A. Open and Close on the same day.
B. Covered, sealed, and heated to prevent freezing.

Subject: Septic Tank Upgrade Permit - Birch Hills Terrace Block 2, Lot 1
The owner has requested we proceed forward to obtain a septic permit to
upgrade the failed septic tank on the subject lot. The proposed upgrade will
serve the existing 3-bedroom house.
The house is served by a private well and is no longer used as a bed and
breakfast. Area lots are served by private and public water as noted on the
attached design. There is no surface water within 100' of the proposed tank.

Dear Ms. Kelliher:
I have reviewed your June 2, 1994 submittal regarding the upgrade of your single family
home into a Bed and Breakfast. Based on this review, I have the following comments.
From the submitted information, it appears that the existing water system met all
regulations and guidelines when it was installed. Based on this information and the fact
that the demand on the existing water system will not be increased, it appears that the
water system will be able to meet the demand without modifications. Therefore, the
existing Class C Public Water System serving the one bedroom Bed and Breakfast and
two bedroom single family home located on the above-referenced property is approved
for the concerns of this Department. A final Operation Certificate, constituting this
approval, is enclosed.
Normally, public water systems serving Bed and Breakfasts are not assigned Public Water
System Identification (PWSID) Numbers and are not required to monitor water being
provided tothe public (your clients). However, due to the elevated level of nigrates (4.16
rog/I) and the existing separation distances, monitoring will be required as a requirement
for operation of the existing public water system. The PWSID Number assigned to your
Class C Public Water System is 218433. Please sample for nitrate (as nitrogen) and total
coliform bacteria once in either January or February and then again either May or June
each year.
It appears that the existing wastewater disposal system was also installed in accordance
with state and local regulations and guidelines at the time of installation. Also, the existing
wastewater disposal system was able to accept the necessary amount of water to pass

an adequacy test on October 9, 1992. The existing wastewater disposal system was
adequate to handle the volume of wastewater from a one bedroom Bed and Breakfast
and a two bedroom single family residence. Thus, in accordance with the provisions of
18 AA_C..72., .Wastewater Disposal Regulations and the Department's guidelines, the
."'lwastewater.:disposal system serving the facility on Lot 1, Block 2, Birch Hills Terrace
Subdivision is approved for operation. This letter constitutes final operation approval for
the existing wastewater disposal system. I wish to emphasize that if the existing
wastewater disposal system fails and is then upgraded, all current regulations will need
to be met.' ..
As we discussed on June 2, 1994, under State Ddnking Water and Wastewater Disposal
Regulations, the Department is now charging plan review fees. I have determined that
the total plan review fee for the above-referenced project is $270.00. I have enclosed the
necessary invoices for your use.
