Ref: Proposed Well and Septic System for Lot 2, Block 5; Mountainside Village
Subdivision #1
To whom it may concern:
The proposed 3 bedroom house is to be served by a private well and septic system. Two test
holes were excavated on the property. The proposed septic system will consist of a 1250 gallon
S.T.E.P. tank and dual pressurized 5 -wide type drainfields. The drainfield will be designed
around the 30 foot radius of test hole #2. The well is to be drilled as shown on the site plan.
Comments regarding the design are summarized as follows:
1. SOILS: See the attached logs which shows the soil classifications, groundwater monitoring,
and the percolation test results.
2. TRENCH DESIGN:
a. Percolation Rate: 2.28 & 1.0 minutes/inch
b. Proposed Application Rate: 1.2 gallons/day/R2
c. Number of Bedrooms: 3
d. Design Flow: 450 gallons per day
e. Minimum Absorption Area: 375 W
f. Total Depth: 3 feet (max.)
g. M.O.A. Sand Filter: 1 feet
h. Effective Depth: 0.5 feet
i. Width: 5 feet
j. Minimum Length: 2 @ 38 feet lo each (76 feet total length)
k Effective absorption area = 380 ft
Note: the distribution lines are to 1.25 inch sch. 40 pvc with 1/4 inch diameter holes spaced 29
inches on center (15 holes per lateral / 30 holes total in entire system).
3. SURFACE WATERS: There is a creek on the north end of the property. The septic system
is to be designed 100+ feet from the creek.

4. TOPOGRAPHY AND WAIVER REQUEST: The area around the proposed drainfields
are 20-30 percent slopes running approximately southeast/east to northwest/west. All slopes
within 35 feet of the proposed drainfields are to be graded to a less than 25 percent slope. We
request a 35 feet waiver from the proposed drainfields to slopes greater than 25 percent. Given
the porosity of the soil the effluent should move downward rather rapidly with minimal lateral
migration. Based upon these factors it is reasonable to assume that there is little risk with the
granting of this waiver.
We are unaware of any adverse impacts this installation would have on adjacent wells or septic

The Municipality of Anchorage Development Services Department (DSD) Issues Certificates of Health Authority
Approval (HAA) based only upon the representations given in paragraph 4 by an independent professional civil
engineer registered in the State of Alaska. Certificates of Health Authority Approval are required for the transfer
of title (except between spouses) for properties served by a single-family on-site wastewater disposal and/or
water supply system. DSD also issues HAAs upon request to homeowners. Certificates of Health Authority
Approval are valid for 90 days from the date of issue for properties served by a private or Class C well and may
be reissued with new water samples. (Certificates may be reissued for a period of up to one year with valid
water samples.) Certificates are valid for one year for properties served by Class A or B wells or a public water
system. The Municipality of Anchorage isnot responsible for errors or omissions in the professional engineer's
work.

investigation, based on procedures outlined in the Health A uthority Appro val Guidelines for this application,
shows that the on-site water supply and/or wastewater disposal system is(are) safe, functional and adequate
for the number of bedrooms and type of structure indicated herein. I further verify that based on the
information obtained from the Municipality of Anchorage files and from my investigation and inspection, the
on-site water supply and/or wastewater disposal system is(are) in compliance with all applicable Municipal
and State codes, ordinances, and regulations in effect at the time of installation.

In conducting this evaluation, GEG, Ltdattempted to provide a thorough,
conscientious engineering analysis of the system in accordance with ADEC and MOA
DSD Guidelines & Regulations. The reported results described the performance of the
system under the conditions encountered at the time of the test, and separation
distances measured to readily identifiable features. The operational life of all wells and
septic systems depend on the local soils condition, groundwater levels that may
fluctuate during the year, and the water usage of the family being served by the system.
These conditions are outside the control of the evaluator of the system. Satisfactory test
results do not guarantee future performance of the system, nor do they guarantee that
there are no hidden defects or encroachments. GEG, Ltd. can therefore not provide
any warranty or future estimate of how long the system will continue to meet the
operational requirements of the ADEC or MOA DSD. The content of this report is for

Dear Connie Brandon:
The Right of Way Division has reviewed a request for a letter of non -objection to an existing well, which
encroaches 2 feet into the 33 -foot Section Line Easement. On the as -built survey dated August 13, 2003,
submitted with the request, the petitioner has shown the well.
Right of Way Section issues this letter of non -objection with stipulations, and by using it the petitioner is
agreeing to the following:
1. Municipality of Anchorage (MOA) will be held harmless, now and forever, for any damages or
injury to any person as a result of the encroachments.
2. All applicable codes and regulations will be observed and maintained within the easement.
3. This letter of non -objection will in no way preclude MOA from full use and enjoyment of its rights
within any portion of the easement.
4. Additional and extraordinary costs incurred during any future required construction, repair or
reconstruction of MOA improvements to accommodate any or all of the encroachments shall be paid
by the property owner.

AWWU's position regarding the well that encroaches into the section line easement that
