2. All requirements specified in Anchorage Municipal code Chapters 15.55 and 15.65 and the State of Alaska Wastewater

3. The wastewater code requires inspections during the installation. The engineer must notify the Development Services
Department at least 2 hours prior to each inspection. Provide notification by calling (907) 343-7904 (24 hours).
4. From October 15 to April 15, a subsurface soil absorption system under construction during freezing weather must either:
A. Open and Close on the same day.
B. Covered, sealed, and heated to prevent freezing.
Special Provisions:
1. Engineer needs to do an additional percolation test prior to construction of septic fields.
Stamped and signed results to be submitted with inspection report. If the results require a design
change, construction of the system will stop pending On-Site review and approval.
2. Protective berm must be in place at time of inspection report.

Subject: Upgrade Sewer Permit— Denaly Block 1, Lot 14
It has been determined that the absorption field of the subject property is
saturated. Subsequently, the owner has requested we proceed forward to obtain
a septic permit to upgrade the septic system.
The general slope of this lot is from north to south at a grade of approximately 8-
13% over the septic area. We will be utilizing the existing MOA records
performed to investigate soils and groundwater for the original and reserve
fields. The proposed upgrade will serve the existing 5-bedroom single-family
residence.
An existing drainage ditch runs along the western portion of the lot and on the
adjacent property. Per MOA recorded observations of this property and our
investigation, no surface water was present or running and it appears to be a
typical drainage ditch handling storm / seasonal events. The ditch was heavily
vegetated with tall grass.
The property and adjacent lots are served by private water. No surface water
was observed within 100' of the proposed system and there are no known curtain
drains within 50'. We do not expect there to be any adverse effect on adjacent lots

Per your review comments, please see the attached modified design for your
review and approval. An additional testhole will be performed prior to
construction.
As far as the review comment regarding the existing drainage ditch that runs
along the western portion of the lot and on the adjacent property -- it appears to
be a typical drainage ditch handling storm / seasonal events. No water was
present per MOA recorded observations of this property on 8/27/01, 4/20/02 &
9/19/02 by Greg Allen Eberle, PE. Also no water was present based on our own
observations on 5/2/11, 10/27/16 and 11/18/16. Furthermore, after speaking
with John Semon of Northern Excavation who installed the adjacent lot's septic
system — no water was present on 3/27/14 or at anytime he was on the site. The
ditch was heavily vegetated with tall grasses and as stated previously handles
storm /seasonal runoff events.
The property and adjacent lots are served by private water. No surface water
was observed within 100' of the proposed system and there are no known curtain
drains within 50'. We do not expect there to be any adverse effect on adjacent lots

Subject: Surface Water Waiver— Denaly Block 1, Lot 14
Per previous discussions regarding the drainage ditch along the western portion
of the subject property, we respectfully request a waiver to surface water of 50'
from the existing and proposed septic fields.
To mitigate the unlikely event of effluent-surface water interaction of these MOA
perceived encroachments, a berm will be installed to direct potential effluent
overflow away from the ditch as shown on the attached design.
We do not expect there to be any adverse effect on the subject or adjacent lots by
granting this waiver.

I have been instructed by Jim Cross to approve this as-built
inspection report. Greg Alan Eberele, P.E. stated on the as-built inspection
report there is no surface water within 100 feet from the septic tank or
absorbtion field. On the proposal site plan there was no water drainage
indicated. However on the as-built site plan Greg Eberle, P.E. then showed
a drainage ditch. But stated on the inspection report, surface water "None".

The attached site plans shows:
a)
Locations of the proposed on-site wastewater (WW) disposal system with
all its components as well as adequate area for a replacement WW
disPosal site.
b) Measured dimensions of the proposed on-site WW disposal field, tank,
and distribution piping.
c)
Measured distances to all VVVV disposal systems, wells, surface water or
drainage courses, roads, property lines, and structures within 200 feet of
the location of the proposed and existing WW disposal systems on this lot.
d) Location of the percolation test/test pit where the water table monitoring
tube was placed.
e) Topography is 10% sloping nearly due south.
The proposed well and septic system will have very limited impact on adjacent
properties for the following reasons:
a) The surrounding lots have sufficient room for septic sites,
b) Future neighboring septic systems will all be 30' distance minirnatly; all
surrounding properties are as yet undeveloped.
c) Reserve space is adequate, due to absorption capacity.
d) Drainage will not be affected and is not a major consideration in our
design.
No impacts of{he WVV disposal system on adjoining properties are anticipated. All
requisite separation distances relative to adjoining wells, VVW disposal systems, and
this lot's backup site are met herein. There are no known curtain drains.

