As certified by my seal affixed hereto and as of the validation date shown below, I verify that my investigation, based
on procedures outlined in the Certificate of On -Site Systems Approval Guidelines for this application, shows that the
on-site water supply and/or wastewater disposal system is (are) safe, functional and adequate for the number of
bedrooms and type of structure indicated herein. I further verify that based on the information obtained from the
Municipality of Anchorage files and from my investigation and inspection, the on-site water supply and/or wastewater

RE: COSA for Colonial Park Block 3 Lot 4 S2
Dear Reviewer,
The above referenced property is currently served by a private well and was recently connected to public
sewer.
The existing As -Built survey still shows the septic system, however we are requesting that it be allowed
for approval of this COSA. I have verified that the septic has been removed and the standpipes no longer
exist.
Due to delays in the availability of Arsenic testing results, we are requesting that the COSA be issued
based on historical arsenic information for the area. Attached to this letter are excerpts from COSAs
issued for nearby properties, which show low or non -detect levels of arsenic. The highest reported
reading was 1 ug/L. The records are from the following properties: Colonial Park B3 L2 N2, Colonial Park
B3 L3 N2, Colonial Park B3 L5 S2. Once the arsenic results are ready, we will submit them to the MOA
and to the Real Estate agents managing the transaction. We believe this approach is warranted due to
the long processing times for arsenic samples, and the low likelihood of high arsenic concentrations in
this area.
