An On-Site Water/Wastewater Permit, number SW000 484, issued by this office for a single-family
system, will expire on November 16, 2001. This permit was valid for 365 calendar days.
If this was a well permit and the well has been drilled, a well log must be sent to this office for
documentation of the installation and to close the permit.
If this permit was for a wastewater disposal system, an original as-built inspection report must be sent to
this office for review, approval and documentation. This as-built inspection report must be signed by
the licensed Professional Engineer ~vho inspected the installation of the system. As-built inspection
reports are required to be submitted within 30 days ofthe completion of the system.
If no system was installed under this permit, and you are still planning to install a well or wastewater
disposal system, a new permit must be obtained from this office. A new permit may be issued free of
charge for a second year if the application for the renewal is received on or before the date of expiration
of the original permit.
When applying for a new permit, the fees are: $320.00 for a wastexvater permit and $120.00 for a well
permit.

AT ANY TIME IN THE FUTURE, IF THIS CHANNELIZATION DOES NOT EXIST THEN THE SURFACE WATER
WAIVER WILL BE VOID AND THE WASTEWATER SYSTEM WILL NOT BE APPROVED BY THIS DEPARTMENT.

REFERENCE: Lot 20, Block 2, Lake Ridge Terrace Subdivision
It is requested that you approve the proposed septic leachfield upgrade to be
installed under existing permit #SW000484 to serve the existing four bedroom
dwelling on the referenced property. A septic tank & lift station was already
installed under this permit on 11/29/00.
One test hole was excavated, and a percolation test was performed on 7/11/81. The
approximate location of the test hole is located on the attached site plan. Ground water
was monitored and was found at 7' as shown on the attached soils logs.
We do not anticipate any adverse effects on neighboring wells, septic systems, reserve
areas or drainage patterns by the installation of the proposed septic system. The
constxuction of this system will not prevent any future development on any of the adjacent
properties.

GENERAL STATEMENT OF WORK
The work and plans covered by these specifications consist of furnishing all labor,
materials, tools, txansportation, and equipment necessary to complete an Intermittent Dosing
Sand Filter system as described herein and on the attached drawings.
All work under this project shall comply with current Municipality of Anchorage (MOA)
Regulations.
4 Bedroom House 600 GPD
Use 18' x 20' Bottomless Sand Filter and absorption area (360 S.F. required)
Sand filter to bo constructed on accepting soil with all organics removed. (bottomless
only.)
SITE INVESTIGATION
The Contractor must carefully examine the project documents, have full knowledge thereof,
have investigated the site, and be satisfied with the conditions affecting the work.
Conditions include, but are not limited to those bearing upon transportation, disposal,
handling and storage of materials, availability of labor, water, electric power, roads, and
uncertainties of weather, physical conditions at the site including all underground utilities,
the conformation and condition of the ground, the character of equipment and facilities
needed preliminary to and during prosecution of the work. The Contractor must be satisfied
with the character, quality and quantity of surface and subsurface materials or obstacles to
be encountered. Any failure by the Contractor to become acquainted with the available
information will not relieve him/her from responsibility of performing the work as stated in
these specifications and drawings.
Location of utilities will be the contractor's responsibility.

B. Materials
1. Tank

2. Manhole Riser or Lift Station Tank
a. An additional second compartment manhole riser shall be capable of being equipped with
the following:
1. A junction (Nema 4x) box or equal, bonded or attached to
the pump basin.
2. UL listed electrical cord grips, installed in the J-box.
b. An additional tank 0VI.O.A. approved) may be provided for the
effluent pumping assembly and the items in (a.) above.
Effluent Pumping Assembly, Controls, Alarm & Piping
a. Effluent pumping assembly shall be of approved materials, including the following:
I. A 110 volt or equal U.L. Listed effluent pump.
2. High-density PVC cylinder housing the pump, with level
controls and screen, serving as a baffle to prevent the screen
from clogging.

