1. The attached approved design.
2. All requirements specified in Anchorage Municipal Code Chapters 15.55 and 15.65 and the State of Alaska
Wastewater Disposal Regulations ( 18AAC72 ) and Drinking Water Regulations ( 18AAC80 ).
3. The engineer must notify DSD at least 2 hours prior to each Inspection. Provide notification by calling
(907) 343-7904 ( 24 hours ). ( Not required for a Water Supply Permit only ).
4. From October 15 to April 15, a subsurface soil absorption system under construction during freezing weather
must be either: A. Open and closed on the same day.
B. Covered, sealed, and heated to prevent freezing.
5. The following special provisions.

Septic Narrative
The subject lot of this narrative does not currently have a structure located on the lot. Once a Municipal
Land Use Permit is obtained, the owner, Mr. Joe Williams plans to place a 2-bedroom residence on the
Iot.
The lot currently has and existing well, and an existing septic located within it boundaries. The current
septic system is old, and is to be abandoned in place according to the current version of the Uniform
Plumbing Code. The well on the lot is not planned to be utilized, and the residence is to be connected to
the Municipal Water System as per MOA and AWWU specifications.
A new septic system is to be constructed and utilized according to current MOA regulations and
specifications. The system will utilize a deep trench design, with 4' of effective absorption depth, and a
length of 47' with 2" of extruded polystyrene insulation covering its entire length. A new 1000-gallon
septic tank (of MOA approved design) will be place outside of the 100' protective radius of the well on
the lot.
All separation distances will be maintained in the construction ofthis system as required by MOA
Regulations and specifications. Should soil conditions or any other factors arise that change the
conditions used for the basis of this septic design, the engineer and MOA DHHS, On-Site Services will
be notified before any modification are made that differ from the design set forth in the design packet
that is part of the MOA approved permit for a subsurface disposal field.
