This permit was valid for 365 calendar days. If this was a well permit and the well has been drilled, a well log must be sent to this office for documentation of the installation and to close the permit. If this permit was for a wastewater disposal system, an original as -built inspection report must be sent to this office for review, approval and documentation. This as -built inspection report must be signed by the licensed Professional Engineer who inspected the installation of the system. As -built inspection reports are required to be submitted within 30 days of the completion of the system. If no system was installed under this permit, and you are still planning to install a well or wastewater disposal system, a new permit must be obtained from this office. A new permit may be issued free of charge for a second year if the application for the renewal is received on or before the date of expiration of the original permit. We intended to install a septic system for a three bedroom house during the fall of 2000. This application has been prepared in accordance with Anchorage Municipal Code section 15.65, which is titled Wastewater Disposal. The application addresses required items of that code. Site Plan and Sematic Trench Dimensions Presented in this application are: • Plate 1, which presents the proposed location for an on-site class C well, existing septic system leach fields, proposed septic system leach fields, proposed 1000 gallon septic tanks, lot lines, proposed building layouts, and existing topographic contours. The ADEC has approved a request to install a Class C well, which will be located as shown on Plate 1, along with a waver of its required septic field separation distance from 150 to 100 feet. • Plates 1 and 2, which present proposed septic system layout and dimensions for the deep trench, piping, septic tank, and cleanouts. • Plate 3, which presents distances to all septic systems, wells, drainage courses, roads, and property lines. • Plate 1, which presents the location of the exploration test pit and percolation test performed at Lot 13. • Plate 1, which presents topographic contours of Lot 13. The area around Lot 13 slopes downward at about a 5 percent grade to the east. Runoff does not pond on the site; it flows to the east. On -Site Septic System Design Design calculations for the on-site septic system are attached. Test Pit and Peculation Test Data A test pit was excavated to a 16 foot depth and a percolation test was performed next to it at the location shown on Plate 1. A description of subsurface conditions encountered and percolation test data are presented on the attached standard MOA Soils -Log Percolation Test form. Adjacent Property The proposed and alternative septic systems will not adversely impact adjacent wells, wastewater systems, reserve space and drainage. The waiver is granted for a reduction to 100 feet.. Attached, you will find the Class "C" Inventory and Source Registration Form for this construction. The form has been marked with what has been submitted. Please proceed with the construction. Inspection and sign -off of as -built plans shall be performed by a professional engineer registered in the State of Alaska. Arrangements for this inspection are the responsibility of the developer. This approval does not imply the granting of additional authorizations, nor obligate any state, federal or local regulatory body to grant required authorizations. If construction or operation have not begun with two years after issuance of plan approval, the approval is void, and plans must be submitted to the Department for review and approval.
