Ref: Reissuance of Septic System Permit for Paradise Valley Subdivision; Lot 1, Block 3,
To whom it may concern:
A permit (#SW050110) was granted by your department on May 3, 2005. It is our understanding
that the well has been drilled under this permit; but the septic has not been installed and the
permit has expired. The current property owners are requesting the permit be reissued. On May,
16, 2006, we performed a site visit and found that the house has been framed and appears to be in
the general location shown on our site plan and design drawings. Based upon our visual
assessment, the proposed septic system is able to be installed as previously designed. The area
below the drainfield will still be required to be filled so that no slope within 50 feet downhill of
the proposed drainfield exceeds 25 percent. The special provision of the existing permit will also
still be required. Based upon this information, we request the on-site wastewater disposal permit

Ref: Proposed Well and Septic System for Paradise Valley Subdivision; Lot 1, Block 3,
To whom it may concern:
A permit (#SW030112) was granted by your department on May 1, 2003. We are resubmitting
the design to show a 1500 gallon 3/16" poly coated advantex tank. The proposed 4 bedroom
home will be served by a private well and septic system. Two test holes were excavated on the
property. We are proposing to design the septic system around the 30 foot radius of these test
holes. Due to the limited amount of receiving soils, and the steep slopes, we are proposing to use
an Advantex treatment system and a 5 -wide trench type drainfield. Comments regarding the
design are summarized as follows:
1. SOILS: See the attached logs which shows the soil classifications, groundwater monitoring,
and the percolation test results.
2. DRAINFIELD DESIGN: See attached design drawing for drainfield specifications.
3. SURFACE WATERS: There are no surface waters within 100 feet of the proposed septic
system.
4. TOPOGRAPHY: As can be seen on the attached topography site plan, the proposed
drainfield is to be installed on a 20-30 percent slope. The contractor at the time of construction
will fill this slope in with fill to create a slope of less than 25 percent 50 feet downhill from the
proposed drainfield and 35 feet from the alternate site. Also there is a cutbank 50 feet plus from
the proposed drainfield and 38 feet from the alternate site. We request a 35 foot waiver from the
alternate site to this cutbank.
5. WAIVER: A waiver to impermeable soil from the bottom of absorption field was requested
in April 23, 2003. Your department has granted this waiver for the design approved in May 1,
2003. We are requesting to use the previously granted waver.

We are unaware of any adverse impacts this installation would have on adjacent wells or septic

Ref: Proposed Well and Septic System for Lot 1, Block 3; Paradise Valley Subdivision
ADVANTEXTM TREATMENT SYSTEM
To whom it may concern:
1. GENERAL: The proposed 4 bedroom home will be served by a private well and septic
system. Two test holes were excavated on the property. We are proposing to design the septic
system around the 30 foot radius of these test holes. Due to the limited amount of receiving soils,
and the steep slopes, we are proposing to use an Advantex treatment system and a 5 -wide trench
type drainfield. The well is to be located in the area shown on the site plan.
2. SOILS: See the attached log, which shows the soil classifications, groundwater monitoring,
and the percolation test results.
3. DRAINFIELD APPLICATION RATE: A percolation test was performed in test hole #1
and #2 between the depths of 1.0 to 1.5 feet and found the rates ranged from 1.9 to 2.0
minute/inch. The Advantex system has received class III approval and we are going to
conservatively assume an application rate of 3.0 GPD/FT2.

5. ORENCO PACKAGE SYSTEM: The STEP tank with the Advantex Treatment System
will be manufactured by Anchorage Tank & Welding to meet the latest design criteria
established by Orenco Systems, Inc. The point of contact at Anchorage Tank is Lowell McNutt.
As with their standard STEP tank, it is equipped with a high water alarm per M.O.A
requirements.
6. SURFACE WATER: There is a creek 100 feet plus away that will be no concern to the
proposed septic system.
7. TOPOGRAPHY: As can be seen on the attached topography site plan, the proposed
drainfield is to be installed on a 20-30 percent slope. The contractor at the time of construction
will fill this slope in with fill to create a slope of less than 25 percent 50 feet downhill from the
proposed drainfield and 35 feet from the alternate site. Also there is a cutbank 50 feet plus from
the proposed drainfield and 38 feet from the alternate site. We request a 35 foot waiver from the
alternate site to this cutbank.
8. WAIVER TO IMPEARMEABLE SOILS: An impermeable layer was found at a depth of
5-6 feet in test hole #1 & #2. We are requesting that the vertical separation distance between the
bottom of the drainfield, and the impermeable soil layer, be waived to 4 feet. Justification for this
reduced separation distance is summarized as follows:
• The effluent being discharged to the drainfield is from an Advantex treatment system,
which has CBOD and TSS levels that are expected to be less than 10 mg1L. In addition,
the total nitrogen levels are expected to be less than the effluent from a lined Intermittent
Sand Filter (effluent from an ISF can be discharged into a drainfield that is only 4 feet
from bedrock and impermeable soils)
• There will still be a 4+ feet separation distance between the bottom of the drainfield and
groundwater
• There will be no discernable benefit (reduced BOD, TSS, or total nitrogen) achieved by
passing the effluent through 6 feet of soil vs. 4 feet of soil. It is generally agreed upon
that the pathogen reduction occurs within the first 4 feet of soil, hence the 4 feet required
separation distance to groundwater. It is reasonable to assume that by the time the
effluent passes through 4 feet of the insitu 30 & 3.33 minute per inch soil that the BOD

