Permit Application
I have reviewed your comments and revised the septic system design for the subject lot
accordingly. We have moved the trench further northeast to provide additional distance
to the steep slopes at the end of the trench. We have also revised the effective depth
to 10' and shortened the length to 25' again to provide additional distance. Fil[will .s, till
be required at the southwest end of the trench to provide separation distance Trom tne
steep slope.
A cross section has been provided showing the slopes on both sides of the trench at
the mid point. The trench location will meet the separation distance requirement of 50'
from any downs ope grade greater than 25.% a.s sh.own .o..n the.c, ro.ss section. The only
questionable area is at the end of the trencn where T~II will De a(3oe(3.
The distance from the house and property line to the absorption trench has been added
to the drawing. Finally, the permit application has been changed to reflect "Sewer
Upgrade."
Please let me know if additional information is required before the construction permit
can be issued.

Impacts to Adjacent Properties
Dear Onsite Services Engineer:.
The exist ng septic system on Lot 4, Block 1, Golden Hills Subdi,vision.has failed ..an.d
must be replaced as soon as possible. We are therefore applying mr a permt to
construct a new septic system on the lot to serve the four-bedroom home. The
attached Site Plan and backup documentation identify the location and configuration of
the new septic system and the parameters used in the design. Also identified on the
plans is the location of the existing well an.d septic system along with the test hole.
Existing dra. inage patterns are shown and w~ll not be altered by the development of the
lot. The exIsting absorption trench will be tied into the system for future use with a Bull
Run Flow Diverter Valve.
A test holes was placed near the location of the new absorption trench. This hole
indicated a well graded sand with some gravel. A percolation test in the material
resulted in a rate approximating 1 minute per inch. No groundwater was found during
tPhlacement of the test hole and none developed during the monitoring period. We are
erefore ropos ng to p ace a deep absorption trenc.,h, withlO'of gravel beneath the
distributionp pipe. The total depth of the trench w~ll be approxima, tely12'. The
distribution pipe will be placed at 2' below the ground surface. The servIce line from the
septic tank to the absorption trench will pass along the alignment of the existing
absorption trench to avoid the newly paved driveway serving the house.
The ground surface on the lot slopes as shown on the attached Site Plan with steep
grades|ust northwest of the new trench. The new absorption trench will be constructed
on a t'aifly flat surface near the top of the lot in conformance with Municipal
requirements and parallel to ground contours where possible.
If the system is constructed in accordance with our design the following statements
apply:
The system, if constructed as designed, will have no adverse impact on the wells
in the area or those to be constructed in the future.

ON-SITE WASTEWATER DISPOSAL SYSTEM
CONSTRUCTION AND MATERIAL SPECIFICATIONS
SUBJECT: LOT 4, BLOCK 1, GOLDEN HILLS SUBDIVISION
GENERAL:
1. The scope of this project includes the pumping, crushing and.
abandoning of the existing septic tank on the lot and the procurement
and placement of a new 1,250-gallon septic tank at the location shown
on the site plan. Work also includes the construction of a new 25'
long X 3' wide XlO,'~ffective depth absorption trench at the location
shown. The distribution line in the trench will be placed at 2' below the
existing ground surface. Total depth of the trench will be 10' below the
existing ground surface. A Bull Run Diverter Valve will be required
and the existing absorption trench tied in to the new system. The new
line from the septic tank to the new trench will run along the same
alignment as the existing trench.
2. Construction sha be n. accordance with the approved site plan,
design draw ngs, Municipal Permit with any special provisions or
conditions, and all applicable State and Muncipal Wastewater
Disposal Regulations.
3. The Contractor shall be responsible for obtaining all underground utility
locates and for the layout of the septic system and verification of the
location of all lot lines.
4. Unless specifically agreed otherwise, the contractor shall be
respons ble for final grading areas subseque.n, tly depressed from soil
settling. Property owner shall be respons~be for revegetation of
affected areas unless specifically agreed otherwise.
5. Contractors installing wastewater disposal systems must be certified
by the Municipal Department of Health and Human Services for
system installat ons Owners installing their own syste.ms mu.st receive
p~ior approval from D.H.H.S. before beginning system instal abon.
SEPTIC TANK INSTALLATION
1. A new 1,250-gallon septic tank must be procured from an approved
source and installed at the location shown.
2. A septic tank is to be constructed by a certified septic tank
manufacturer. Construction shall include two 4" cleanouts for pumping
access.
3. The septic tank shall be sufficiently bedded to prevent settling or
shifting of the tank.
4. All standpipes on the septic tank shall extend a minimum of 12 inches
above final grade.
5. Tanks installed without 4' of cover shall have a minimum of 2" of direct
burial insulation.

