Constructing Engineers 9601 Buddy Werner Dr. Anchorage, AK 99516 Dear Mr. Wilson: We have finished reviewing the paper work for the projects we have been discussing, and I lmve the following comments: Lot 20, Block 1, Scln'oeder East Subdivision. The as-built for this project must be resubmitted in the spring after the additional work is completed. Items missing on this as-built include both tank inverts, the elevation of the bottom of the test hole, and the elevation of any groundwater encountered in the test hole. Also, the Certificate of Health Authority must be signed. Lot 10, Block 1, Eagle Park Subdivision. This as-built needs some additional data, including the second tank invert, the second dlstHbution pipe invert, the elevation of the bottom of the test hole and the elevation of any ground water encountered in the test hole. Lot 7, Block 1, Sampson Estates Subdivision. This as-built should include the second tank invert and the second distribution pipe clean-out invert. A lot line waiver must be obtained for the trench to lot line distance of 3 feet. We have no record of any waiver in our files. Also, the Certificate of Health Authority approval must be signed. Lot 9, Block 4, Itylen Crest Subdivision. After modifications to the surface water on the property, the as-built should be resubmitted. Updated information should include corrected distance from tank to surface water and from field to surface water. Also, the elevation of the bottom of the test bole should be shown on the profile, including any ground water encountered. The Health Authority Approval Checklist should be completed (information is missing). The Certificate of Healtb Authority must be signed. Lot 3, Block 2, llomestead ltills Subdlvlsion. A new soils log must be submitted to replace the incorrectly signed soils log submitted with the original permit. Mr. Henry Wilson, P.E. Also, we advised that the as-built drawing would not be approved until a water monitoring tube was installed adjacent to the absorption bed to a depth of at least four (4) feet below the bottom of the leachfield. We received the subject as-built on February 19, 1992, which at that time, was already over nine (9) months late. As you are aware, this system is not considered to be legal until the as-built has been approved by this office. Either we must receive the above requested information in the very near future and approve the as-built, or we must advise the property owner that their system is not an approved system. Constructing Engineers 9601 Buddy Werner Drive Anchorage, Alaska 99516 Subject: Lot 10 Block 1 Eagle Park Subdivision, PID #050-782-10 Permit #SW910057, As-built Drawing Dear Mr. Wilson: On February 19, 1992, we received an as-built drawing of the on-site wastewater disposal installed on the subject lot. In the "remarks" portion of the as-built it is stated that the trench type system originally proposed and approved (Permit #SW910057) was not installed due to water being observed in the trench during installation, and that a bed absorption system was installed. It is assumed that the 15'x60' bed was not installed over the 12' deep trench excavation or over the 18' deep x 10' long soil test pit. This being the case, at least two soil/percolation tests would normally be required to cover the original and replacement bed areas. Assuming that there was no change in the accepting soil stratum noted during construction of the absorption bed, additional soil/percolation testing will not be required. Inasmuch as groundwater was encountered during construction of the trench, a monitoring tube must be installed adjacent to the bed to a depth of at least four feet below the bottom of the bed. The as-built drawing will be approved when the above mentioned groundwater monitoring tube is installed. We bring to your attention the delinquency of the submittal of the subject as-built drawing. AMC 15.65.150.E requires that the as-built drawing (inspection report) be submitted "within. 30 working days from the date of the final inspection of an on-site wastewater disposal system. . ." In the future, please be more punctual in the submittal of as-built drawings.
