The proposed system will serve a 3 -bedroom single-family residence. The lot was developed in 1983 and the existing drain field is 95% saturated. Water is provided by a private well. System: The proposed system consists of a Biocycle advanced treatment unit followed by a shallow trench. The proposed drain field is 5 feet wide, 20 feet long and contains 3 feet of sewer rock below the distribution piping. Soils: A test hole was excavated on May 14'h, 2008. See the attached soil logs. Ground water was observed during the test hole excavation at 10 feet below the ground surface. Ground water was level was check on May 21", 2008. It was observed at a depth of 9.8 feet. Monitoring will continue through construction of the system Surface Nater: A creek parallels the south property line. There is also a drainage ditch along the west property line. Flow is observed each spring during break up and during periods of heavy precipitation. A 100 foot separation distance will be maintained between the nearest component of the proposed septic system and the creek and ditch. Topography: The lot slopes to the west and south. Generally the slopes do not exceed 20%. A cut bank exist above the proposed septic system location. Waivers: A drain field to lot line waiver is required. The proposed drain field location is at the base of an existing cut slope and will have no significant impact on the neighboring property. Borehole AWM-3 was originally drilled on March 3, 1983 and groundwater was encountered at 13 feet below existing ground surface. We .could not re -drill the borehole with a hand auger because the surficial material had frozen. Approximately ten (10) feet to the west of this borehole is a test pit that is presently open to a depth of nine (9) feet. Groundwater was not observed in the bottom of the pit,therefore, we feel that the groundwater level in borehole AWM-3 is expected to be below nine 9) feet from existing ground surface. In addition, we re-measured—Me water level in borehole AWM-6 located on Lot 3, Block 2 of this subdivision and encountered a water level at 6.2 feet below ground surface. We feel, however, that surface runoff may be affecting the water level at this time of the year. Additional boreholes may be necessary to confirm this. Municipality grants permission to Owner to utilize and operate an Advanced Wastewater Treatment System (AW WTS), described as bioC.uciP �1t,,,r�G located at (legal description). Any change to the design or function of an AW WTS that includes the installation or removal of any parts, components or pieces not included in the original construction permit and design. Certificate of On -Site Systems Approval (COSA). An approval by the Municipality of existing water and wastewater disposal systems given at the time of property sale and title transfer in accordance with Anchorage Municipal Code (hereinafter, "AMC") 15.65. These approvals certify that the systems are adequate for the homes that they support and meet the codes that were in place at the time of system construction. Damage. Any man-made or natural change in a system that would inhibit the system from performing as designed. Maintenance and Repair. The scheduled and as needed replacement of existing parts, components and pieces of an AWWTS that were included in the original design which would allow the AWWTS to continue to perform as designed. Permit. An On -Site Wastewater Disposal Permit as required by AMC 15.65 to construct and operate an AWWTS. 3. Term. The term of this Maintenance and Repair Agreement shall begin on the date of approval by the Municipality to operate the installed system or issuance of a COSA, and shall continue while the AWWTS is in use or is operational or until the property is sold or title is transferred by the owner and a new COSA is issued to the new owner or transferee of the property. 4. Alterations. Installation and Removal of Additional Eauinment Prior to performing any alterations to an AWWTS, the owner agrees to obtain an On-site Wastewater Disposal Permit from the Municipality in accordance with AMC 15.65. 5. Maintenance and Repairs. A. Throughout the term of this Maintenance and Repair Agreement, the Owner shall maintain their AWWTS in a satisfactory condition capable of producing treated septic effluent in accordance with the equipment's approval for operation in the Municipality. The owner shall enter into a service agreement with an AWWTS service and maintenance provider approved by the municipality and the manufacturer of the AW WTS for the entire term of the AW WTS. In addition, it shall be the responsibility of the Owner during the term of this Maintenance and Repair Agreement, and any renewals thereof, at the owner's sole expense, to pay for any and all: (1) repair(s), (2) maintenance, (3) adjustment(s), (4) replacement costs, and (5) inspection costs. B. Owner agrees to comply with all applicable ordinance, laws, regulations, rules and orders for the AW WTS. C. Upon request by the Municipality, the owner agrees to provide the Municipality a written schedule of routine maintenance and