Attached is the revised permit application to upgrade the septic system on the subject
lot. We have added dimensions from the mobile home to the septic tank. Dimensions
from the lot lines were previously shown. We have also added slope information
showing the lot to be virtually fiat. An easement has been executed allowing the
absorption trench on the adjoining lot. A copy of the easement is attached. We have
also included the waiver to the Class C Well, which was finally issued by ADEC. The
permit application has been modified to show a two-bedroom home. All comments
have been incorporated into the design and permit application. Please let me know if
you have additional questions.

Review
Dear MrJ Anderson:
This is response to your letter received in this office on June 20, 2002 in which you requested the
Department to grant separation distance waivers between the proposed wastewater disposal
system to be located on Lots 23 and 24, Block 5, Sand Lake Subdivision, #2 and the Class C
public water system source well located on Lot 21, Block 5, Sand Lake Subdivision #2. I have
completed my review of the Department's files on the existing public water system, the
previously issued separation distance waivers in Block 5, Sand Lake Subdivision, and the
submitted information. Based on this review, I believe that the requested waivers are reasonable.
As noted above, you have requested the Department to grant separation distance waivers
between the proposed wastewater disposal system to be located on Lots 23 and 24, Block 5, Sand
Lake Subdivision, #2 and the Class C public water system source well located on Lot 21, Block
5, Sand Lake Subdivision #2. Noting not only the depth of the source well on Lot 21 bm other
source wells in the area, the continued low levels of nitrates, and the lack of total coliform
bacteria, the waiver requests are reasonable. Therefore, in accordance with the Drinking Water
Regulations [18 AAC 80], the following waiver is being granted:
· The horizontal separation distance between the Class C public water system source
well located on Lot 21, Block 5, Sand Lake Subdivision, #2 and the proposed septic
tank on Lot 23, Block 5 Sand Lake Subdivision #2 serving a single family home has
been waived from 150 feet to 1 l0 feet.
· The horizontal separation distance between the Class C public water system source
well located on Lot 21, Block 5, Sand Lake Subdivision, #2 and the proposed
drainfield located on Lot 24, Block 5, Sand Lake Subdivision, #2, serving a single
family home has been waived from 150 feet to 125 feet.

The above noted distances were taken from your submittal. If the proposed wastewater disposal
system located on Lots 23 and 24, Block 5, Sand Lake Subdivision #2 fails or needs to be
upgraded, this waiver becomes invalid.
For this waiver to remain valid the following items will need to be provided:
1. A copy of the record drawings of the installation of the proposed septic tank and soil
absorption systems to ensure that the above noted separation distances are being
maintained.
2. The copy of the Inventory and Source Registration for Class C Public Water System
with Sections A1 and D completed will need to be submitted to this office.
3. Authorization from the owner of Lots 23 and 24, Block 5, Sand Lake Subdivision
allowing you to apply for a separation distance waiver from this Department.
The issuance of these separation distance waivers does not imply the granting of any additional
authorizations, nor obligate any local, state, or federal regulatory body to grant required
authorizations.

Septic System Design and Permit Application
Impacts to Adjacent Properties
Dear Onsite Services Engineer.
The existing septic system on Lot 23, Block 5, Sand Lake Subdivision has failed and
must be replaced to avoid damage to private property. We are therefore requesting a
permit be issued for the construction of a new septic system to serve the existing
mobile home and work shop. The attached Site Plan and backup documentation
identify the location and configuration of the new septic system and the parameters
used in the design. Also identified on the plans are the test hole locations and the
locations of the wells and septic systems on the adjacent lots. Drainage arrows are
also shown and current drainage patterns will be maintained al'l. er construction. Both
Lots 23 and 24 have the same owner, The absorption trench will be constructed on Lot
24 with the septic tank on Lot 23. The property is served by a Class C Well located on
Lot 21. A waiver of the protective radius to 100' was requested from ADEC over a
month ago. We ara confident the waiver will be issued and a copy will be forwarded to
you when it becomes available.
The test holes placed near the location of the proposed absorption trench indicated
clean well graded well graded sand with varying amounts of gravel and silt and a
percolation rate of 1 to 2 minutes per inch. No groundwater was found dudng the
placement of the test hole and none has been noted since. We are therefore proposing
to construct a 3' wide deep absorption trench with 8.5' of drain field rock beneath the
distribution pipe. The total depth of the trench will be 12' with the distribution line at 3.5'
below the surface. The total length of the trench will be 30'.
The ground surface on the lot slopes as shown on the attached Site Plan with fairly
shallow to no slope from west to eas. t in the area of the new trench. The new
absorption trench will be constructed ,n a fiat area in conformance with Municipal
requirements and parallel to ground contours where possible. Ail components of the
new trench will be constructed a minimum of 100' from the Class C Well on Lot 21. The
system will also be constructed a minimum of 100' from any surface water in the area.

