Septic System Waiver Request
Ladies and Gentlemen:
I am writing to document an existing system on this property. The existing system serves as an agricultural
well for this property. The existing three (3) bedroom house is served by AWWU. Currently the lot is
developed. The existing system was installed by the owner under an approved permit in August of 1975.
PES documented the system and tested it in September of 2014. The system appears to be installed as
permitted and is functioning adequately. The lots to the west are also served by AWWU water with
septics, and the lots to the north, south, and east have private wells and septic systems. The private wells
on the surrounding lots are located over 100' from this system. The agricultural well on this lot is located
78 feet from the drainfield on this lot. See waiver request.
1. Soils. One test holes were performed by 0&E Geotechnical & Development Co. August of 1976. The
engineer recommended an absorption area of 100 square feet per bedroom at that time.
2. Soil Absorption System Design.
a. See Sheet 2 of the Documentation package.
3. Surface Water: There is no surface water within 100 feet of the existing septic tank and drain field.
4. Topography: The existing topography slopes from the northeast to the southwest at
approximately 10%. There are no steep slopes in the vicinity of the drain field.
S. Waiver Request: I request a waiver for the minimum distance from the drainfield and the agricultural
well on this lot to 78 feet. As outlined under 18 AAC 80.020 and 18 AAC 72.021(a), I have calculated the
following points:

pollution, possible chemical pollution but unlikely from household chemicals." It is in my opinion, this
waiver request meets the above criteria for approval and does not constitute risk to health. C hope the

The existing installation does not affect the future development of the surrounding or existing lots.

5. STATEMENT OF INSPECTION BY ENGINEER
As certified by my seal affixed hereto and as of the validation date shown below, I verify that my investigation, based on procedures
outlined in the Certificate of On -Site Systems Approval Guidelines for this application, shows that the on-site water supply and/or
wastewater disposal system is (are) safe, functional and adequate for the number of bedrooms and type of structure indicated
herein. I further verify that based on the information obtained from the Municipality of Anchorage files and from my investigation and
inspection, the on-site water supply and/or wastewater disposal system is (are) in compliance with all applicable Municipal and State
codes, ordinances, and regulations in effect at the time of installation.
In conducting an adequacy test, I attempt to provide a thorough, conscientious engineering analysis of the system in accordance with MoA
COSA guidelines and regulations. The reported results describe the performance of the system under the conditions encountered at the time
of the test, and separation distances measured to readily identifiable features. The operational life of all wells and septic systems depend on
the local soil condition, ground water levels that may Fluctuate during the year, and the water usage of the family being served by the system.
These conditions are outside the control of the evaluator of this system. All systems eventually fail and satisfactory test results do not
guarantee future performance of the system, nor do they guarantee that there are no hidden defects or encroachments. Therefore we cannot
provide any warranty for future performance, nor can we estimate remaining life of the system. The content of this report is for the sole
benefit of the owner listed above. Reliance on this report by another person is at their own risk. Pannone Engineering Services LLC highly
recommends buyers hire their own engineer to evaluate this report.

As certified by my seal affixed heretoandas of the validation date shown below,.I verify that my investigation, based on procedures outlined
in the Certificate of On -Site Systems Approval Guidelines for this application, shows that the on-site water supply and/or wastewater
disposal system is (are) safe, functional and adequate for the number of bedrooms and type of structure indicated herein. I furtherverify that
based on the information obtained from the Municipality of Anchorage files and from myinvestigation and inspection, the on-site water supply
and/or wastewater disposal system is(are) in compliance with all applicable Municipal and State codes, ordinances, and regulations in effect at
the fime of installation.
In conducting an adequacy test, I attempt to provide a thorough, conscientious engineering analysis of the system in accordance with MoA COSA
guidelines and regulations. The reported results describe the performance of the system under the conditions encountered at the time of the test,
and separation distances measured to readily identifiable features. The operational life of all wells and septic systems depend on the local soil
condition, ground water levels that may fluctuate during the year, and the water usage of the family being served by the system. These conditions
are outside the control of the evaluator of this system.. All systems eventually fail and satisfactory: test results do hot guarantee future
performance of the system, nor do they guarantee that there are no hidden defects or encroachments. Therefore we cannot provide any warranty
for future performance, nor can we. estimate remaining life of the system. The content of this report is for the sole benefit of the owner listed

Certificate of On -Site Systems Approval # osc151103
A Certificate of On -Site Systems Approval inspection and test of potable
water was recently conducted on the well water supply on Block 1, Lot 6 of
Birch Hills Terrace #2 subdivision. This inspection revealed a nitrate
concentration of 9.03 milligrams per liter (mg/L) was reported for the
property's well water sample. The Environmental Protection Agency (EPA)
has established a maximum contaminant level (MCL) of 10.0 mg/L for
public drinking water systems. While private wells are not subject to this
regulation, EPA standards are based on existing health information and can
therefore be used to gauge the relative quality of water from private wells.
Please see the attached "Nitrate Fact Sheet" for important information
regarding nitrate.
This advisory must be attached to all copies of the subject Certificate of On -
Site Systems Approval.