repairs which have been performed on the system. When a record of maintenance is documented and maintained by the system vendor, the owner agrees to allow the Municipality access to this information. D. Owner acknowledges that the fine for failing to maintain and repair an AW WTS may be assessed in accordance with AMC 14.60 for improper discharge. R Owner agrees that only maintenance and repair personnel approved by the Municipality will inspect and make any necessary maintenance, repairs or permitted alterations to the system. F. Owner agrees to grant the Municipality reasonable access to test and inspect the AWWTS upon 24 hours written notice. G. Owner agrees that any sale or transfer of title of the property will not occur without a new Certificate of On -Site Systems Approval. H Owner agrees that the AWWTS installation and maintenance requirements as provided by the AWWTS vendor/installer and approved by the Municipality are the governing guidelines for the construction, maintenance and repair of the Owner's AWWTS. 6. Nonwaiver. The failure of either party at anytime to enforce a provision of this Maintenance and Repair Agreement shall in no way constitute a waiver of the provisions, nor in any way affect the validity of the Maintenance and Repair Agreement or any part hereof, or the right of such party thereafter to enforce each and every provision hereof. 7. Amendment. A. This Maintenance and Repair Agreement shall only be amended, modified or changed by a writing, executed by authorized representatives of the parties, with the same formality that this Maintenance and Repair Agreement was executed with, and such writing shall be attached to this Maintenance and Repair Agreement as an amendment. B. For the purposes of any amendment modification or change to the terms and conditions of this agreement, the only authorized representatives of the parties are: a. Owner: VomPS 4 ILonP TU—pM i b. Any attempt to amend, modify, or change this contract by either an unauthorized representative or unauthorized means shall be void. 8. Jurisdiction: Choice of Law. Any civil action arising from this Maintenance and Repair Agreement shall be brought in the Superior Court for the Third Judicial District of the State of Alaska at Anchorage. The laws of the State of Alaska shall govern the rights and obligations of the parties under this Maintenance and Repair Agreement. 9. Severability. Lot 5, Bk -2, Park Hills S/D- #1. To whom it may concern: The subject lot has a 3 bedroom house- on it which is- served by a private welt and septic system. The results of the field investigation and adequacy tests are summarized as follows: A. WELL: The static water level on 8/4/97 was 83' BTC. Water was pumped from the well at an average rate of 4.93 gpm for a total -of 146 minutes (720 gallons). Within the first 46 minutes of the pumping- period- the water level dropped- 67 feet, to a 150feetBTC. The level stabilized at that- level throughout the rest of the test. In short, the well was recovering as fast as the water was being pumped out. This -test is- consistent with the previous- adequacy testperformedon-it in March of 1992. Based upon our test results, it was determined that the capacity of the well exceeds the- Municipal requirements for a 3 bedroom house, (.31 gallons per minute} and will continuously produce greater than 3 gallons per minute (as required for FHA financing). B. SEPTIC TANK: The existing septic tank was installed in May of 1983 (approx. 14 years old). According to the M.O.A records, it is 1000 gallons, has two compartments and is made of steel. Most tanks of this type typically have a structural life of approximately 20 years. No warrantee is made regarding the future life of the septic tank. C. SEPTIC SYSTEM ADEQUACY TEST: The drainfield is a 3 foot wide trench, which is 45 feet long, and has an effective depth of 5 feet (per the 1983 inspection report). Based upon our field measurements, it appears that the bottom of the sump (4inch dia.) is approximately 71.5 inches below the invert of the drainpipe. On the 1994 HAA performed by John Simpson, RE, it was also noted that the effective depth was 72 inches. On the day of the inspection (8/4/97), the liquid depth in the sump was 64.5 inches deep (about 7 inches below the invert), indicating that the trench was over 90% full. Water was introduced into the sump at an average rate of 4.7 gpm for 125 minutes (588 gallons), which caused the liquid level to rise 10 inches, to a total depth of 74.5 inches (2 inches above the invert). This corresponds to 58.8 gallons per inch. Twenty-three and one-half (23.5) hours later the liquid level in the trench had dropped 8.5 inches, indicating that approximately 500 gallons had been absorbed. Since the tank was not pumped, the trench also absorbed the water used by the occupants during that 23.5 hour period, which was probably somewhere from 150 to 200 gallons. Based upon this data, it was determined that the absorption rate of the trench exceeds 450 gallons per day, as required for a 3 bedroom house. NOTE: The adequacy of a septic system