Ladies and Gentlemen:
I am writing to request a permit to upgrade the septic system for the above referenced property.
The proposed system will serve a 3 -bedroom single-family residence. The existing system did
not meet COSA operational requirements. A soils log, design calculations, a site plan, design
drawings and construction specifications are enclosed for your review.
Design Calcs: No groundwater observed to a depth of 5.5 feet below
ground surface (5/22/13)
Soil Rating. From Testhole 5/15/13
5.5 min/in = 3 gal per sq.ft/day

Required Area per Bedroom: 150/ 3 =50 sq.ft.
Total area required: 50 x 3 = 150 sgft
System: We are proposing a BioCycle AWWTS, and a mounded bed absorption field. The
absorption field will be 20 feet long and 15 feet wide and contain 0.5 feet (300 sq.ft. effective) of
sewer rock.
Soils: A test hole was excavated on May IP, 2013. Seethe attached soil logs. Groundwater
was monitored for through May 22 d, 2013. Ground water was observed at a depth of 5.5.
Surface Water: No surface waters were observed with 100 feet of the proposed septic system.
Topography: The lot slopes to west at approximately 10%.
Waivers: No waivers are required at this time.

Ladies and Gentlemen:
We are requesting a waiver of the required 3:1 side slopes on a mounded absorption field. At 3:1 side
slopes the toe of the slope would extend approximately 22 feet from the edge of the absorption field. A
2:1 slope would not require placing fill in the utility easement and bring the toe of the slope to
approximately 14.'
The proposed system is a mounded bed with advanced treatment and pressurized distribution.
Pressurized distribution should ensure that effluent is distributed over the entire bed surface, preventing
localized overloading. Treatment provided by the BioCycle AW WTS will maintain aerobic conditions in
the absorption field, which will also prevent localized overloading.
One and a half to 3.0 feet of MOA filter sand has be placed beneath the sewer rock. Effluent from the
drain field will pass through the filter sand vertically with little horizontal migration. This fine grained
soil will ensure that septic system effluent will be sufficiently polished as it migrates through the soil
column. The receiving soils are silty gravel to a depth of 10 feet. A percolation rate of 5.5 minutes per
inch was observed at a depth of 1 foot. With a percolation rate of 5.5 minutes per inch effluent ponding at
the receiving soil interface is not anticipated. Increasing the slide slopes of the mound should not have

SEPTIC SYSTEM REDESIGN
PROJECT NOTES:
1. All construction shall be in accordance with Chapter 15.65 of the
Anchorage Municipal Code, Wastewater Disposal Regulations, as
currently amended.
2. All fill slopes over system shall be 3:1 max. (3 horizontal to 1 vertical.)
3. Contractor shall excavate, remove, and reuse existing 1000 gal. septic
tank if possible. Contractor shall notify Engineer upon removal of septic
tank to allow inspection, prior to replacement.
4. Contractor shall expose existing sewer gravel and piping. Remove and
reuse existing PVC perf. pipe if possible. Contractor shall verify that
existing system has been installed at a level grade. If existing system
subgrade is not level, contractor shall regrade to provide a level subgrade.
5. Contractor shall leave existing septic gravel in place.
6. Maintain a minimum of 4 ft. of cover over PVC lines and septic tank.
7. Filter fabric shall be placed between septic rock over PVC perf. pipe and
backfill material, filter fabric shall be typar 3401 or approved equal.
8. Filter sand shall conform to Anchorage Municipal Code, Section
15.65.060, D.
9. All pipe to be 4" PVC per ASTM D3034 or approved equal.

