Subject: Prospect Heights #1 B5 UB - 9600 Slalom Dr COSA testing
Dear Lois:
On June 5, 2024 Forge Engineering arrived at the property at 9600 Slalom Drive for a full COSA
test. We found the static water level in the well to be at 63' and the production rate to be 5.0 gallons
per minute after the multi -hour flow test. The well had a sanitary cap on it and the wires were
properly protected in conduit. We measured the well casing at 8" above grade- which doesn't meet
the current code requirement of 18". The well appears to have been drilled before 1996, in which
case the requirement would be 12". We took a water sample for analysis and submitted it to the
lab.
We measured the levels of liquid in the septic tank and determined that to be holding water
adequately, even at 24 years old. However, when we measured levels in the crib, we found it to be
in failure. According to septic records the crib is 9' deep. We could visibly see in the pipe that the
level of liquid was at the top of the crib, and we measured 95" without pushing too hard into a
level of solids at the bottom.
After some initial discussion and consultation, we agreed to have the septic tank pumping company
(Isaac's) attempt to pump the crib empty at the same time they scheduled to pump the septic tank.
We hoped that this would help to unblock the buildup of biomat in the rock in the crib. They pulled
as much liquid as they were able on that trip. Isaac's scheduled a second trip to try to pump more
effluent out of the crib, and the engineer visited the site to investigate and talk about options. We
discussed at renewal treatments of the crib, although those are not guarantees. We agreed to wait
as long as we could before scheduling a retest of the crib, with the intent of avoiding expensive
septic system replacement.
Subsequently, after further discussion and consultation, we understand that you have decided to
look at other alternatives. We hope that this documentation will help your next engineer in his
pursuit of a working system and a COSA for your property.

Septic System Design and Permit Application
Impacts to Adjacent Properties
Dear Onsite Services Engineer:
The septic tank serving the four -bedroom home on Lot 4B, Block 5, Prospect. Heights
Subdivision Addition No. 1 has failed and must be replaced immediately to prevent a
potential health hazard and damage to private property. We are therefore applying for
a permit to construct a new septic tank on the lot to serve the home. The attached Site
Plan and backup documentation identify the location and configuration of the existing
septic system and the location of the new tank. It also shows the location of the
existing well which is currently closer than 100' from the existing tank. Sufficient room
exists on the lot to move the tank out of the protective radius.
The ground surface on the lot slopes dramatically from east to west from the location of
the house. The absorption crib is approximately 20' lower than the elevation of the
house. The tank will be placed into the slope, but will have a minimum of 4' of cover or
2' of cover and 2 of direct bury insulation to provide protection from freezing. The
existing septic tank will be pumped, crushed and buried on site.
If the system is constructed in accordance with our design the following statements
apply:
The system, if constructed as designed, will have no adverse impact on the wells
in the area or those to be constructed in the future.
2. The system, if constructed as designed, will have no adverse impact on existing
septic systems in the area or those to be constructed in the future.
3. The system, if constructed as designed, will have no adverse impact on reserve
space, either surface or subsurface, on any lots located in the area.
4. The system, if constructed as designed, will have no adverse impact on drainage
patterns in the area.

In conducting this evaluation, GEG provided an engineering evaluation of the well and/or septic system in
accordance with the guidelines and regulations established by the Municipality of Anchorage and industry
practices. The reported results describe the condition of the system/s on the date/s of the evaluation.
Separation distances were measured to readily identifiable features. Hidden defects or encroachments may

