INSPECTION The above referenced lot was permitted on 10/16/1995 permit #S W950342 for 4 bedrooms and reissued on 9/30/1997 permit #SW970339 with a design by S & S Engineering for 4 bedrooms. A test hole was excavated and percolation test performed. The approximate location of the test hole is located on the ENGINEERING STUDIES attached site plan. ANDREPORTS At the time of excavation no water was encountered in the test hole and after seven day ground water monitoring, the monitoring tube was found to be dry. WELLINSPECTION & FLOW TEST This property has enough area for a future septic upgrade which can be seen on the attached site plan. SITEPLANS We do not anticipate any adverse effects on neighboring wells, septic systems or drainage patterns by the installation of the proposed septic system. There is a home on Lot 13 with a water and wastewater system. This system is recorded with the Municipality. Lots 5 and 7 are vacant and owned by the same builder. Proposed systems locations for these lots are indicated on the site plan. There are no existing wells within 200 feet of the proposed wastewater system for Lot 6. All the wastewater systems for the adjacent lots are in the same general vicinity of the proposed wastewater system on lot 6. The structure proposed for Lot 6 is a single family 3 bedroom home. The access to the lot will be from Eagle River Road through Lot 7. Since both Lot 6 and 7 are owned by the builder, an easement to access Lot 6 will be provided. The builder proposes single family homes on both lot 7 and 5. The lot in relatively flat on the SE half. A swale runs from the extreme SW corner of the lot, across the middle to a drainage area along Eagle River Road. The swale is a gentle 5-10% slope and shows no signs of continuous drainage and no standing water. The swale was observed before breakup and was continuously observed during and after breakup. The swale was devoid of an established drainage channel and slopes from the SW to the NE. The area of the proposed structure and waste water system is flat and along the east lot line. Based upon these observations, it is my professional opinion that a deep trench wastewater system, installed parallel to Test Hole 3, in the location proposed will operate correctly and not impact the surface water below the system. WELLS No impact to the wells on the adjacent lots is anticipated from this system. There is sufficient room on adjacent lots to locate a well and wastewater system. WASTEWATER SYSTEMS No impact to wastewater systems on adjacent lots is anticipated from this system. The house, however, only has 3 bedrooms. 'NEIL INSPECTION sFLcwTEST To the best of our knowledge, the system was installed on 10/17/1997 through g Y g 10/20/1997 for 3 bedrooms. These dates are based on inspections called in and noted on the MOA copy of the permit. Since it was not the practice of the MOA to document site visits at the time this system was installed, we have no information sITERIFr,s relative to the inspections from the MOA files. The Lot 7 adjoining property septic system which was installed by the same developer, was inspected by Dee High Engineering. Contacts with Dee High have indicated that he did in fact perform the ROADDESICN inspections on Lot 6, however, he did not document the system since he had not been paid for his work. We believe that the system on the referenced property was installed by Tom Seaward, owner of Mastercrafl Homes. Since he installed the system on the adjoining Lot 7, as documented by Dee High Engineering, and SOLTEST approved by the Onsite Department, we are assuming that he was certified for the installation at that time. Replace the monitoring tube in the existing trench from the level of the horizontal distribution pipe to 2 feet above ground surface. The existing perforated pipe which extends above grade will be cut off and a section of D3034 PVC will be installed. When the monitoring tube is replaced, we will inspect the sewer rock for MOA code compliance, and will ensure that a filter fabric was placed over the sewer rock prior to backfill. Since Dee High Engineering was on site daring the installation of this system, we have no reason to believe that any component of this system would not meet MOA design and construction criteria that existed at the time of the installation. Attached is an email from Alaska USA Mortgage Company confirming the escrow for the septic system repairs shown above. These repairs will be made after break-up, and prior to July 1, 2010. The Onsite Wastewater Disposal System Inspection Report submitted under my seal February 8, 2010 is a documentation of the system on the referenced property based upon field inspections, measurements, and verbal conversations between our office and Dee High Engineering as well as personnel in the MOA Onsite Department.