more reduction in total nitrogen achieved by building the mound several feet higher. The
highly treated effluent will probably not see further reduction in nitrogen levels without
introduction of a carbon source (as is the case with ISF effluent).
Based upon the facts presented we are requesting that the vertical separation distance between
the bottom of the proposed drainfield and an impermeable soil layer be reduced to 4 feet.

In conducting this evaluation, GEG provided an engineering evaluation of the well and/or septic system in
accordance with the guidelines and regulations established by the Municipality of Anchorage and industry
practices. The reported results describe the condition of the system/s on the date/s of the evaluation.
Separation distances were measured to readily identifiable features. Hidden defects or encroachments may
exist that were not identified during the evaluation. The operational life of all wells and septic systems depend
upon a variety of variables, including (but not limited to) soil conditions, groundwater levels (that may fluctuate
during the year), quality of construction (materials and workmanship), and the water usage of the family utilizing
the system/s. These conditions can vary, and are outside the control of GEG. Satisfactory test results do not
guarantee future performance of the system/s; therefore, GEG makes no warranty (express or implied) regarding
the future performance of the well or septic system. GEG makes no representation whether an alternative well
or septic system can be installed on the property in the event either of the current systems fail to perform
adequately in the future. The content of this report is for the sole benefit of the person/party that retained GEG

Attached is an annual inspection sheet from my inspection on 5/11/23. From my inspection there is cosmetic damage to
the pod flange that interfaces with the lid. The flange has two cracks from the right side end being pushed inwards. This
right side indention only effects the right gas shock that supports the lid. The shock was removed to allow the lid to open
and close freely. The lid will stay open with only the left shock. The lid does close and seal securely. No leaking is present
from inside out when running the recirculating pump and no leaking is present from outside the pod to the inside. The
bottom of the pod is clean and free of fluid. In my professional opinion the damage to this pod is purely cosmetic and
this AX20 Pod can easily last a lifetime.
I spoke with David at Garness he is willing to sign our inspection sheet as Garness doesn't service Advantex systems A+
Home Service is the service provider for these systems. Let me know if you need anything else we will be burying the
well electrical tomorrow.

Dear Mr. Dunlap:
This letter report presents the findings of our geotechnical evaluation for the existing on-site
wastewater treatment system at 18531 Portugal Place in Anchorage, Alaska. We understand
that the property has been recently sold and that funds are currently being held in escrow
pending the resolution of concerns regarding apparent damage to the treatment system.
The Municipality of Anchorage (MOA) has expressed concerns regarding a recent
inspection of the system which revealed damage to one of its components. The MOA has
requested that a geotechnical engineer observe the damage and provide an opinion
regarding the cause of the damage prior to issuing a Certificate of On-Site Systems
Approval (COSA) which would release the escrow funds to the seller that are currently
being held.
SITE VISIT AND OBSERVATIONS
On May 30, 2023, Shannon & Wilson mobilized an experienced geotechnical engineer from
our Anchorage office (Mr. Kyle Brennan) to observe the site conditions. While on site, we
observed exposed portions of the system and the ground surface/topography around the
system. Based on our conversations with you and observations at the site, the system
consists of an AdvantTex treatment system by Orenco Systems. The system incudes a large
underground tank that collects wastewater from the residence and recirculates the liquid
effluent from the tank through a filter pod that rests above the tank. The filter pod at the
subject residence is approximately 6 feet long by 2 feet wide, with the long axis of the pod
oriented roughly east-west. During our site visit, we opened the pod lid and observed that
the pod chamber was roughly 3 feet deep. The east end of the pod exhibited inward flexing
and minor cracking along the rim. Cracking did not appear to propagate down the side of

