Dear Onsite Services Engineer:.
A permit was issued for the construction of a well and septic system for Lot 16, Block 5,
Riverview Estates Subdivision in May of 1998. The permit subsequently expired and
the property changed hands. The current owner of the property intends to construct a
three-bedroom home on the lot. We are therefore requesting the permit be reissued for
the construction of a new well and septic system to serve the home. The attached Site
Plan and backup documentation identify the location and configuration of the new well
and septic system and the parameters used in the original design. Also identified on
the plans are the location of the well on the lot, test hole locations and the locations of
existing wells and septic systems in the area. Since the original permit was issued both
the lot to the south and to the northeast have been developed. The locations of the
wells and septic systems on these lots have been added to the drawings.
The test holes placed near the location of the proposed absorption trenches and
alternate site indicate slightly silty gravel with a percolation rate of less than 3 minutes
per inch. Groundwater was found during the placement of the test holes and was
monitored during the high water period. We are therefore proposing to place 2 - 5' wide
by 27' long by 2' effective depth absorption trenches on the property. The total depth of
the trench will be 4'. The distribution pipe will be placed at 2' below the ground surface
to provide sufficient separation from groundwater.
The ground surface on the lot slopes as shown on the attached Site Plan with moderate
slopes east to west. The new absorption trenches will be constructed In conformance
with Municipal requirements and parallel to ground contou ,rs, where possible. All
components of the new septic system will be constructed a m~n~mum of 100' from the
well on the lot and those In the area and 10' from the service line from the well to the
house.

CONSTRUCTION AND MATERIAL SPECIFICATIONS
SUBJECT: LOT 16, BLOCK 5, RIVERVlEW ESTATES SUBDIVISION
GENERAL:
The scope of this project includes the procurement and placement of a
new 1,250-gallon combination septic tank and lift station and pressure
distribution system. Work also includes the construction of 2 - 27' long
x 5' wide x 2' effective depth absorption trenches at the location shown
on the attached Site Plan. The new trench distribution piping shall be
constructed 2' below existing ground level for a total trench depth of 4'.
Direct bury insulation may be required to provide the minimum
coverage required over the new trench. In lieu of insulation 3' of cover
may be placed atop geotextile fabric over the new trenches.
Construction shall be in accordance with the approved site plan,
design drawings, Municipal Permit with any special provisions or
conditions, and all applicable State and Municipal Wastewater
Disposal Regulations.
The Contractor shall be responsible for obtaining all underground utility
locates and for the layout of the septic system and verification of the
location of all lot lines.
Unless specifically agreed otherwise, the contractor shall be
responsible for final grading areas subsequently depressed from soil
settling. Property owner shall be responsible for revegetation of
affected areas unless specifically agreed otherwise.
Contractors installing wastewater disposal systems must be certified
by the Municipal Department of Health and Human Services for
system installations, Owners installing their own systems must receive
prior approval from D.H.H.S. before beginning system installation.
SEPTIC TANK/LIFT STATION INSTALLATION
The 1,250-gallon septic tank/lift station must be constructed by
certified tank manufacturer. Construction shall include an 18" manhole
to provide access to the lift station.
2. The septic tank shall be sufficiently bedded to prevent settling or
shifting of the tank.
3. All standpipes on the septic tank shall extend a minimum of 12 inches
above final grade,
4. Tanks installed without 4' of cover shall have a minimum of 2" of direct
burial insulation.
A foundation cleanout shall be installed one to four feet from the
building foundation. Two cleanouts are required between the tank and
the drain field.

6. Final grading over the tank shall be such that a positive slope exists
away from the septic tank.
DRAIN FIELD CONSTRUCTION:
1. The drain field shall be constructed to the dimensions shown on the
design, The bottom of the trenches shall be within 2" of level.
2. Distribution piping must be placed level with perforations down atop a
level bed of drain field rock. Rock should then be placed over the pipe
to provide a minimum of 2" of cover.
3. A silt barrier or geotextile fabdc must be placed between the drain field
rock and the natural soil backfill.
4, Monitor tubes must be 4" in diameter and installed at the locations
shown on the design. The portion below ground must be perforated.
Contractor shall vedfy the septic tank and drain field are a minimum
100' away from any pdvate water wells in the area, 150' from a Class
"C" Well or 200' from any community well.
Direct bury insulation must be placed over the distribution system if
less than 3' of backfill depth is available. Finish grade over the trench
must be mounded to prevent settlement or depressions.
7. Grade area surrounding the absorption trenches to drain away.
A minimum 2' of accepting soil is required below the drain field rock for
a 5' wide trench. Contractor shall verify this condition prior to
placement of the rock. All pockets of unacceptable materials must be
removed and replaced.
MATERIAL SPECIFICATIONS:
The lift station must be constructed by a municipally approved septic
tank manufacturer. An Orenco 20 OSI 05 HHF-5 pump is
recommended.
2. The following pipe materials are approved for use in septic system
installations in the Municipality of Anchorage:
Cast Iron (perforated and solid), ASTM D3034 or P.V.C. (perforated
and solid). ASTM F810 or H.D.P.E. (perforated, but not solid) and
ASTM D2662 or A.B.S. (perforated and solid).
3. Insulation shall be at least 2" thick extruded direct burial polystyrene
(Dow Chemical Co. Styrofoam HI or equal).
4. Septic tank inlets and outlets shall be fitted with watertight couplings
(Caulder, Femco, or equal).