3. All piping shall be PVC or other non-corroding material.
b. Pump controls and alarm system shall be an approved system with the following options
consisting of:
I. Event counter
2. Elapsed time meter
3. Program timer
4. 10 watt heater
Remote alarm panel option with a minimum of 80 DB
sound pressure at 24", operating temperature 30AC to
65AC, continuous sound to be located in the home.
6. Oil-tight visual alarm with push-to-silence feature.
Automatic audio-alarm reset.
15 amp motor rated toggle switch, double pole, double
throw with three positions; manual (man), automatic (auto)
and center (off) (H.O.A.).
8. Nema 4x-rated, fiberglass, or equal, enclosure with hinged
cover.
9. All controls to be installed as per manufacturer's
recommendations.
10. Control panel to be installed on outside wall of home within
sight of septic tank/lift station.
Piping from the septic tank to the sand filter is to be 1 1/4" PVC schedule 40 solvent ~veld
piping properly bedded as described in Materials Specifications and sloped to drain back
to the lift station. Insulation beard 2" thick and 2' wide centered over pipe is to be placed
where soil cover is less than 4' depth over pipe.
d. The air line from the house to the filter is to be HDPE arctic insulated pipe 3/4"-2" SDR-
I 1. The compressor will have a gauge (0-I0 psi) and be installed in a heated area, crawl
space, or garage.

4. Air Compressor
a. Air compressor shall bca THOMAS LINE^R SERIES model number 5060-A or $070V-A or
M.O.A. approved equal.
CONSTRUCTION SEQUENCE
In order to insure proper installation, the system is recommended to be installed in the following order:
I. Sewer line from house to tank
II. Septic tank/lift station
IlL Sand filter
IV. Leachfield bed (if required)
V. Final grading
CONSTRUCTION
1. Construction Instruments
Contractor shall provide instruments such as transits, or levels, for establishing proper
elevations and levelness ofpipo or materials.
2. Pipe Laying
Ail pipe shall be laid with material free of large or sharp rocks and organic matter.
Each section of pipe shall be handled carefully and placed accurately. The spigot end shall
be fully inserted pointing in the direction of flow and glued in place to make a water tight
seal.
Each section of pipe shall be properly supported to insure true alignment and an invert
which is smooth and free from roughness or irregularity.
At all times, when work is not in progress, open ends of pipe and fittings shall be closed
so that no undesirable substance will enter the end of the pipe or fittings.

3. Bedding of Pipe for Sewer Lines
The pipe should be bedded with materials free of large or sharp rocks and organic matter,
compacted. Native materials may be used as back fill material above the pipe, provided it is
free of large rocks and organic matter.
All lift stations shall be installed in accordance with the manufacturers recommendations
and standards. Electrical installation is to be in accordance with the National Electric Code
(latest adopted edition) by a licensed electrical contractor, with M.O.A. inspection where
required. IfMOA inspection is not required, a licensed electrician is to provide a letter to the
Engineer stating that the system meets or exceeds all applicable codes.
4. Excavation
Excavate to proper depth to allow gravity flow, install tank level, tolerance 0.05 foot, on
undisturbed or compacted classified sand fill material.
Create a level sand surface over the tank and add 2" burial foam +35 PSI directly over
tank. Mound unclassified soil over tank with a minimum of 2 feet of cover and anti-flotation
weights that may be required in areas of shallow ground water.
5. Connections
All connections to be as per MOA requirements, PVC 3034 piping with Caulder or
equivalent couplings for inlet riser pipes.
All tank connections are to be completely water tight.
SAND FILTER
The work under this section consists of the performance of all operations pertaining to furnishing and
installing the sand filter.
A. Materials
1. Sand Filter
a. Liner (if required - not required for bottomless sand filter) - OSI Model LI or
equivalent. 30 mil PVC cut and fabricated to provide water tight enclosure of filter
sand volume. Seams to be permanent as per Manufacturer's recommendations. All
connections, such as boot flanges, through sand filter to be permanently bonded to the
liner.