6. A foundation cleanout shall be installed one to four feet from the
building foundation. Two cleanouts are required between the tank and
the drainfield.
7. Final grading over the tank shall be such that a positive slope exists
away from the septic tank.
DRAINFIELD CONSTRUCTION:
1. The drainfield shall be constructed to the dimensions shown on the
design. The bottom of the trench shall be within 2" of level.
2. Distribution piping must be p aced level with perforations down atop a
level bed of drainfleld rock. Rock should then be placed over the p pe
to provide a minimum of 2" of cover.
3. A silt barrier or geotextile fabric must be placed between the drainfield
reck and the natural soil backfill.
4. Monitor tubes must be 4" in diameter and installed at the locations
shown on the design. The portion below greund must be perforated.
5. Contractor shall verify the septic tank and drainfield are a minimum
100' away from any private water wells in the area, 150' from a Class
"C" Well or 200' from any community well.
6. Direct bury insulation must be placed over the distribution system if
less than 3' of backfill depth is available. Finish grade over the trench
must be mounded to prevent settlement or depressions.
7. Grade area surrounding the absorption trenches to drain away.
8. A minimum 2' of accepting soil is required below the drainfleld reck for
a 5' wide trench. Contractor shall vedfy this condition pdor to
placement of the reck. All pockets of unacceptable materials must be
removed and replaced.
MATERIAL SPECIFICATIONS:
1. Septic tanks must be constructed by a municipally approved septic
tank manufacturer.
2. The following pipe materials are approved.for use in septic system
installations in the Municipality of Anchorage.
Cast Iron (perforated and solid), ASTM D3034 or P.V.C. (perforated
and solid), ASTM F810 or H.D.P.E. (perforated, but not solid) and
ASTM D2662 or A.B.S. (perforated and solid).
3. Insulation shall be at least 2" thick extruded direct burial polystyrene
(Dow Chemical Co. Styrofoam HI or equal).
4. Septic tank inlets and outlets shall be fitted with watertight couplings
(Caulder, Femco, or equal).

5. A permeab e geotextile fabric (Typar Mirafi or equal) must be installed
between the final drain rock layer and the native so ayer,
6. All drain rock shall be .5" to 2.5" in diameter with less than 3% passing
the #200 sieve.
INSPECTIONS:
Municipal Ordinance requires a minimum of two inspections. These
inspections must be conducted under the supervision of a professional
engineer registered in the State of Alaska. The first inspection must be
conducted after the excavation of trenches, beds or pits and before the
installation of any gravel. A septic tank may be set in place, but may not
be backfilled.
The second inspection must be conducted after the placement of the
geotextile fabdc, gravel, distribution piping, standpipes, cleanouts and
insulation. No backfill should be in place at the time of inspection.
Contractor shall provide a copy of all field survey layout and construction
notes for use in preparing the certified as-built of the completed system.

The absorption trench for Lot 4, Block 1, Golden Hills Subdivision was placed at the
only suitable location on the lot. Excess excavation from the trench was placed at the
west end of the trench to provide as much flattening as possible. Our calculations
indicate the ma|odty of the trench is at least 50' from any. break in grade great..e[~t,han
25%. The end'of tee trench is at least 35' from any break in grade greater than
Soils encountered in the excavation for the trench were clean well graded sands with
some cobbles. Effluent from the septic tank will percolate through this material at a
fairly rapid rate in a vertical direction. Little, if any, effluent is expected to travel
horizontally through the porous material. The chances of effluent surfacing downslope
from the trench are extremely remote. We therefore request the absorption trench be
approved in its current configuration.

The Muncipality of Anchorage Department of Health and Environmental Protection (DHEP) issues Health Authority
Approval certificates based solely upon the representations given in paragraph 5 above by an independent professional
engineer registered in the State of Alaska. The DHEP does this as a courtesy to purchasers of homes and their lending
institutions in order to satisfy certain federal and state requirements. Employees of DHEP do not conduct inspections or
analyze data before a certificate is issued. The Municipality of Anchorage is not responsible for errors or omissions in the
professional engineer's work.