SPECIFICATIONS FOR ON-SITE SEPTIC SYSTEM
Lot 14, Block 1, Denali Subdivision
August 25, 2001
A. GENERAL
1. The septic plan is for a single-family residence only.
2. The drawing and or site plan shall be a part of this specification.
3. Ail materials and workmanship shall meet the Anchorage Department of Health and Alaska
Department of Environmental Conservation requirements.
4. All soil tests are advisory to the design and are to be verified or modified in the field by the
engineer.
5. Ail excavations and depths are advisory and are to be verified in the field by the contractor to
meet Municipality of Anchorage and Alaska Department of Environmental Conservation
requirements.
6. It is the responsibility of the owner to obtain all necessary permits or easements and to locate
any adjacent multi-family wells.
7. The excavation is to be exactly in the area shown on the site plan; any deviation requires
engineer approval.
8. It is always recommended that a surveyor locate the nearest lot line position and the location
of any easements.
9. Any remaining open test hole excavations shall be filled.
B. BED
1. The bed is to follow the natural land contour to maintain uniform total depth of the bed
bottom.
2. The bottom of the bed shall be level, plus or minus 1.5".
3. The total depth of the bed excavation is not to exceed 4' at any point.
4. The effluent line within the bed shall be laid level within 0.03'.
5. The bed gravel Is to be covered with typar fabric material.
6. Soil or combination of soil and extruded board insulation to a depth of 3' minimum or
equivalent is to be placed over the leachfield.
7. The area over the bed is to be finish graded to prevent ponding of surface water runoff.
8. The septic tank and leachfield must not be closer than 100' to any existing private well, 150'
to any Class "C" well, or 200 feet to any community well.

The Municipality of Anchorage Development Services Department (DSD) issues Certificates of On-Site Systems
Approval (COSA) based only upon the representations given in paragraph 4 by an independent professional civil
engineer registered in the State of Alaska. Certificates of On-Site Systems Approval are required for the transfer of
title (except between spouses) for properties served by a single-family on-site wastewater disposal and/or water
supply system. DSD also issues COSAs upon request to homeowners. Certificates of On-Site Systems Approval
are valid for 90 days from the date of issue for properties served by a private or Class C well and may be reissued
with new water sample results. (Certificates may be reissued for a period of up to one year with valid water
samples.) Certificates are valid for one year for properties served by Class A or B wells or a public water system.
The Municipality of Anchorage is not responsible for errors or omissions in the professional engineer's work.
4. STATEMENT OF INSPECTION BY ENGINEER
As certified by my seal affixed hereto and as of the validation date shown below, I verify that my investigation,
based on procedures outlined in the Certificate of On-Site Systems Approval Guidelines for this application,
shows that the on-site water supply and/or wastewater disposal system is (are) safe, functional and adequate
for the number of bedrooms and type of structure indicated herein. I further verify that based on the information
obtained from the Municipality of Anchorage files and from my investigation and inspection, the on-site water
supply and/or wastewater disposal system is(are) in compliance with all applicable Municipal and State codes,
ordinances, and regulations in effect at the time of installation.

Engineer's Comments: This investigation was comploted in compliance with ADEC and MOA regulations. The
assessment of the condition of the well and septic applies only to the conditions as of the day tested. The flow
and absorption rates may change due to subsurface conditions that may not be observed from the surface,
changes inland use, local soil characteristics, groundwater levels that may fluctuate during the year and the
water usage of the family being served by the system. The operational life of all well and septic systems are
subject to these various and dynamic characteristics and are outside the control of the
evaluator of the well and septic system. Therefore,
ArcTerra can not give any estimate of how long a
system will function satisfactory for current or future
occupants or can ArcTerra guarantee that no unseen
encroachments, deficiencies or discrepancies exist.