b. The Contractor shall provide a washed sand to meet ASTM C-33 Concrete Sand
Specifications with the additional requirement that all but 1% of the materials
passing the #100 sieve be removed. All aand shall be approved for use by the
Municipality of Anchorage (M.O.A.). If the sample does not meet specifications, all
sand will be removed and replaced.
c. The Contractor shall be responsible for providing pea gravel that shall be a
washed material containing no fines (0% passing the #100 sieve) and a maximum
size of 1/4" diameter.
d. The Contractor shall provide the manifold assembly. The manifold assembly
shall be of approved materials with orifice shields and flushing assembly. All
drilled holes to be cleaned of burrs and pipes flushed, prior to assembly. Flushing
valve to include riser to finish grade. Lateral distribution piping is to be laid level
(@ + 0.053.
e. Side walls and top ofthe filter box to be insulated with 2" direct burial polystyrene
insulation.
f. The Contractor shall be responsible for providing filter fabric.
g. Air coil in sand filter to be geoflow WF164-24 150 LF placed 6" above bottom
of sand.
Construction of Sand Filter
The sand filter construction is to follow procedures as outlined below, as applicable to this
design.
Pea gravel and filter sand are to be hand leveled to proper depth (tolerance within 0.05
feet) in filter, taking care not to allow mud or fine soil material into the filter or gravel layer.
This would include any soil material picked up by excavation machinery buckets while
loading specified material. The Contractor should estimate filter material volumes such that
allowance is made for wastage of material in contact with the ground at site.
The area over the sand filter to be finish graded with a minimum of 2 feet ofcover and to
prevent ponding of surface water nm off. Side slopes shall not exceed 3:1.
The Engineer will reject any sand or pea gravel that does not meet specifications or is
contaminated in transit or while handling. If such material is placed in the filter, all soil
material may be required to be removed and replaced with new material.

Reference: Lot 20; Block 2; Lakeridge Terrace Subdivision
Request you grant a permit to relocate and upgrade the septic tank
and lift station, replacing the steel culvert lift station basin with a 550
Gallon Fat Albert plastic tank and the existing steel septic tank with
a 1300 gallon HDPE septic tank, to serve the existing four bedroom
dwelling on the referenced property. Also request that you waive the
following horizontal separation distances on Lot 20:
(1.) Fifty feet between the proposed Premier HDPE septic tank
and surface water (Fire Lake). The current septic tank is
located 73 feet from Fire Lake.
(2.) Fifty feet between the proposed Fat Albert 550 gallon lift
station tank and surface water (Fire Lake). The current
culvert/lift station is located 81 feet from Fire Lake.
(3.) Ninety feet between the leachfield and surface water
(Fire lake).

(4.) Zero feet between the leachfield and the common Iotline
between Lot 20 and Lot 19.
The existing culvert/lift station is located inside a 3 foot steel culvert
(which leaks) and is infiltrated by groundwater. The existing 18 year old
septic tank will be inspected, but is assumed to be leaking also. It has
been determined that the system should be upgraded to function in
compliance with Municipality of Anchorage code. These tank upgrades
should improve the current situation, since the polyethylene tanks will not
corrode and the tanks are expected to remain leak-free indefinitely.

The following is pertinent information for your consideration:
A. SURROUNDING TOPOGRAPHY:
Lot 20 is moderately vegetated, with the area in question covered with grass
lawn. Any surfacing, migrating effluent would receive some treatment as it
passes through the vegetation. Because of a retaining wall near the lake
edge, and after the installation of the proposed swale, any surfacing effluent
would have to travel in excess of 100 feet prior to reaching the surface water
of Lower Fire Lake.
B. SOIL ABSORPTION:
From the well and soils logs from Lot 20's existing well and septic systems,
the top 11 feet of soil are silt/gravel/sand mixes which should provide good
septic effluent treatment as well as keeping any spill/overflow localized by the
immediate area absorption and vegetation. These soils would minimize the
distance that the effluent could migrate, in the event of an overflow.
C. PERMEABILITY:
The referenced soils should provide a good catchment for any overflow of
surfacing effluent. Coupled with the vegetation, and proposed swale, the
soils should effectively isolate the surface water from contamination from
surfacing effluent.
D. HORIZONTAL SEPARATION:
The horizontal separation distance between the surface water (Fire Lake)
and the proposed septic tank and lift station is 50 feet. The horizontal
separation distance between the surface water and the leachfield (bed) is 90
feet.
E. ADDITIONAL CONSIDERATIONS:
(1.) The existing system has been in service for eighteen years with no
apparent adverse effects to Fire Lake, the onsite well or surrounding
properties.
(2.) The proposed upgrades would replace an aging septic tank and an
obviously perforated lift station basin, with leak-free HDPE tanks, which
would then service the existing leachfield.
(3.) The proposed tanks are being used as a long term solution to a
multifaceted problem.
(a.) Preexisting development of small lots precludes multiple
replacement sites, as well as dictates an anticipated upgrade to an

