Septic System Upgrade Permit
Design Narrative
The owner of the subject property is in the process of selling the property and has agreed to the buyer's
request for the drainfield to be upgraded prior to the sale. The existing drainfield was installed in 2002
by South Fork Construction using a design by KND Engineering, Inc. (currently ArcTerra Engineering).
The 2002 design included a replacement drainfield. During construction,the locations for the primary
and replacement drainfields were swapped. The 2002 inspection report was approved by Onsite 9-10-
2003.
After doing a site visit to confirm the possibility of using the 2002 replacement drainfield design,and per
a conversation later the same day with Deb Wockenfuss on 6-8-17,Crewdson Engineering, LLC(CE)
proposes to use the permitted 2002 design for construction of the replacement drainfield. CE is
providing a copy of that design with this submittal for your review. CE will be doing the construction
inspection and providing new record drawings under CE's seal with the final inspection report.
Since the project involves a COSA, an new asbuilt survey will be completed after construction.

Water Well Advisory
Certificate of On-Site Systems Approval (COSA) # OSC 171431
During a recent COSA on-site inspection and test of the potable water
supply well on Block , Lot 15 of T15N R1W Sec 8 subdivision, the
well's productivity was determined to be .08 gallons per minute. The
minimum well productivity required by this Department (AMC 15.55) for a
3-bedroom residence is .31 gallons per minute. Restriction of non-critical
water uses such as washing cars and watering lawns and gardens may be
required.
This advisory must be attached to all copies of the subject Certificate of On-
Site Systems Approval.
