Dear Mr. Cross:
The purpose of this letter is to respond to your letter dated June 8, 2001 regarding the
subject work. Mr. Richard Powers, owner of the property, reinitiated my services to
provided the necessary information and obtain final approval for the on-site sewer
system. Enclosed are the following documents for your review:
1. Inspection Report
2. Additional Percolation Test
3. Asbuilt location plan
The construction of the sewer system was inspected three separate times. The first
inspection was during the exposure of the native receiving soils. The second inspection
was for placement of bedding, pipe and tank and a final inspection after placement of
mounding materials.
Based on my telephone conversation yesterday with one ofyou staff, it is our
understanding that no additional fee is required.

Dear Richard & Emma:
An On-Site Water/Wastewater Permit, number SW000210, issued by this office for a single-family
system, will expire on July 6, 2001. This permit was valid for 365 calendar days.
If this was a well permit and the well has been drilled, a well log must be sent to this office for
documentation of the installation and to close the permit.
If this permit was for a wastewater disposal system, an original as-built inspection report must be sent to
this office for review, approval and documentation. This as-built inspection report must be signed by
the licensed Professional Engineer who inspected the installation of the system. As-built inspection
reports are required to be submitted within 30 days of the completion of the system.
If no system was installed under this permit, and you are still planning to install a well or wastewater
disposal system, a new permit must be obtained from this office. A new permit may be issuedfiee of
charge for a second year if the application for the renewal is received on or before the date of expiration
of the original permit.
When applying for a new permit, the fees are: $320.00 for a ~vastewatcr permit and $120.00 for a well
permit.

Gentlemen;
We received your review comments for the subject property. Attached for your review
and approval are revised documents consisting of:
Soil Log - Percolation Test Report for TP-1 and TP-2 located at the new
and alternate disposal field.
Site Plan showing the location of TP-2.
Sewer system details and calculations
It is our understanding that the groundwater monitoring will not be needed providing the
absorption field is raised 12 inches and sand filter is placed under the sewer rock. The
details now show sand filter material below the sewer rock. The field has also been
raised 12 inches. I have specified MOA approved sand filter material from T & J's
located on the Parks Highway.
All of the above information has been originally signed.
A typo occurred in my letter to you dated June 13, 2000. The second sentence in the
third paragraph should read, "The lot is covered with native birch and spruce and slopes
to the south and southeast at approximately 2 to 4 percent".

Dear Mr. Cross,
The purpose of this letter is to request your review and approval to construct a on-site
well and sewer system on the subject lot. Attached are the following items:

The lots to the north and east are developed with single-family residences. The new well
and existing wells in the area are greater than 100 feet from the new septic system or the
alternate absorption field location.
The area of the subject lot is 2.5 acres. The lot is covered with native birch and spruce
and slopes to the south and southeast at approximately 43 percent. There are no areas of
excessive slopes (greater than 25%), water bodies or well within 100 feet of the new
sewer system. The new sewer system will consist of a 1000 gallon septic tank, cleanouts
outside the house and within the absorption field and a 363 square feet mounded
absorption field. The new sewer system is not a replacement system.
We do not anticipate any negative impacts to adjacent properties or conflicts with the
current drainage feature on the subject site.