innovative leachfield system. For this reason, we are recommending
the use of a Fat Albert tank after the HDPE septic tank as a
precursor to this future upgrade. These tanks have indefinite
lifespans and are a logical choice for this situation. The primary
detractor for the use of these plastic tanks is their maximum burial
depth, which requires locating them closer to Fire Lake.
(b.) The requirement for a long-lived tank and high ground water
levels indicate the need for an especially noncorrosive tank, negating
future worries of a leaking tank, presenting a contamination hazard to
Fire Lake, despite the requested horizontal separation distance of 50
feet.
(c.) The presence of an alarmed residential sewage pump in a 30
gallon basin in the basement of the home, as well as an alarmed lift
station tank, with 300 gallons of surplus storage (above and beyond
the MOA requirement of 250 gallons of storage) provide an
additional measure of protection against surfacing effluent.
We do not anticipate any adverse effects on neighboring wells, septic
systems, reserve areas, or drainage patterns by the granting of the requested
waiver. The requested upgrade to this system's tanks will not prevent future
development on adjacent properties.

GENERAL:
The scope of this project includes the upgrade of the existing system,
with replacement of the lilt station with a 550 gallon 'Fat Albert' tank,
and replacement of the 1500 gallon septic tank with a 1300 gallon
Premier Polyethylene septic tank to serve the existing four bedroom
residence located on the referenced property.
Construction shall be in accordance with the approved site plan and
design drawings, Municipal permit with any special provisions or
conditions, and all applicable State and Municipal Wastewater Disposal
Regulations.
The contractor shall be responsible for obtaining any necessary
underground utility locates.
Unless specifically agreed otherwise, the property owner shall be
responsible for final grading areas subsequently depressed from soil
settling.
Contractors installing wastewater disposal systems must be certified by
the Municipal Health Department for system .installations. Owners
installing their own systems must also receive prior approval from the
Municipal Health Department.
SEPTIC TANK INSTALLATION:
A septic tank is to be constructed by a certified septic tank manufacturer.
Construction shall include two 4" cleanouts for pumping access.

The septic tank shall be sufficiently bedded to prevent settling or shifting
of the tank.

All standpipes on the septic tank shall extend a minimum of 12 inches above final
grade.
4. Septic tanks installed with less than 4 ft. ofcover shall be insulated.
A foundation cleanout shall be installed one to four feet from the building
foundation. In the line between the tank and the leachfield there shall be two
adjacent cleanouts (unless an effluent pumping system exists within the septic tank).
These cleanouts shall be located on undisturbed soil not more than 10 fi. from the
tank. The first cleanout, in line, shall be to clean toward the leachfield. The second
cleanout shall be to clean toward the septic tank.
Final grading over the septic tank shall be such that a positive slope exists away
from the septic tank.
MINIMUM MATERIAL SPECIFICATIONS:
1. Any septic tank proposed for installation must be constructed by a Municipal
approved septic tank manufacturer.
2. The following pipe materials are approved for use in septic system installations in
the Municipality of Anchorage:

Use of a type of pipe other than listed above must be approved by the inspecting
engineer.
Insulation shall be at least 2" thick extruded direct burial polystyrene (Dow
Chemical Company Styrofoam HI or equal).
Septic tank inlets and outlets shall be fitted with watertight couplings (Caulder,
Femco, or equal).
A permeable nontoxic silt barrier (Typar 3401, Mirafi 140N, or equal) must be
installed between the final leachfield gravel layer and the native soil backfill.

