indicated herein. I further verify that based on the information obtained from the Municipality of Anchorage files and from my investigation and inspection, the on-site water supply and/or wastewater disposal system is (are) in compliance with all applicable Municipal and State codes, ordinances, and regulations in effect at the time of installation. In conducting an adequacy test, I attempt to provide a thorough, conscientious engineering analysis of the system in accordance with MoA COSA guidelines and regulations. The reported results describe the performance of the system under the conditions encountered at the time of the test, and separation distances measured to readily identifiable features. The operational life of all wells and septic systems depend on the local soil condition, ground water levels that may fluctuate during the year, and the water usage of the family being served by the system. These conditions are outside the control of the evaluator of this system. All systems eventually fail and satisfactory test results do not guarantee future performance of the system, nor do they guarantee that there are no hidden defects or encroachments. Therefore we cannot provide any warranty for future performance, nor can we estimate remaining life of the system. The content of this report is for the sole benefit of the owner listed above.
