TEST PROCEDURE; The absorption system was inspected visually and
measured. It was found to be operating in failure according to initial measurements. Standing liquid depth in the
field was measured at 70" (seventy inches). Gravel beneath pipe is 48' (forty-eight inches) according to the
installation in~pectlon of 1981 on file with the Municipality. This means the absorption £eld is saturated and
absorption into the earth is occurring 22' above the intended absorption area. The owner was present and
indicated that'he knew the absorption system was in poor condition and had expected it would not pass
Municipal requirements. He had already started the process of having a new absorption field designed and
installed by a certified well/septin engineer. He indicated that it will be installed within the next re-ac weeks.
The tank on this lot is 97 feet from the well on an adjoining lot. The well on the adjoining lot was installed after
the tank and absorption system on this lot were installed. Well, septic, and t,xnk requirements were met at the
time of installation on this lot.
The well was tested and water samples were taken. The static water level in the well was measured to be 67 feet
below the top of the casing. The water level in the well was drawn down to g4 feet below the top of the casing
while the pump produced 4.0 GPM. It appears that the well is able to produce approxinmtely 4.0 OPM. The
water was tested for bacteria and nitrates on May 4, :2002. The test indicates nitrates, coliform, and other
bacteria to be well under the maximum allowed by Federal and Municipal standards (see attached results).
~ This well on this lot meets the code and operational requirements of the Municipality of
Anchorage for a four (4) bedroom house. The absorption field is in failure and the owner is in the process of
having it replaced.
In conducting an adequacy test, I attempt to provide a thorough, conscientious engineering snalysis of the
system in accordance with MOA Guidelines & Regulations. The reported results describe the performance of
the system under the conditions encountered at the time of the test, and separation distances measured to readily
identifiable features. The operational ilfe of all wells and septic systems depend on the local soll conditions,
ground water levels that may fluctuate during the year, and the water usage of the family being served by the
system. These conditions are outside the control of the evaluator ofthls system. All systems eventually fail and
satisfactory test results do not guarantee future pefformanee of the system, nor do they guarantee that there are
no hidden defects or encroachments. PES can therefore not provide any warranty for future performance nor
give any estimate of how long the system will continue to meet the operational requirements of the MOA. The
content of this report is for the sole benefit of the owner listed above. Any reliance upon or use of this report by
any other person or party is not authorized nor will it confer any legal right whatsoever.

Reft Proposed Septic System Upgrade for Lot I 1, Block H; Timberlux Subdivision #2
ADVANTEXTM TREATMENT SYSTEM
To whom it may concern:
1. GENERAL: The existing 4 bedroom home is served by a private well and septic system. The
existing system is in a'state of failure and needs to be upgraded. A test hole was excavated on the
property. We are proposing that an Advantex treatment system and a 5-wide type drainfield be
installed within the 30' radius of the test hole.
2. SOILS: See the attached log, which shows the soil classifications, groundwater monitoring,
and the percolation test results.
3. DRAINFIELD APPLICATION RATE: The intent is to install an Advantex Treatment
System that will allow the use of a small drainfield in the area around the 30 foot radius of the
test hole. Given the pemolation rote, the allowable soil application rote for Advantex effluent is
5.0 GPD/FT2. Regardless, we are going to conservatively assume an application rate of 3.0
GPD/FT2. We are going to provide 2.0'+ separation distance to groundwater.
4. TRENCll DESIGN:

b. Allowable Application Rate: 3.0 gallons/day/fi~
c. Number of Bedrooms: 4

5. ORENCO PACKAGE SYSTEM: The STEP tank with the Advantex Treatment System will
be manufactured by Anchorage Tank & Welding to meet the latest design criteria established by
Orenco Systems, Inc. The point of contact at Anchorage Tank is Lowell McNutt. As with their
standard STEP tank, it is equipped with a high water alarm per M.O.A requirements.
6. SURFACE WATER: There is no surface water within 100 feet of the proposed septic
system upgrade.
7. TOPOGRAPIIY: As can be seen on the topography site plan, there are no slope concerns
8. CLOSING: I am open to any suggestions from your department that would be an
improvement to the proposed system. I am unaware of any negative impacts that this installation
would impose on adjacent wells, or septic systems. If you have any questions, please call us at