6. All leachfield gravel (sewer rock) shall be 0.5"-2.$" screened gravel with less than
3% passing the//200 sieve.
When sand is being used as a filter material, its gradation specifications must
conform to current M.O.A. or D.E.C. requirements, which ever requirement applies.
INSPECTIONS:
Typically there will be a minimum of three (3) inspections required during the installation
of the wastewater disposal system. These inspections will occur as follows:
The first inspection must be conducted after the excavation of ditches, pits,
trenches, or beds and before the installation of any gravel. A septic tank
may be set in place, but may not be backfilled before this inspection.
The second inspection must be conducted after the placement of the silt
barrier, gravel, distribution lines, standpipes, cleanouts, and insulation, but
before the placement of any other backfill.
The final inspection is to occur upon final grading of the property.
Often there will be more than these 3 inspections required. Especially with the installation
of multiple trenches, sand filters, pressurized distribution systems, etc. Thus, the inspecting
engineer is to be contacted at least 24 hours prior to the start of construction. If necessary, a
pre-construction meeting will take place on-site. The inspecting engineer will not
coordinate, direct or control in any way the contractors activities.
The owner shall contract with the contractor to perform the work outlined in these
specifications and plans and in accordance with the attached M.O.A. permit. There will be
no contractual arrangement existing between the contractor and S & S Engineering. S & S
Engineering shall be the owner's representative and will inspect the work as stated above to
document the contractors activities. Final acceptance of the contractors work rests with the
owner and the M.O.A.
S & S Engineering shall have no liability to the owner or to others for acts or omissions of
the contractor or any other persons performing work on this project or the failure of the
contractor to carry out the work in accordance with these construction documents. S 8: S
Engineefing's inspecting engineer will not be responsible for the construction means,
methods, techniques, sequence, procedures or the safety precautions incident to this project.

The length dimension is the length(in feet) of the trench or drainfield. The
depth of a trench or pit is the distance between the surface of the ground and
the bottom of the excavation(in feet). There is no set width for trenches.
The gravel depth is the minimum depth of gravel between the outfall pipe and
the bottom of the excavation(in feet).

Permit applicant has the responsibility to inform this department during the
installation inspections of any wells adjacent to this property and the number
of residences that the well will serve.

Backfilling of any system without final inspection.and approval by this department
will be subject to prosecution.
Minimum distance between a well and any on-site sewage disposal system is 100 feet
for a private well or 150 to 200 feet from a public well depending upon the type
of public well. Minimum distance from a private well to a private sewer line
is 25 feet and to a community sewer line is 75 feet. Well logs are required
and must be returned to this department within 30 days of the well completion.
Other requirements may apply. Specifications and construction diagrams are
available to insure proper installation.

The septic tank pumped with a receipt submitted to this
department. The total number of gallons pumped needs %o be
on %]~e .... receipt and verified by a re~istered~, . _~ engineer as to
the actual number of gallons pumped. This is %o verify the
size of the seDtic tank.
Expose the septic tank manhole 'to verify its existence.
An adequacy test needs to be performed on tJ~e existing
leaching area. Tlnis test will ~etermine if ~fhe system is
adequate according to National Standards. A listing of
private firms performing the test is enclose~]. This report
needs to b{~ submitted to this offi~e for our review.
_~e sewer system serving the white house on the neighboring
lot needs to be located so that we can measure the distance
bet¥;een your well and that sewer system. ·

Reference: Lot 20: Block 2: Lake Ridge Terrace Subdivision
A sewer system adequacy test was performed on the system located
on the referenced property as"~ou requested. The septic tank was
pumped and verified to have a capacity of approximately 1250
gallon~ The seepage pit was full of water and approximately
1000 gallons was removed° A~t000 gallons of fresh water was then
placed ba~ into the crib and after a period of 24 hours measurements
indicate approximately 225 gallons had percolated out of the crib°
It can be concluded from the above test that the septic t~nk:is
adequate for the three bedroom residencellocated on this property,
however, I regret to inform you that the seepage pit is only adequate
for slightly over one bedroom and it will be necessary for you to
have the system upgraded to satisfy the requirements for a three
bedroom home° Even though the seepage pit is not adequate for
the number of bedrooms in this house, based upon planned occupancy
for the remainder of this winter the system should provide adequate
percolation until the upgrade can be performed during the summer
of 1982.

At the request of Mr. Paul Gibbs, I performed a soil log on the
referenced property, a copy of which is enclosed for your use.
It is my understanding from talking to the Municipality of Anchorage,
Department of Health and Environmental Protection that they are
no longer issuing permits for on-site sewage systems on any property
within this subdivision that borders upon Lower Fire Lake. If
you intend to upgrade the system located on this property in your
further development of the property it will be necessary for you
to make application to the Municipality for a holdinq tank.
I also undemstand that there is currentlv a study being performed
by a consulting enqineering firm in Anchorage to determine the
feasibility of a central collection system to serve those properties
fronti~q on Lower Fire Lake. You may be able to obtain further
information on these studies from the Municipality of Anizhorage.
