Septic Tank Replacement Permit Request
Ladies and Gentlemen:
I am writing to request that a permit to install a new septic tank be issued for this property. The
system currently serves an existing threebedroom house. The lot is currently developed and is
served by a community water system.
1. Tank Replacement. The original 1000 -gallon septic tank was installed in 1978. During a
recent repair it was noticed that the steel in the second compartment of the tankwas corroded
away. The remainder of the tank is also pitted and is in danger of collapsug. The proposed
replacement tank location is east of the existing tank, and centered between the existing field
and house maintaining five feet from both See the attached site plan. The contractor will install
a foundation cleanout, and double cleanouts after the tankThe existing drain field was inspected
and found to be dry on January 16, 2006.
2. Surface Water: There is no surface water within 100 feet of the proposed system.
3. Topography: The average topography in the area of the proposed septic system is
approximately 2-5 percent to the northwest.
The proposed installation will not affect the future development of tle surrounding or existing
lots. There are no wells or septic systems within 100 feet of the proposed septic tank location.

Submitted herewith is a visual soils log conducted by myself. The hole
was excavated by a backhoe to a relative depth of eleven feet. At
eleven feet bedrock was encountered.
As can be seen from the soils log, the soil absorption rate could be
classified at 125 square feet per bedroom. However, since only one
system is being installed I have designed for an absorption rate of
150 square feet per bedroom.
As noted in previously submitted data, no construction is to be permitted
within six vertical feet of the bedrock.

As certified by my seal affixed hereto and as of the validation date shown below, I verify that my investigation, based on procedures outlined
in the Certificate of On-Site Systems Approval Guidelines for this application, shows that the on-site water supply and/or wastewater
disposal system is (are) safe, functional and adequate for the number of bedrooms and type of structure indicated herein. I further verify that
based on the information obtained from the Municipality of Anchorage files and from my investigation and inspection, the on-site water supply
and/or wastewater disposal system is(are)in compliance with all applicable Municipal and State codes, ordinances,and regulations in effect at
the time of installation.
In conducting an adequacy test,I attempt to provide a thorough,conscientious engineering analysis of the system in accordance with MoA COSA
guidelines and regulations.The reported results describe the performance of the system under the conditions encountered at the time of the test,
and separation distances measured to readily identifiable features. The operational life of all wells and septic systems depend on the local soil
condition,ground water levels that may fluctuate during the year,and the water usage of the family being served by the system.These conditions
are outside the control of the evaluator of this system. All systems eventually fail and satisfactory test results do not guarantee future
performance of the system,nor do they guarantee that there are no hidden defects or encroachments.Therefore we cannot provide any warranty
for future performance, nor can we estimate remaining life of the system. The content of this report is for the sole benefit of the owner listed
above.

Approval (COSA) based only upon the representations given in paragraph 4 by an independent professional civil
engineer registered in the State of Alaska. Certificates of On -Site Systems Approval are required for the transfer of
title (except between spouses) for properties served by a single-family on-site wastewater disposal and/or water
supply system. DSD also issues COSAs upon request to homeowners. Certificates of On -Site Systems Approval
are valid for 90 days from the date of issue for properties served by a private or Class C well and may be reissued
with new water sample results. (Certificates may be reissued for a period of up to one year with valid water
samples.) Certificates are valid for one year for properties served by Class A or B wells or a public water system.
The Municipality of Anchorage is not responsible for errors or omissions in the professional engineer's work.

Thunderbird Heights, Block 3, Lot 6, Grid NW1865, 27336 Golden Eagle Court
The Right of Way Division has reviewed a request for a letter of non -objection to an existing septic vent
pipe, which encroaches approximately 9' into the 10' Utility Easement running south to north along the
western property line. On the as -built survey dated October 7, 2003, submitted with the request, the
petitioner has shown the septic vent pipe.
This letter of non -objection is issued with stipulations, and by using it the petitioner is agreeing to the
following:
1. Municipality of Anchorage (MOA) will be held harmless, now and forever, for any damages or
injury to any person as a result of the encroachments.
2. All applicable codes and regulations will be observed and maintained within the easement.
3. This letter of non -objection will in no way preclude MOA from full use and enjoyment of its
rights within any portion of the easement.
4. Additional and extraordinary costs incurred during any future required construction, repair or
reconstruction of MOA improvements to accommodate any or all of the encroachments shall be
paid by the property owner.

In conducting this evaluation, AKWWC, inc. attempted to provide a thorough,
conscientious engineering analysis of the system in accordance with ADEC and MOA
DSD Guidelines & Regulations. The reported results described the performance of the
system under the conditions encountered at the time of the test, and separation
distances measured to readily Identifiable features. The operational life of all wells and
septic systems depend on the local soils condition, groundwater levels that may
fluctuate during the year, and the water usage of the family being served by the system.
These conditions are outside the control of the evaluator of the system. Satisfactory test
results do not guarantee future performance of the system, nor do they guarantee that
there are no hidden defects or encroachments. AKWWC, Inc. can therefore not provide
any warranty or future estimate of how long the system will continue to meet the
operational requirements of the ADEC or MOA DSD. The content of this report is for

AWWU sanitary sewer is not available to the lot. AWWU water mains are located within the
Thunderbird Drive and Golden Eagle Court rights-of-way.
AWWU issues this letter with stipulations to allow the existing encroachment and by using it
property owners are agreeing to the following:
1. AWWU will be held harmless, now and forever, for any damages or injury to any person as a
result of the encroachment;
2. All applicable codes and regulations will be observed and maintained within the easement;
3. This letter will in no way preclude AWWU from full use and enjoyment of its rights within any
portion of the easement; and,
4. Additional and extraordinary costs incurred during any future required construction, repair or
reconstruction of AWWU facilities to accommodate the encroachment shall be paid by the
property owner.

This letter of non -objection in no way precludes GCI Cable from full use and enjoyment
of any rights it may have within any portion of the utility easement and or the right-of-
way, including; unlimited access for servicing its facilities. Also any additional and
extraordinary costs incurred during any future required construction, repair or
reconstruction of GCl's facilities to accommodate any or all of the encroachments shall
