A test hole was excavated and a percolation test performed in the area of the proposed upgrade. The approximate location of the test hole is located on the attached site plan. At the time of excavation no water was encountered in the test hole and after seven day ground water monitoring, the monitoring tube was found to be dry. Attached is the proposed upgrade design. We do not anticipate any adverse effects on neighboring wells, septic systems or drainage patterns by the installation of the proposed septic system. The mitigating factors involved which support the issuance of the waivers are as follows: 1. Reference to the site plan/topo, surface effluent would not flow toward the wells. e The well located on Lot 4 is located in the home, this would also prevent surface effluent from reaching the well. 3e We are unable to locate a log for the existing wells on Lots 4 and 6. However, in viewing well logs for Lots 11 and 12 we find several soil strata between the ground surface and the aquifer. Several of these strata have a high clay content. The clay layers act as a barrier between the septic system and the aquifer. ON-SITE WASTEWATER DISPOSAL SYSTEM CONSTRUCTION PRACTICES and MATERIAL SPECIFICATIONS CIVIL ENGINEERS (907) 694-2979 FAX (907) 694-1211 REFERENCE: Lots 4 and 5, Block 11, Granite View S/D September 9, 1995 GENERAL: 1. e e e The scope of this project includes the relocation of the existing 500 gallon S.T.E.P. tank and the abandonment of a portion of the existing absorption bed and the installation of a new portion of an absorption bed to serve the four bedroom residence located on the referenced property. Construction shall be in accordance with the approved site plan and design drawings; Municipal permit with any special provisions or conditions; and all applicable State and Municipal Wastewater Disposal Regulations. The contractor shall be responsible for obtaining any necessary underground utility locates. Unless specifically agreed otherwise, the property owner shall be responsible for final grading areas subsequently depressed from soil settling. On all leachfield mound systems, the property owner shall be responsible for ensuring a satisfactory vegetation growth over the mounded area. Contractors installing wastewater disposal systems must be certified by the Municipal Health Department for system installations. Owners installing their own systems must also receive prior approval from the Municipal Health Department. SEPTIC TANK INSTALLATION: A septic tank is to be constructed by a certified septic tank manufacturer. Construction shall include two 4" cleanouts for pumping access. The septic tank shall be sufficiently bedded to prevent settling or shifting of the tank. 17034 NORTH EAGLE RIVER LOOP · SUITE 204 · EAGLE RIVER, ALASKA 99577 Page Two Lots 4 and 5, Block 11, Granite View S/D September 9, 1995 Se 0 Se Se Ail standpipes on the septic tank shall extend a minimum of 12 inches above final grade. Septic tanks installed with less than 4 ft. of cover shall be insulated. A foundation cleanout shall be installed one to four feet from the building foundation. In the line between the tank and the leachfield there shall be two adjacent eteanouts (unless an effluent pumping system exists within the septic tank). These cleanouts shall be located on undisturbed soil not more than 10 ft. from the tank. The first cleanout, in line, shall be to clean toward the leachfield. The second cleanout shall be to clean toward the septic tank. Final grading over the septic tank shall be such that a positive slope exists away from the septic tank. LEACHFIELD BED INSTALLATION: Excavate the proposed Bed Area to the depth shown on the design. The bottom of the excavation shall be within 2 inches of level. If the bottom of the excavation becomes smeared, it must be raked or scratched (ruffed-up) before gravel or sand placement. If a sand layer is required, place sand over entire excavation to the required depth shown on the design. The top of the sand layer must be within 2 inches of level. Sewer rock shall be placed uniformly throughout the entire bed. Perforated distribution pipe must be installed level with perforations down. Gravel depth below the perforated pipe shall be a minimum of six (6) inches. Gravel depth above the perforated pipe shall be a minimum of two (2) inches. The total gravel depth throughout the entire bed shall be a minimum of twelve (12) inches. The perforated distribution pipes must be as shown on the design but, no more than six feet apart. The distance between the outermost perforated distribution pipes and the sidewall of the absorption bed must be no more than