Subject: Upgrade Sewer Permit - Mountainside Village Block 1, Lot 3
It has been determined that the existing absorption field of the subject property is
in failure. Subsequently, the owner has requested we proceed forward to obtain
a septic permit to upgrade the septic system.
The general slope of this lot is from east to west at a grade of approximately 5-
10% in the area of the septic system. On July 9, 2013 a testhole was performed to
investigate soils and groundwater. The results of this test are attached for your
review. The proposed upgrade will serve the existing 4 -bedroom house. We
propose to install Y -wide gravity trench with a diverter valve for the old leach
field. Groundwater was not encountered at the time of digging or at monitoring.
The property and adjacent lots are served by private wells and there are no wells
within 200' other than noted. There are no known curtain drains within 50'. We
do not expect there to be any adverse effect on adjacent lots by the development

Gentlemen:
We have completed percolation tests on Block 1, Lot 3;
Block 2, Lots 1, 2, 31 4; and Block 3, Lot 5 of Mountainside
Village :subdivision.
Test holes were drilled by Denali Drilling, Inc. using a
continuous flight auger mounted on a Nodwell carrier. Test
hole logging and percolation tests were performed by Mr. O.M.
!latch and Mr. T. Barber, Alaska Testlab geologists, and
Mr. M.J. Holum, DOWL engineer.
After the hole was drilled, a 3/4" slotted PVC pipe was in-
serted in the hole to maintain an open hole. The hole was
filled with water and left overnight to saturate: On the
following day, the hole was refilled with water. The water
level was recorded at 10 minute intervals for the next hour.
During the tests, the water levels were between 3 and 7 feet
below ground surface.
The minimum percolation rates were observed during the last
10 minute interval. We suggest that on-site waste disposal
systems be designed as follows:

Absorption trenches are suitable for all tested lots. The
recommended absorption rate per bedroom is based on criteria
in the "Manual of Septic Tank Practice", U.S. Department of
Health, Education, and Welfare.
A well in the Keno Hills Subdivision is -located 104 feet
from the test hole at Block 1, Lot 3. The drainage system
should not be placed within 100 feet of this well.
The water table was observed at 14 feet below ground surface
at Block 2, Lot 2. Drainage trenches should remain six feet' -
above this level.
We recommend the absorption trenches be inspected prior to
installing the system since the tests are based on a 6 inch
diameter test hole. Trench conditions will provide a more
extensive picture of subsurface conditions. If trench
conditions differ significantly from the test hole condi-
tions, the trench length may be changed to improve system
performance.

Nitrate Advisory
Certificate of On -Site Systems Approval # 131411
A Certificate of On -Site Systems Approval inspection and test of potable
water was recently conducted on the well water supply on Block 1, Lot 3 of
Mountainside Village subdivision. This inspection revealed a nitrate
concentration of 5.12 milligrams per liter (mg/L) was reported for the
property's well water sample. The Environmental Protection Agency (EPA)
has established a maximum contaminant level (MCL) of 10.0 mg/L for
public drinking water systems. While private wells are not subject to this
regulation, EPA standards are based on existing health information and can
therefore be used to gauge the relative quality of water from private wells.
Please see the attached "Nitrate Fact Sheet" for important information
regarding nitrate.
This advisory must be attached to all copies of the subject Certificate of On -
Site Systems Approval.

Residence within the Municipality of Anchorage (MOA).

Per your request, we inspected the well and septic system which serves the subject property for
the purpose of obtaining an MOA Certificate of Onsite Systems Approval (COSA). A site visit
was performed on 71512013. Our findings are summarized as follows:
WELL: According to the drilling log, the well on the property is 87 feet deep. At the beginning
of our test, the static water level was 22 feet below the top of casing (B.T.C.). We pumped 500
gallons from the well over a period of 90 minutes, with a maximum drawdown of 13 feet.
According to our test, the well produces at least 5.5 gallons per minute (GPM). This is more than
the minimum requirement for a 4 bedroom house in the M.D.A. (0.41 GPM)
WATER QUALM: Water samples were pulled on 7!5€"2013 and delivered to a state certified
laboratory for arsenic, nitrates and bacteria. When we receive the results, we will forward them
to you.
SEPTIC SYSTEM: The septic system serving the property consists of a 1000 gallon septic tank
and a trench type drainfield. Upon our inspection, the drainfield was completely surcharged and
could not be tested. In short, the drainfield is in a state of technical failure and must be upgraded
before the house can be sold. The following is a summary of costs associated with upgrading the
septic system:
.Services to date for the well flow test, water sampling septic evaluation and preparation
of this report: $850.00
.Excavation of test holes: *$600.00 for the first test hole, $300.00 for each additional test
hole. Only one test hole is anticipated.
-Soils analysis, percolation test and groundwater monitoring: $600.00 for the first test
hole, $300.00 for each additional test hole_ This is based upon the condition that all of
the test holes are evaluated on the sante day.
.Engineering services to design the septic system: $900.00*. This is based upon a
conventional gravity flow septic system with no waivers to property lines, surface waters,
or nearby wells.