ONSITE SEPTIC SYSTEM NARRATIVE
PROJECT: LOT 9, BLOCK 7, TRAILS END SUBDIVISION.
Owner: Mr. David Riley
The following narrative is submitted as part of the bed type septic system
upgrade for the onsite septic system for lot 9, block 7, Trails End Subdivision,
and is submitted in accordance with the Anchorage Municipal Code, Section
15.65, wastewater disposal regulations, Section 030.13.4.
The submitted design shown on the attached drawings and notes is a
modification to the 'existing unused septic system in place at this time. A brief
history of this site shows that the existing system was installed on 2/8/86 and
approved by Dept. of Health and Human Services on 2/26/86. Mr. Riley
contacted High Phukan Consulting Engineers to inspect the existing system on
Lot 9 prior to purchase. Our inspection showed that the system had inadequate
cover, and we recommended that additional cover be applied to bring the
system within municipal standards. After purchase, Mr. Riley contacted the
DHHS, and an inspector from that department visited the site, noting high
ground water in the system and indicated the system was condemned, as it did
not meet minimum separation distances above seasonally high ground water.
High Phukan Consulting Engineers was again contacted by Mr. Riley, to find a
solution to the problem at hand. Mr. Dee High of our firm visited the site, and
advised Mr. Riley to dig 2 water monitoring pits to allow the ground water
fluctuation to be measured. During the following weeks, it was noted that the
ground water was approximately 41 inches below the ground surface above the
existing system, and approx. 25 inches below the ground surface below the
lower end of the system.
The soil above the seasonally high ground water is an organic silt which is tight
and somewhat plastic. Because of the high level of the seasonally high ground
water, and the unsuitability of the existing soil above this ground water
elevation, we have chosen to utilize a 4 foot thick filter -sand bed system over
the existing unused septic system. During telephone conversations with Mr.
Dan Bowles of the DHHS, we were informed that this type of system design
would be acceptable for this specific site.
Please note on sheet 1 of 5 of the design drawings that the slope across the
extent of the site exceeds the maximum 10% allowed by municipal regulations.
'The degree to which this site exceeds the regulations is minor in that the slope
falls between 10% and 11 %.T -We ask that a waiver be granted to the
maximum slope regulations for this lot and septic design.
We have shown topographic contour lines on our desr,.l, which indicate the
relocation of soil taken by Mr. Riley to provide the necessary 3 feet of cover
over the existing system, prior to his being informed that the system had been
condemned.
Our design considers the existing contours, and we have shown the design site
grading in our drawings to modify the drainage of the site such that surface
water is directed away from the design system.
Item A: Wells
Lot 9 is ordered on 3 sides by developed lots. Due east is Doggie ave. which
borders the entire east side of lot 9. To the north is lot 10, to the south is lot
8, and to the west is lot 2, all of which have systems well away from subject
property. None of the lots surrounding lot 9 have wells within 200 feet of
either the existing or proposed systems.
The existing system does not affect any of the existing wells or placement of
new wells on adjacent properties.
Item B: Wastewater Svstems
All of the directly adjacent lots bordering lot 9 have existing septic systems in
place. The reconstruction of the septic system on lot 9 will have no impact on
those lots adjacent.
Item C: Reserved Space
There is space remaining on all adjacent lots for placement of new systems or
upgrade of existing systems. Lot 9 has adequate area in the southwest corner
for a replacement system as shown on our sheet 1 of 5 design drawing.
Item D: Drainage
Drainage for the area is generally to the West. Lots 8, 9 and 10 drain to the
west away from Doggie ave. The contour on lot 9 show a slight swale from
the northwest corner of the proposed 3 bedroom house across the proposed
system. Sheet 4 of 5 of our design drawings show regrading of the site to
prevent surface drainage from travelling across the proposed system.

A water sample revealed a nitrate concentration of 5.13 milligrams per liter (mg/L).
The Environmental Protection Agency (EPA) has established a maximum
contaminant level (MCL) of 10.0 mg/L for public drinking water systems. While
private wells are not subject to this regulation, EPA standards are based on existing
health information and can therefore be used to gauge the relative quality of water
from private wells. Please see the attached "Nitrate Fact Sheet" for important
information regarding nitrate.
This advisory must be attached to all copies of the subject Certificate of On-Site
Systems Approval.