A water sample revealed a nitrate concentration of 7.96 milligrams per liter (mg/Q.
The Environmental Protection Agency (EPA) has established a maximum
contaminant level (MCL) of 10.0 mg/L for public drinking water systems. While
private wells are not subject to this regulation, EPA standards are based on existing
health information and can therefore be used to gauge the relative quality of water
from private wells. Since nitrates are known to slowly increase, we recommend
you monitor the water quality. Please see the attached "Nitrate Fact Sheet" for
important information regarding nitrate.
This advisory must be attached to all copies of the subject Certificate of On -Site
Systems Approval.
Nitrate Fact Sheet
From Northern Testing Laboratories, Inc.
Nitrate is a negatively charged compound of nitrogen and oxygen, which is very soluble in water. Nitrate
is not readily filtered or otherwise removed in the soil and can pass rapidly into ground water wells.
SOURCE: Nitrate is a major component of fertilizer and wastewater. Often the nitrate is in the form of
ammonia or protein first, which through contact with oxygen and certain bacteria, converts to the
oxidized form known as nitrate. Sources of nitrate from wastewater include urea, ammonia cleaners,
food solids, and bacterial cells. It may also result from the breakdown of organic matter buried in the soil.
TOXICITY: Nitrate is generally not toxic to adults or children over the age of two or three years, but is
associated with a potentially fatal infant disease called methemoglobinemia. In the digestive system of
young children, nitrate converts to nitrite, which can pass through the intestinal wall into the blood
stream. There it combines with the hemoglobin and interferes with the ability of the blood to carry
oxygen. For this reason, methemoglobinemia is referred to as "blue baby" disease. The EPA limits the
concentration of nitrate in public drinking water supplies to 10 mg/L. The standard has been lowered
from a previous level of 45 mg/L set by the US Public Health Service and the World Health Organization.
TREATMENT: due to its solubility in water and negative ionic charge, filtration and other common home
water treatment systems such as softening or iron filtration does not readily remove nitrate. The best
method for limiting nitrate in well water is source control. This can include avoiding overdosing of
fertilizer near the well and maintaining good separation distances between septic tank leach fields and
the well. A special anion exchange filter that contains a media with a strong affinity for negatively charged
ions in water, or by a reverse osmosis treatment system or distillation can remove nitrate.
TESTING: Nitrate analysis is usually done by one of the several "wet chemical' methods using a
spectrophotometer to read the final color endpoint. Specific ion electrodes also can be used to detect
the activity of nitrate in water. This laboratory uses several different wet chemical methods approved
under the public water supply laboratory certification program. They also have test kits available, which
the laboratory uses to perform an inexpensive "screening test", and with which the homeowner can
monitor the change in nitrate levels from their well. They recommend comparing the test kit results
against a certified analysis from the lab occasionally to verify the accuracy of the kit. We recommend
using a specially prepared bottle that has been rinsed in hydrochloric acid for collecting samples.

Subject: Prospect Heights #1 B5 UB - 9600 Slalom Dr COSA testing
Dear Lois:
On June 5, 2024 Forge Engineering arrived at the property at 9600 Slalom Drive for a full COSA
test. We found the static water level in the well to be at 63' and the production rate to be 5.0 gallons
per minute after the multi -hour flow test. The well had a sanitary cap on it and the wires were
properly protected in conduit. We measured the well casing at 8" above grade- which doesn't meet
the current code requirement of 18". The well appears to have been drilled before 1996, in which
case the requirement would be 12". We took a water sample for analysis and submitted it to the
lab.
We measured the levels of liquid in the septic tank and determined that to be holding water
adequately, even at 24 years old. However, when we measured levels in the crib, we found it to be
in failure. According to septic records the crib is 9' deep. We could visibly see in the pipe that the
level of liquid was at the top of the crib, and we measured 95" without pushing too hard into a
level of solids at the bottom.
After some initial discussion and consultation, we agreed to have the septic tank pumping company
(Isaac's) attempt to pump the crib empty at the same time they scheduled to pump the septic tank.
We hoped that this would help to unblock the buildup of biomat in the rock in the crib. They pulled
as much liquid as they were able on that trip. Isaac's scheduled a second trip to try to pump more
effluent out of the crib, and the engineer visited the site to investigate and talk about options. We
discussed at renewal treatments of the crib, although those are not guarantees. We agreed to wait
as long as we could before scheduling a retest of the crib, with the intent of avoiding expensive
septic system replacement.
Subsequently, after further discussion and consultation, we understand that you have decided to
look at other alternatives. We hope that this documentation will help your next engineer in his
pursuit of a working system and a COSA for your property.