5. A permeable geotextile fabdc (Typar, Mirafi or equal) must be installed
between the final drain rock layer and the native soil layer.
6. All drain rock shall be .5" to 2.5" in diameter with less than 3% passing
the #200 sieve.
INSPECTIONS:
Municipal Ordinance requires a minimum of two inspections. These
inspections must be conducted under the supervision of a professional
engineer registered in the State of Alaska. The first inspection must be
conducted after the excavation of trenches, beds or pits and before the
installation of any gravel. A septic tank may be set in place, but may not
be bacldilled.
The second inspection must be conducted after the placement of the
geotextile fabdc, gravel, distribution piping, standpipes, cleanouts and
insulation. No backfill should be in place at the time of inspection.
Contractor shall provide a copy of all field survey layout and construction
notes for use in preparing the certified as-built of the completed system.

The subject permit, issued May 7, 1998 by this office for a single family well and/or on-
site wastewater system, has expired as of May 7, 1999.
A new permit must be obtained from this office for a well and/or on-site wastewater
system NOT installed by the expiration date.
If you have drilled the well, a well log must be sent to this office for documentation of the
installation and to close the permit.
If a licensed Professional Engineer has inspected the installation of the on-site wastewater
system, the original as-built inspection report must be sent to this o~ice for review,
approval and documentation. All inspection reports must be submitted within 30 days of
construction completion.
When applying for a new permit, the fees are: $320.00 for an on-site wastewater permit;
$120.00 for a well permit and $440.00 for a combined on-site wastewater and well permit.

Request you issue a permit to iustall a septic system to serve the proposed four
bedroom house on the referenced property.
Txvo test holes were excavated and a percolation test was perfo~ned. The approximate
locations of the test holes are located on the attached site plan.
At the time of excavation, groundwater was encountered in the test hole #1 at 4 fi. After
seven day groundwater monitoring, the monitoring tube was checked and gn'oundwater
was found a 3 ft. In test hole #2, at the time of excavation, the groundwater level was at
9 ft. After seven day groundwater monitoring, the water level was unchanged.
We do not anticipate any adverse effects on neighboring wells, septic systems or
drainage patterns by the installation of the proposed septic system.
There are no points of contamination within the proposed well radius which can be seen
on the attached site plan.

GENERAL:
The scope of this project includes the installation of a 1500 gallon
S.T.E.P.system, and a five foot wide drainfield to serve the proposed four
bedroom house located on the referenced property.
Construction shall be in accordance with the approved site plan and design
drawings, Municipal permit with any special provisions or conditions, and all
applicable State and Municipal Wastewater Disposal Regulations.
The contractor shall be responsible for obtaining any necessary underground
utility locates.
Unless specifically agreed otherwise, the property owner shall be responsible for
final grading areas subsequently depressed fi'om soil settling. On all leachfield
mound systems, the property owner shall be responsible for ensuring a
satisfactory vegetation growth over the mound area.
Contractors installing wastewater disposal systems must be certiffed by the
Municipal Health Department for system installations. Owners installing their
own systems must also receive prior approval from the Municipal Health
Department.
SEPTIC TANK INSTALLATION:
A septic tank is to be constructed by a certiffed septic tank manufacturer.
Construction shall include two 4" cleanouts for pmnping access.
The septic tank shall be sufficiently bedded to prevent settling or shifting of the
tank.
All standpipes on the septic tank shall extend a miuimum of 12 inches above
final grade.
4. Septic tanks installed with less than 4 ft. of cover shall be insulated.