three feet. Page Three Lots 4 and 5, Block 11, Granite View S/D September 9, 1995 e 7e Silt barrier material must be installed between the final gravel layer and the native soil backfill, unless insulation is used, then the silt barrier should be installed over the insulation. Ensure the silt barrier covers the entire gravel surface before placing backfill. Monitor tubes shall be of four (4) inch diameter, installed at the locations shown on the design, and extend a minimum of 12 inches above final grade. The portion of the monitoring tube extending through the gravel depth shall be perforated six (6) inches below the bottom of the horizontal distribution lines. The monitoring tube should not extend below the bottom of the gravel surface. Backfill over the final gravel layer must not be less than twenty-four (24) inches. Insulation must be installed when the backfill depth is less than thirty-six (36) inches. The finished grade over the bed must be mounded to prevent the formation of a depression after settling. MINIMUM MATERIAL SPECIFICATIONS: Any septic tank proposed for installation must be constructed by a Municipally approved septic tank manufacturer. Insulation shall be at least 2" thick extruded direct burial polystyrene (Dow Chemical Company Styrofoam HI or equal). e Septic tank inlets and outlets shall be fitted with watertight couplings (Caulder,. Fernco, or equal). Page Four Lots 4 and 5, Block 11, Granite View S/D September 9, 1995 Se e 7e A permeable nontoxic silt barrier (Typar 3401, Mirafi 140N, or equal) must be installed between the final leachfield gravel layer and the native soil backfill. Ail leachfield gravel (sewer rock) shall be 0.5"-2.5" screened gravel with less than 3% passing the %200 sieve. When sand is being used as a filter material, its gradation specifications must conform to current M.O.A. or D.E.C. requirements. INSPECTIONS: Typically there will be a minimum of three (3) inspections required during the installation of the wastewater disposal system. These inspections will occur as follows: The first inspection must be conducted after the excavation of ditches, pits, trenches, or beds and before the installation of any gravel. A septic tank may be set in place, but may not be backfilled before this inspection. e The second inspection must be conducted after the placement of the silt barrier, gravel, distribution lines, standpipes, cleanouts, and insulation, but before the placement of any other backfill. e The final inspection is to occur upon final grading of the property. Often there will be more than these 3 inspections required. Especially with the installation of multiple trenches, sand filters, pressurized distribution systems, etc. Thus, the inspecting engineer is to be contacted at least 24 hours prior to the start of construction. If necessary, a pre-construction meeting will take place on-site. The inspecting engineer will not coordinate, direct or control in any way the contractors activities. Page Five Lots 4 and 5, Block 11, Granite View S/D September 9, 1995 The owner shall contract with the contractor to perform the work outlined in these specifications and plans and in accordance with the attached M.O.A. permit. There will be no contractual arrangement existing between the contractor and S & S Engineering. S & S Engineering shall be the owner's representative and will inspect the work as stated above to document the contractors activities. Final acceptance of the contractors work rests with the owner and the M.O.A. S & S Engineering shall have no liability to the owner or to others for acts or omissions of the contractor or any other persons performing work on this project or the failure of the contractor to carry out the work in accordance with these construction documents. S & S Engineering's inspecting engineer will not be responsible for the construction means, methods, techniques, sequence, procedures or the safety precautions incident to this project. We also request that the fees for the permit and any required waivers be forgone entirely. Finally, we request that the Health Authority Approval (HAA) which we certified, based on the Municipal as-built certified by Municipal staff, be cancelled. The reason for all the above requests is that we excavated the system on January 18, 1990 because it was not functioning properly and we discovered that the information supplied to us by the Municipality was totally erroneous. We also paid for a sewer permit and waiver review in October 1989 and, had we known at the time, the replacement system could have been included under the same fees. The actual facilities installed on this lot