If the system is constructed in accordance with our design the following statements
apply:
1. The system, if constructed as designed, will have no adverse impact on the wells
in the area or those to be constructed in the future.
2. The system, if constructed as designed, will have no adverse impact on existing
septic systems in the area or those to be constructed in the future.
3. The system, if constructed as designed, will have no adverse impact on reserve
space, either surface or subsurface, on any lots located in the area.
4. The system, if constructed as designed, will have no adverse impact on drainage
patterns in the area. The current drainage pattern will be maintained.

ON-SITE WASTEWATER DISPOSAL SYSTEM
CONSTRUCTION AND MATERIAL SPECIFICATIONS
SUBJECT: LOTS 23 & 24, BLOCK 5, SAND LAKE SUBDIVISION
GENERAL:
The scope of this project includes the pumping, crushing and
backfilling of the existing septic tank and absorption cdb. It also
includes the procurement and placement of a new 1,250 gallon septic
tank at the location shown on the site plan and the construction of a
30' long by 3' wide by 8.5' effective depth absorption trench also
shown on the Site Plan. The total depth of the trench will be 12' below
existing ground and the distribution pipe will be placed at 3.5' below
the surface. All components of the absorption trench must be
constructed a minimum of 100' from the well on this and all adjacent
lots.
Construction shall be in accordance with the approved site plan,
design drawings, Municipal Permit with any special provisions or
conditions, and all applicable State and Municipal Wastewater
Disposal Regulations.
The Contractor shall be responsible for obtaining all underground utility
locates and for the layout of the septic system and verification of the
location of all lot lines.
Unless specifically agreed otherwise, the contractor shall be
responsible for final grading areas subsequently depressed from soil
settling. Property owner shall be responsible for revegetation of
affected areas unless specifically agreed otherwise.
Contractors installing wastewater disposal systems must be certified
by the Municipal Department of Health and Human Services for
system installations. Owners installing their own systems must receive
pdor approval from D.H.H.S. before beginning system installation.
SEPTIC TANK INSTALLATION
1. The 1,250 gallon septic tank must be procured from an approved
septic tank manufacturer and installed at the location shown.
A septic tank is to be constructed by a certified septic tank
manufacturer. Construction shall include two 4" cleanouts for pumping
access.
3. The septic tank shall be sufficiently bedded to prevent settling or
shifting of the tank.
4. All standpipes on the septic tank shall extend a minimum of 12 inches
above final grade.
5. Tanks installed without 4' of cover shall have a minimum of 2" of direct
budal insulation.

6. A foundation cleanout shall be installed one to four feet from the
building foundation. Two cleanouts are required between the tank and
the drainfield.
7. Final grading over the tank shall be such that a positive slope exists
away from the septic tank.
DRAINFIELD CONSTRUCTION:
1. The drainfield shall be constructed to the dimensions shown on the
design. The bottom of the trench shall be within 2" of level.
Distribution piping must be placed level with perforations down atop a
level bed of drainfield rock. Rock should then be placed over the pipe
to provide a minimum of 2" of cover.
3. A silt barrier or geotextile fabric must be placed between the drainfield
rock and the natural soil backfill.
4. Monitor tubes must be 4" in diameter and installed at the locations
shown on the design. The portion below ground must be perforated.
Contractor shall vedfy the septic tank and drainfield are a minimum
100' away from any private water wells in the area, 150' from a Class
'C" Well or 200' from any community well.

Direct bury insulation must be placed over the distribution system if
less than 3' of backfill depth is available. Finish grade over the trench
must be mounded to prevent settlement or depressions.
7. Grade area surrounding the absorption trench to drain away.
A minimum 2' of accepting soil is required below the drainfield rock for
a 5' wide trench. Contractor shall vedfy this condition prior to
placement of the rock. Ail pockets of unacceptable materials must be
removed and replaced.
MATERIAL SPECIFICATIONS:
1. Septic tanks must be constructed by a municipally approved septic
tank manufacturer.
2. The following pipe materials are approved for use in septic system
installations in the Municipality of Anchorage:
Cast Iron (perforated and solid), ASTM D3034 or P.V.C. (perforated
and solid), ASTM F810 or H.D.P.E. (perforated, but not solid) and
ASTM D2662 or A.B.S. (perforated and solid).
Insulation shall be at least 2" thick extruded direct burial polystyrene
(Dow Chemical Co. Styrofoam HI or equal).
Septic tank inlets and outlets shall be fitted with watertight couplings
(Caulder, Femco, or equal).

5. A permeable geotextile fabdc (Typar, Mirafi or equal) must be installed
between the final drain rock layer and the native soil layer.
6. All drain rock shall be .5" to 2.5" in diameter with less than 3% passing
the #200 sieve.
INSPECTIONS:
Municipal Ordinance requires a minimum of two inspections. These
inspections must be conducted under the supervision of a professional
engineer registered in the State of Alaska. The first inspection must be
conducted after the excavation of trenches, beds or pits and before the
installation of any gravel. A septic tank may be set in place, but may not
be backfilled.
The second inspection must be conducted after the placement of the
geotextile fabdc, gravel, distribution piping, standpipes, cleanouts and
insulation. No backfill should be in place at the time of inspection.
Contractor shall provide a copy of all field survey layout and construction
notes for use in preparing the certified as-built of the completed system.