A foundation cleanout shall be installed one to four feet fi'om the building foundation.
In the line between the tank and the leachfield there shall be two adjacent cleanouts
(unless an effluent pumping system exists within the septic tank). These cleanouts shall
be located on undisturbed soil not more than 10 ft. from the tank. The first cleanout, in
line, shall be to clean toward the leachfield. The second cleanout shall be to clean
towm'd the septic tank.
Final grading over the septic tank shall be such that a positive slope exists axvay from
the septic tank.
ABSORPTION TRENCH/DRAINFIELD INSTALLATION:
Excavate the proposed trench to the dimensions shown on the design. The bottom of
the excavation shall be within 2 inches of level. If the sidewalls of the excavation
become smeared, they must be raked or scratched (ruffed-up) before gravel (sewer rock)
placement.
Once the gravel is installed, the distribution pipe is to be installed level with the
perforations faced downward. Gravel is then to be placed over the distribution pipe to
provide a minimum of 6 inches of cover over the pipe.
A silt barrier must be installed between the final gravel layer and the native soil backfill.
Ensure the silt bmTier covers the entire gravel surface before placing backfill.
Monitor tubes shall be of four (4) inch diameter, installed approximately in the
locations shown on the design, and extend a minimum of 12 inches above final grade.
The portion of the monitoring tube extending through the gravel shall be perforated
fi'om the bottom of the trench to the invert of the distribution pipe. This is equivalent to
the effective depth of the gravel as noted on the design.
Backfill over the final gravel layer must not be less than twenty-four (24) inches.
Insulation must be installed when the backfill depth is less than thirty-six (36) inches.
The finish grade over the trench must be mounded to prevent the formation of a
depression after settling.
MINIMUM MATERIAL SPECIFICATIONS:
Any septic tank proposed for installation must be constructed by a Municipal approved
septic tank manufacturer.

Use of a type of pipe other than listed above must be approved by the inspecting
engineer.
Insulation shall be at least 2" thick extruded direct burial polystyrene (Dow Chemical
Company Styrofoam HI or equal).
Septic tank inlets and outlets shall be fitted with watertight couplings (Caulder, Fernco,
or equal).
A pe~rneable nontoxic silt barrier (Typar 3401, Mirafi 140N, or eqnal) must be installed
between the final leachfield gravel layer and the native soil backfill.
All leachfield gravel (sewer rock) shall be 0.5"-2.5" screened gravel with less than 3%
passing the #200 sieve.
When sand is being used as a filter material, its gradation specifications must conform
to CUlrent M.O.A. or D.E.C. requirements, which ever requirement applies.
INSPECTIONS:
Typically there will be a minimum of thi'ee (3) inspections required during the installation of
the wastewater disposal system. These inspections will occur as follows:
The first inspection must be conducted after the excavation of ditches, pits,
trenches, or beds and before the installation of any gravel. A septic tank may be
set in place, but may not be backfilled before this inspection.
The second inspection must be conducted after the placement of the silt barrier,
gravel, distribution lines, standpipes, cleanouts, and insulation, but before the
placement of any other backfill.
3. The final inspection is to occur upon final grading of the property.

Often there will be more than these 3 inspections required. Especially with the installation of
multiple trenches, sand filters, pressurized distribution systems, etc. Thus, the inspecting
engineer is to be contacted at least 24 hours prior to the start of construction. If necessary, a
pre-construction meeting will take place on-site. The inspecting engineer will not coordinate,
direct or control in any way the contractors activities.
The owner shall contract with the contractor to perform the work outlined in these
specifications and plans and in accordance with the attached M.O.A. pernfit. There will be no
contractual arrangement existing between the contractor and S & S Engineering. S & S
Engineering shall be the owner's representative and will inspect the work as stated above to
document the contractors activities. Final acceptance of the contractors work rests with the
owner and the M.O.A.
S & S Engineering shall have no liability to the owner or to others for acts or omissions of the
contractor or any other persons perforating work on this project or the failure of the contractor
to carry out the work in accordance with these construction documents. S & S Engineering's
inspecting engineer will not be responsible for the construction means, methods, techniques,
sequence, procedm'es or the safety precautions incident to this project.

The Municipality of Anchorage Development Se,vices Department (DSD) Issues Certificates of Health Authority
Approval (HAA) based only upon the representations given in paragraph 5 by an independent professional civil
engineer registered in the State of Alaska. Certificates of H,~alth Authority Approval are required for the transfer of
title (except between spouses) for properties served by a single family on-site wastewater disposal and;or water
supply system. DSD a!so issues HAAs upon request to homeowners. Certificates of Health Authority Approval are
valid for 90 days from the date of issue for properties seined by a private or Class C well and may be reissued with
new water sample results less than 30 days old. (Certificates may be reissued for a period of up to one year with