the pod more than an inch or two. Similar flexing, albeit with less total deflection and no
cracking, was observed on the west end of the pod. It appeared that deflections on the pod
sides were greater at the ground surface than deeper on the pod walls. Flexing was not
observed on the north or south ends of the pod. The exterior of the pod is surrounded by
vertical blueboard insulation.
The system is located on the north end of the residence driveway where the grade is
approximately 2 feet above the driveway grade. Landscaping on the north and east sides of
the pod consist of large rock and gabion retaining structures. The ground immediately
around the portions of the system exposed at the surface is flat, extending at least 4 to 5 feet
in all directions. Beyond this distance on the north and east sides of the system, the ground
slopes up at approximately 1 horizontal (H) to 1 vertical (V) to the base of a gabion retaining
wall that is roughly 15 feet from the system on the north side and greater than 30 feet on the
east side. The gabion wall on the east side of the system shows significant signs of internal
instability, but the portion of the wall closest to the system on the north side appears to be
relatively stable.
Soil borings were not conducted or reviewed for our evaluation, however, based on our
experience in the area, the native soil conditions likely consist of relatively dense materials
of glacial origin and bedrock may be present within approximately 20 to 30 feet of the
ground surface. The soils in which the system is embedded likely consist of imported fill or
soils sourced from on-site to backfill around the system components. At the time of our site
visit there were no obvious signs of surface subsidence and/or settlement suggesting that
the soils were placed and compacted with moisture/density control around the system
during construction.
CONCLUSIONS
Based on our observations on site, it is our opinion that the damage observed in the system
pod is likely due to shallow lateral earth pressure and seasonal ice pressure from soils
immediately around the pod walls. We do not believe that the apparent instabilities in the
retaining walls and slopes east of the system are directly related to the observed pod
damage. We believe that fill placement and compaction during construction of the system
likely imparted horizontal loads on the pod walls which would have caused some minor

flexing. Due to the topography around the pod, it appears that surface drainage likely
collects around the pod and during winter months ice wedges likely form around the pod
walls. Seasonal development of these ice lenses have likely caused increased lateral
pressures on the pod walls and progressive displacement/flexing and subsequent cracking.
If left as is, we anticipate that it will be possible for continued deflection and cracking to
progress on the system’s pod feature. Seasonal frost action could cause progressive
development of this damage, but it is not possible to definitively estimate how long it will
take for the damage to progress to a point where the system will need replacement.
Improvements to the site drainage and backfill material around the pod could be
undertaken to reduce the effects of seasonal frost on the system’s shallow components.
Development of such recommendations are outside of the scope of our services, but can be
provided for additional fee if desired.
It should be noted that a detailed evaluation of the slopes and retaining walls above the
system was not performed by Shannon & Wilson. However, the distress observed on site,
appeared to be related to internal stability of the wall and not related to larger scale
foundation issues under and in front of the wall. That said, if a larger scale failure of the
retaining wall and/or slopes around the site is experienced, damages to the system could
occur. However, given the proximity of the residential structure and other site features, the
damages to the onsite septic system would be relatively minor in comparison to the other
damages to the site that would occur.
CLOSURE AND LIMITATIONS
This report was prepared for the exclusive use of our client and their representatives for
evaluating the site as it relates to the geotechnical aspects discussed herein. The conclusions
and interpretation contained in this report are based on site conditions as they are inferred
from existing information and as described herein. Actual conditions may vary significantly
which may influence system performance.
Unanticipated soil conditions are commonly encountered and cannot fully be determined
by reviewing existing information and observing surface conditions. Such unexpected
conditions frequently require that additional expenditures be made to attain a properly

constructed project. Therefore, some contingency fund is recommended to accommodate
such potential extra costs. Please read the Important Information section at the back of this

Site visit to assess AX filter pod riser lids damage.
Snow was cleared from pod lid and no visible damage reported.
Due to heavy frost, the tech was not able to open the lid.

theres was damage to the filter pod consisting of cracks that may
or may not be repaired at this time. The AX pod does not appear
to be leaking it also appears to be a good seal on the lid.