are as shown on the attached as-built sketch. The septic tank was still tied to a crib which then had a small semi-trench (20 ft. +/-) running out of it. There was never an installation of an 80 ft. trench nor was gravel used in what little trench there is. We believe that our test of the sewer system passed because the crib had been pumped. Had we known a crib existed, we would have performed our test differently and justifiably failed the system as it should have been. The actual seepage area of the systems installed on this lot were not and are not adequate. Our proposed solution to fix the sewer system is to install a shallow bed system with a total area of 1,440 sq. ft. As proposed the existing crib and semi-trench will need to be excavated and the material removed to the Municipal landfill. The excavated areas will then need to be backfilled with a silty sand material somewhat similar to insitu soil conditions. Then the bed can be installed at a depth of 3.5 ft. below the surface. A new lift station will be required to lift the sewage from the septic tank to the bed. All the details of construction will be as shown on the Sewer System Location Plan attached, our soils test which was rated at 240 sq. ft. per bedroom is also attached. This information would normally indicate that both the well and septic system on Lots 4, 5 was also serving Lot 3. 'The following information was obtained from site visits on October 6 and 9, 1989: 1. There is a greenhouse and shop on Lot 3. 2. There does not appear to be any form of residence on Lot 3. 3. There is no indication of a well or septic system located on Lot 3. 4. The owner of Lot 3 (who resides on Lot 1) stated that Lot 3 was not served by the well or septic system on Lots 4, 5 Block 11. 5. In view of the fact that there is a greenhouse (evidence of recent use) and evidence of a relative large outside plant growing area, it is assumed that water from somewhere is being furnished to Lot 3. Street, Fifth Floor Anchorage, Alaska 99501 SUBJECT: REQUEST FOR WAIVER OF WELL TO FIELD ON LOTS 4 AND 5, BLOCK 11, GRANITE VIEW SUBDIVISION Dear Mr. Roth: We are writing to request a waiver of separation distance from the well to. the sewer system. The existing sewer system on the subject property is located 90 ft. from the well on the subject property. The required separation distance at the time the system was constructed was 100 ft. We are .no requesting a waiver for the separation distance of the sewer system on the subject property to the well on Lot 6 because the development of Lot 6 occurred after the development of the subject property. The installation of the': sewer system on the subject property was 'inspected and approved as it exists today by the Municipality of Anchorage in around 1972. The system was further upgraded in 1974 and inspected and approved again by the Municipality of Anchorage. The subject sewer system serves Lots 3, 4, and 5 and Lot 6 is served separately. The well on the subject property is 210 ft. deep and has a static water level of 75 to 80 ft. There are no other adjacent wells which would be pertinent to this request except the well on Lot 6. The well is located above the sewer system and drainage is from the well to the sewer system not the other way around. There is no well log for the well on the subject property, but we judge, based on the soils test for the sewer system that there at least six (6) ft. of silty sand immediately below the surface and then eight (8) ft. of silty sandy gravel to at least 14 ft. Both these soil strata were rated at above 200 sq. ft. per bedroom in terms of percolation rate. We believe this provides adequate protection to the well and suspect that one would find similar soils down to the 210 ft. level. There is limited possibility of the well on this property becoming contaminated due this waiver as the systems have existed in their present state for over 15 years and the nitrate level in the subject well is not detectible and less than 0.10 mg/1. Mr. Dan Roth October 3, 1989 Page Two Along with this waiver request, we are also requesting a permit to upgrade the septic tank to a 1,250 gallon tank and plan to install the new tank as far as possible away from the well. We would like to also obtain a waiver of the septic tank to well distance to 90 ft. We believe this will also not harm the well as the existing tank has been located 75 ft. from the well and we will be improving this situation. It is also obvious that these systems were approved by the Municipality of Anchorage in 1974. The as-builts also show that the system on our clients lot was approved prior to the system on Lot 6.