Nitrate Fact Sheet
From Northern Testing Laboratories, Inc.
Nitrate is a negatively charged compound of nitrogen and oxygen, which is very soluble in water. Nitrate
is not readily filtered or otherwise removed in the soil and can pass rapidly into ground water wells.
SOURCE: Nitrate is a major component of fertilizer and wastewater. Often the nitrate is in the form of
ammonia or protein first, which through contact with oxygen and certain bacteria, converts to the
oxidized form known as nitrate. Sources of nitrate from wastewater include urea, ammonia cleaners,
food solids, and bacterial cells. It may also result from the breakdown of organic matter buried in the soil.
TOXICITY: Nitrate is generally not toxic to adults or children over the age of two or three years, but is
associated with a potentially fatal infant disease called methemoglobinemia. In the digestive system of
young children, nitrate converts to nitrite, which can pass through the intestinal wall into the blood
stream. There it combines with the hemoglobin and interferes with the ability of the blood to carry
oxygen. For this reason, methemoglobinemia is referred to as "blue baby" disease. The EPA limits the
concentration of nitrate in public drinking water supplies to 10 mg/L. The standard has been lowered
from a previous level of 45 mg/L set by the US Public Health Service and the World Health Organization.
TREATMENT: due to its solubility in water and negative ionic charge, filtration and other common home
water treatment systems such as softening or iron filtration does not readily remove nitrate. The best
method for limiting nitrate in well water is source control. This can include avoiding overdosing of
fertilizer near the well and maintaining good separation distances between septic tank leach fields and
the well. A special anion exchange filter that contains a media with a strong affinity for negatively charged
ions in water, or by a reverse osmosis treatment system or distillation can remove nitrate.
TESTING: Nitrate analysis is usually done by one of the several "wet chemical" methods using a
spectrophotometer to read the final color endpoint. Specific ion electrodes also can be used to detect
the activity of nitrate in water. This laboratory uses several different wet chemical methods approved
under the public water supply laboratory certification program. They also have test kits available, which
the laboratory uses to perform an inexpensive "screening test", and with which the homeowner can
monitor the change in nitrate levels from their well. They recommend comparing the test kit results
against a certified analysis from the lab occasionally to verify the accuracy of the kit. We recommend
using a specially prepared bottle that has been rinsed in hydrochloric acid for collecting samples.

MAINTENANCE AND REPAIR AGREEMENT
THIS MAINTENANCE AND REPAIR AGREEMENT,herein the"AGREEMENT" made and

Adam&Erika Fay ,herein the"OWNER,"and the Municipality of
Anchorage,herein the"MUNICIPALITY", in accordance with Anchorage Municipal Code
(AMC) 15.65.365. In consideration of the mutual covenants contained herein, the parties to this
Agreement agree as follows:
1. Advanced Wastewater Treatment Systems. The Municipality grants permission to the
Owner to utilize and operate an Advanced Wastewater Treatment System(AWWTS),

2. Maintenance, Repairs and Alterations.

manufacturer's representative. The AWWTS shall be maintained in a satisfactory
condition capable of performing as designed and producing treated septic effluent in
accordance with the equipment's approval for operation in the Municipality.
It shall be the responsibility of the Owner during the term of this Agreement to pay for all
repair(s), maintenance, adjustment(s),replacement costs, and inspection costs. This
includes an annual maintenance fee(typically$400 to$600).

or the manufacturer's representative will inspect and make any necessary maintenance,
repairs or permitted alterations to the system.

4. Nonwaiver. The failure of the Municipality at any time to enforce a provision of this
Agreement shall in no way constitute a waiver of the provisions,nor in any way affect
the validity of the Agreement or any part hereof, or the right of the Municipality
thereafter to enforce every provision hereof.
5. Amendment. This Agreement shall only he amended by authorized representatives of
the Owner and Municipality. Any attempt to amend this agreement by either an
unauthorized representative or unauthorized means shall be void.
6. Jurisdiction: Choice of Law. Any civil action arising from this Agreement shall be
brought in the Superior Court for the Third Judicial District of the State of Alaska at
Anchorage. The laws of the State of Alaska shall govern the rights and obligations of the
parties under this Agreement.
7. Severability. Any provisions of this Agreement decreed invalid by a court of competent
jurisdiction shall not invalidate the remaining provisions of the Agreement.