This system has been plagued since day one as the rocky hillside continues to throw boulders at the system and anyone
who approaches it.
Larry conducted the 2022 Annual Inspection last Thursday and took the attached photos.
IMG 60: A boulder slid towards the Discharge Pump Basin a few years ago. That lateral movement sheared the insulation
off the bottom of the lid. The basin cannot seal properly as long as the boulder remains in position.
IMG 62: The lateral movement of the hillside has bent in the wall of the AX20 pod inward so much that the manifold
snapped. The blue glue you see in the corner of the pod is Larry's piping repair. With the manifold break in that position
water simply poured into that portion of the filter fabrics — which explains the nasty goo pileup. That has been cleaned
away.
IMG 63: This is a closeup of the pod wall at the bending point. It's hard to determine if the wall has cracked. There is the
potential of a pod replacement and the unit itself (insulated) is north of $5000.
The seller has been reluctant to remove boulders or to provide a means for remote monitoring for the past nine years so
we feel it's in the best interest to fully disclose these discoveries before another homeowner purchases this potential
problem.

THIS MAINTENANCE AND REPAIR AGREEMENT, herein the "AGREEMENT" made and
entered into as of this 1- Day of of 20R3 , by and between

Anchorage, herein the "MUNICIPALITY", in accordance with Anchorage Municipal Code
(AMC)15.65.365. In consideration of the mutual covenants contained herein, the parties to this
Agreement agree as follows:
1. Advanced Wastewater Treatment Systems. The Municipality grants permission to the
Owner to utilize and operate an Advanced Wastewater Treatment System (AWWTS),

2. Maintenance. Repairs and Alterations.
(Owner is required to read, understand and initial each section)
Throughout the term of this Agreement, the Owner shall enter into a service agreement
with an AWWTS service and maintenance provider approved by the Municipality or the
manufacturer's representative. The AWWTS shall be maintained in a satisfactory
condition capable of performing as designed and producing treated septic effluent in
accordance with the equipment's approval for operation in the Municipality.
It shall be the responsibility of the Owner during the term of this Agreement to pay for all
repair(s), maintenance, adjustment(s), replacement costs, and inspection costs. This
'4vv AD includes an annual maintenance fee (typically $400 to $600).
1W11" n Owner agrees that only maintenance and repair personnel approved by the Municipality
or the manufacturer's representative will inspect and make any necessary maintenance,
repairs or permitted alterations to the system.

Owner agrees to maintain remote monitoring of the AWWTS as required by the

3. Term. The term of this Agreement shall begin on the date of approval by the
Municipality to operate the installed system, or upon transfer of title, and shall continue
while the AWWTS is operational or until title is transferred.
4. Nonwaiver. The failure of the Municipality at any time to enforce a provision of this
Agreement shall in no way constitute a waiver of the provisions, nor in any way affect
the validity of the Agreement or any part hereof, or the right of the Municipality
thereafter to enforce every provision hereof.
5. Amendment. This Agreement shall only be amended by authorized representatives of
the Owner and Municipality. Any attempt to amend this agreement by either an
unauthorized representative or unauthorized means shall be void.
6. Jurisdiction: Choice of Law. Any civil action arising from this Agreement shall be
brought in the Superior Court for the Third Judicial District of the State of Alaska at
Anchorage. The laws of the State of Alaska shall govern the rights and obligations of the
parties under this Agreement.
7. Severability, Any provisions of this Agreement decreed invalid by a court of competent
jurisdiction shall not invalidate the remaining provisions of the Agreement.

In conducting this evaluation, GEG, LID. attempted to provide a thorough,
conscientious engmeeting analysis of the system in accordance with ADEC and MOA
DSD Guidelines & Regulations. The reported results described the performance of the
system under the conditions encountered at the throe of the test, and separation
distances measured to readily identifiable features. The operational life of all wells and
septic systems depend on the local soils condition, groundwater levels that may
fluctuate during the year, and the water usage of the family being served by the system.
These conditions are outside the control of the evaluator of the system. Satisfactory test
results do not guarantee future performance of the system, nor do they guarantee that
there are no hidden defects or encroachments. GEG, LTD. can therefore not provide
any warranty or future estimate of how long the system will continue to meet the

engineer registered in the State of Alaska. Certificates of On -Site Systems Approval are required for the transfer
of title (except between spouses) for properties served by a single-family on-site wastewater disposal and/or
water supply system. DSD also issues COSAs upon request to homeowners. Certificates of On -Site Systems
Approval are valid for 90 days from the date of issue for properties served by a private or Class C well and may
be reissued with new water samples. (Certificates may be reissued for a period of up to one year with valid water
samples.) Certificates are valid for one year for properties served by Class A or B wells or a public water system.
The Municipality of Anchorage is not responsible for errors or omissions in the professional engineer's work.
