Box 196650 Anchorage, Alaska 99519 Subject: Lot 7, Block 1 South Hills S/d Septic Tank Replacement Permit - Emergency Replacement Request Gentlemen: My firm was contacted in regards to a collapsed septic tank at the above referenced lot. The owner has indicated the tank is completely collapsed and sewage is spilling onto the ground surrounding the tank. There is an emanate health threat. A+ Home Services is currently pumping the tank on a daily basis to prevent back up of sewage. I am ~witing to request an immediate issuance ora septic tank replacement permit for this lot. I have attached n site plan showing where the proposed replacement tank will be located. Tank Replacement The lot is approximately 1.2 lacres in size. Lot 7 slopes to the west-northwest at approximately 1 to 3 percent in the area of the ,septic system. The slope is considerable steeper north of the system. The proposed installation will be located in the central portion of the lot. The existing tank is located approximately 95 feet from the well serving this lot and will be abandoned in place. The proposed septic tank will be installed over 100 feet from thc existing well serving this lot. The proposed location is greater than 25 feet from the water service lines. The other surrounding wells are located greater than 100 feet from the proposed instaLlation. The proposed installation will not affect the future development of the surrounding or existing lots.