is influenced by numerous factors, including, but not limited to, seasonal surface water infiltration, groundwater variations, septic system maintenance (frequency of septic tank pumping, usage of biological additives), condition of drainpipe and pipe joints (which can be damaged by seismic activity and deteriorate with age), type of substances deposited in septic system (cigarette butts, sanitary napkins, misc. objects), and the amount of water being introduced on a continual basis. Consequently, the results of this adequacy test are only valid for the specific day of the test. Furthermore, because of the limited nature of this investigation, it is possible that there are hidden defects which may not have been detected. No warrantee is made regarding the future performance of this well or septic system D. SEPARATION DISTANCE FROM ABSORPTION SYSTEM TO SURFACE WATER: It was noted during our investigation that a water was running (shallow trickle) in the ditch which parallels the east side of Park Hills Drive. The ditch is only about 93 feet from the trench on the subject property. This "surface water" was not noted on the initial inspection report, or any of the previous HAA evaluations (3/9/92, and 6/15/94) for this property. A conditional HAA was issued by your department on 8/28/97, with the stipulation that the subject waiver be applied for. The specifics regarding this waiver are summarized as follows: It was noted during our initial investigation (8/4/97) that a water was running (shallow trickle) in the ditch which parallels the east side of Park Hills Drive. The ditch is only about 93 feet from the trench on the subject property. This "surface water" was not noted on the initial inspection report, or any of the previous HAA evaluations (3/9/92, and 6/15/94) for this property. On 8/28/97, Jim Cross and I made a site visit, during which time he made the determination that it is a "surface water". Attached is a sketch which shows the general topography from the septic system to the ditch. The ground is generally flat for about 28 feet, and then it slopes downward (towards the ditch) at approximately 26%. In order for wastewater from the drainfield to reach the surface water, the system would have to overflow, or daylight. The trench sump is in a highly visible location, near the driveway, so if wastewater did overflow/daylight, it would not go unnoticed for any significant period of time. The aesthetics associated with the overflow would motivate the homeowners to take corrective action rather quickly. The ground from the trench to the ditch is heavily vegetated, and would provide some filtration of the wastewater, prior to it reaching the ditch; however, if untreated wastewater is running over the ground surface into the ditch, it is going to be a problem if the separation distance is 93 feet, or 100 feet. I located the West lot corner common to lots 4 & 5, and measured with a rag tape the indicated distances to the two test holes. Due to the snow cover I could not verify that I arrived at the identical locations, but Ms. Cable indicated that she had seen evidence of testholes at these general locations. Corwins testhole on lot 4 is approximately 50~60 feet distant from the septic system on lot 5, and approximately 6 feet higher in elevation. Both the house and the septic system on lot 5 is located on a ridge with a relative steep slope to the southeast. Corwins testhole seems to be located at the toe of the ridge or somewhat up on the ridge. The soil as reported by Corwin is what I would expect as being typical of a ridge like this^ Testhole AWM-1; or -3, as may be, seems to be located off the ridge, 50 feet to the west of the septic system and at a ground elevation 6 feet or more below the ground at the system. It seems very unlikely that the groundwater table will follow the groundsurface across the ridge" Therefore I am confident that the conditions shown in Corwins testhole is representative of the conditions at the septic system. At the beginning of the test water level was found at 68 feet below top of casing. At a pumping rate of 6 gal1ons per minute the water level dropped to 150 feet after 70 minutes of pump irill. At this level the pump produced 5 gpm with the water level remaining at 150. A total of 550 gallons were pumped. The well recovered to 75 feet within 75 pump shut off. TEST FOR E.00LI AND TOTAL NITROGEN: Water was tested for E.Coli and total nitrogen on 3/10/92 E.Coli 0. Total Nitrogen 0"19 mg., I. Max. allowable Total Nitrogen 10 mg/l. TEST RESULTS: This well meets the requirements of the Municipality of Anchorage. THIS WELL WILL PRODUCE MORE .THAN 3GALLONS PER MINUTE FOR MORE THAN -FOUR HOURS The Municipal requirement for well flow is 150 gallons of water per bedroom per day. This well exceed this requirement. The assessment of the condition of the well only to the conditions as of the day tested. The flow rate may change due to subsurface conditions that may not be observed from the surface, and changes in the land use and other factors that may impact the aquifer feeding the well.