Per your request a soil investigation for the purpose of determining the
suitability of on site sewer disposal was conducted on Lot 1101ock ri
of Timber].ux Subdivision during the Weeks of Feb 22nd and March 1, 1981.
A MF. -70 backhoe was used for test pit excavation.
On February 24th a teat hole was dug approximately in the! cot-of.the- lot.
The soil was found to:consist of a 1°111 silt with medium p7astioJtys slow
dilatancy and'1ow dry strength, Rounded rocks, cobbles,jc+nd:sOme-;,and lens
were randomly formed,an the soil. The soal !was damp, but friable and loose
in the excavated stato. At the` 11 foot depth ground water apiwered,
The bottom 6 feet of this hole was backfilled and a. bench excravated at the S
foot level.' With the use of a post hole digger a 2 -foot hole was dug into
this bench and this hole was saturated with water for 24jhours The next day
a percolation test was started and the following readinga taken:

TH 5 was dug on top of the ridge. A 4 foot thick layer of HL silt was found
<' between 5 and 9 feet A percolation test was done in the soil, the result
shown on the Soil Log -Percolation Test Report„ The method of test was as
describedearlier, however the test results were irregular and not conclusive
to det- . i- the--ahaa tion capability of the soil. Normally the percolation
rate diminishes during tha + hour test. In this case the percolation rate
ie.s to whertJ e est hole was full ofjtater and increased as the water Level s
subsided. The test o51e was filletwice and the readings repeated both times.
Due to the generally poor soil conditions :in the 'Timbe:rlux subdivision and the
eytremely poor percolation rate observed in TH 1 1 co idcrr it prudent to base _,,
the soil rating on a visual analysis of the soil. A rating of 2w1fl-:prvide
for some excess capacity of the soil remains uniform for the length of the
trench, but will also allow for some changes in the soil.
When the system is installed care must be taken to insure that the trench
is actually :in the sandy silt in which the percolation was performed. If this
;.;oil strata disappears into the HR soil, the Health Department .should be notified
for redesign of the system.
Based on a three bedroom residence and a system installed in the 4. -foot strata
of M.i, silt the following trench dimensions will provide the required absorption:

The Municipality of Anchorage Development Services Department (DSD) Issues Certificates of Health Authority
Approval (HAA) based only upon the representations given in paragraph 4 by an independent professional civil
engineer registered in the State of Alaska. Certificates of Health Authority Approval are required for the transfer
of title (except between spouses) for properties served by a single-family on-site wastewater disposal and/or
water supply system. DSD also issues HAAs upon request to homeowners. Certificates of Health Authority
Approval are valid for 90 days from the date of issue for properties served by a private or Class C well and may
De reissued with new water samples. (Certificates may be reissued for a period of up to one year with vaii(~
water sampies.) Certificates are valid for one year for properties served by Class A er B wells or a public water
system. The Municipality cf Anchorage is not responsible for errors or omissions in the professional engineer's
WORK.

information obtained from the Municipafity of Anchorage fi/es and from my investigation and inspection, the

In conducting this evaluation, AKW1/VC, Inc. attempted to provide a thorough,
conscientious engineering analysis of the system in accordance with ADEC and MOA
DSD Guidelines & Regulations. The reported results described the pedormance of the
system under the conditions encountered at the time of the test, and separation
distances measured to readily identifiable features. The operational life of all wells and
septic systems depend on the local soils condition, groundwater levels that may
fluctuate during the year, and the water usage of the fam#y being sen/ed by the system.
These conditions are outside the control of the evaluator of the system. Satisfactory test
results do net guarantee future performance of the system, nor do they guarantee that
there are no hidden defects or encroachments. AW1/VC, Inc. can therefore not provide
any warranty or future estimate of how long the system will continue to meet the
operational requirements of the ADEC or MOA DSD. The content of this report is for
the sole benefit of the owner listed above. Any reliance upon or use of this report by any
other person or party is not authodzed, nor will it confer any legal Mght whatsoever.
