applies only to the conditions as of the day tested. The flow and absorption rates may change
due to subsurface conditions that may not be observed from the surface, changes in land use,
local soil characteristics, groundwater levels that may fluctuate during the year, quality of
construction (workmanship & materials), the water usage of the family being served by the

wastewater disposal system is (are) safe, functional and adequate for the number of bedrooms and type of structure indicated
herein. I further verify that based on the information obtained from the Municipality of Anchorage files and from my investigation and
inspection, the on-site water supply and/or wastewater disposal system is (are) in compliance with all applicable Municipal and State
codes, ordinances, and regulations in effect at the time of installation.
In conducting an adequacy test, I attempt to provide a thorough, conscientious engineering analysis of the system in accordance with MoA
COSA guidelines and regulations. The reported results describe the performance of the system under the conditions encountered at the time
of the test, and separation distances measured to readily identifiable features. The operational life of all wells and septic systems depend on
the local soil condition, ground water levels that may fluctuate during the year, and the water usage of the family being served by the system.
These conditions are outside the control of the evaluator of this system. All systems eventually fail and satisfactory test results do not
guarantee future performance of the system, nor do they guarantee that there are no hidden defects or encroachments. Therefore we cannot
provide any warranty for future performance, nor can we estimate remaining life of the system. The content of this report is for the sole
benefit of the owner listed above. Reliance on this report by another person is at their own risk. Pannone Engineering Services LLC highly
recommends buyers hire their own engineer to evaluate this report.

Gatos Property
July 21, 1981: Meeting and Findings between Mr. Gatos
and the Department of Health and Environmental Protection
The Gatos property may or may not have been approved when Mr. Gatos
was purchasing the property. Records from a satillite office are
incomplete on this property. In any case, the current inadequate
seepage pit would have to be upgraded for resale approval.
The upgrade project(a new absorption trench) undertaken by Mr. Gatos.:
was found to be too close to a public well, which means Mr. Gatos was
installing the trench in public roadway per registered surveys on
record.
The lot apparently has no room for a new absorption trench without
encroaching on Mr. Gatos well and his neighbor's well.
Mr. Gatos in 1978 filed a claim against the Municipality of Anchorage.
I referenced Mr. Gatos to Risk Management since he stated he was

Dear Mr. Kelley:
Your inquiry regarding the subject property has been reviewed
and the following items noted:
(1)
The public well you mentioned serves Colonial Park Subdivision.
Actually there are two(2) wells within ten(10) feet of each
other now due to adequacy problems. Attached is a survey
showing protective radius for the original well. At no
point is the radius overlapping Lot 61. The second well
is ten(10) feet to the west of the original well, which
according to the survey scale, places its radius in
Hillcrest Lane and not Lot 61.
(2)
The upgrade of the sewer and water facilities on Lot 61 is
limited to a holding tank due to the well location in the
middle of the lot. A typical on-site septic/absorption
system possibly could be installed if the well were
relocated to facilitate 100'feet separation-between the
sewer and the well head. This type of upgrade would
require a well permit and a current sewer installation permit.
Additionally, the sewer must remain 100 feet from neighboring
wells.
The original cesspool on Lot 61 was tested for adequacy and
failed. From surveys it appears the problem is not one of
the public well radius but Lot 61's own well radius being
inadequate.

This department initially issued a permit for an upgrade.
Evidently code requirements can not be met. The subject
of the upgrade falling within the Colonial Park public well
radius appears to be an error of the excavator or owner
attempting to install the system in the IIillcrest ro_~_~y
from engineering survey data. This data appears on a
recorded plat stamped by a professional surveyor.

Health Certificate
Dear Mr. Strickland:
Mr. Gatos is the owner of Lot 61, Tract A, EAGLE CREST SUB-
DIVISION. He desires to obtain a health certificate from
the Municipality of Anchorage. The Municipality has issued
health certificates on this Lot in the past.
It is my understanding that a certificate will not be issued
because Mr. Gatos' septic tank is within 200 feet of a com-
munity well, which was drilled after Mr. Gatos purchased the
property. The Gatos water and sewer facilities are not
causing any health hazard.
I also understand that a conditional appraisal of the sewer
and water facilities would be granted if Mr. Gatos installed
a 2,000 gallon holding tank, which must be placed 50 feet
from any individual well and 200 feet from any community well.
This solution is not practical considering the size of the
lot an--d-l-6-~A~ion of sewer and water facilities on adjacent
properties and the relative cost of continuous pumping.
The undersigned hereby requests that a health certificate be
issued by ~o2~-~9 on the aforementioned property.
If a health certificate will not be granted, specify in writing
whether or not the facilities can be upgraded and the conditions
of the upgrade permit.

(2) Their is no record of a permit issued for the drilling of
this well.
(3) Their is no ASABUILT survey.
(4) Their has been no health certificate issued.
Until this TTell situation is resolved, I will be unable to
sell my h~me. I feel I have Grandfather rights, as my septic system,
has been in for 15 years. Mr. Jack Webb, who is ~?ith the Porough
Risk Management, is looking into this matter.

and wrote a letter updating the upgrade requirements. -The

The leaching field is to close to the community well
across the street and also to close to the neighbor's

it is not possible to permit the existing leaching
area to remain where it is at or to relocate it.
The septic'tank is of no importance because it must be
used in conjunction with the leaching area.

I have recently been provided a'c0py of your
letter of May 12, 1978 subject as above, an enclosure is

I represent ~. & Mrs. Robert Hart who are
presently involved in an attempt to purchase the above-
described property. It is my understanding from a letter
of representation by the present owner and by your letter
that at this juncture the Municipality of Anchorage is
unwilling to certify the condition of the property because
of the sewer and water deficiencies you have discovered
and reported.
At present it remains the intention of both the
seller and my clients that the sale should conclude if at
all possible. In that regard I would appreciate your
advice if there should exist any possibility that other
corrective action might be taken. Of course I would also
greatly appreciate your advice regarding the future time
table for public sewer and water facilities if that is
known to you.
I am advised that in the past few years the
property has been sold at least twice previously. I look
