Drain Field to Foundation Waiver
Drain Field Trench Separation Waiver
We are requesting a waiver for the separation distance from the absorption field to the house foundation
to 5.5'. The location of the field was tight due to an addition to the house and the location of large trees
the owner did not want to lose. The top of the drain field is located below the elevation of the
foundation, and the effluent and foundation will not affect the integrity of the other.
We are requesting a waiver for the separation distance between 2 tranches to 6.5'. The percolation rate
of the soil is less than 1 mpi and the effluent is unlikely to travel horizontally to a distance that the
trenches can effect each other.
Granting these waivers will not affect the health or safety of this lot, nor the future development.

Septic System Upgrade Permit Request
Ladies and Gentlemen:
I am writing to request a permit to install an upgrade septic system be issued for this property. The
upgrade to this system will be to accommodate the expansion of the house to four (4) bedrooms. The
existing drain field is operating adequately and will be expanded. The existing 1000g septic tank will be
abandoned per code and a new 1250gseptic tank will be installed. This lot and the surrounding lots are
served by private wells. There are no wells within 100' of this system.
1. Soils. One test hole was excavated in the area of this drain field by Kenneth Duffus in April of 1999,
and neither groundwater nor bedrock was encountered in the test hole. It is my opinion, based on the
results of the percolation tests and overall soils appearance; an application rate of 1.0 gallons/day/square
feet should be used for a conventional wastewater system with a 2' sand filter.
2. Soil Absorption System Design.
a. See Sheet 2 of the design package.
3. Surface Water: There is no surface water within 100 feet of the proposed drain field. The
proposed drain field upgrade will maintain at least 100 feet from all surface water and drainage ditches.
Eagle River Green Belt is located adjacent to this lot, but the river itself is located over 900' from this
system.
4. Topography: The existing topography slopes from the northeast to southwest at approximately
12% in the area of the proposed drain field. There are no steep slopes in the vicinity of the septic system

To whom it may concern:
The proposed 3 bedroom house will be served by a private well and septic system. Several test
holes have been excavated in the area of this property. See the attached soils logs
performed/prepared by Ken Duffus, P.E.. The septic system will be designed around the 30 foot
radius of test hole #99-27A. We are proposing that a 1000 gallon septic tank and dual five foot
wide drainfields be installed. Comments regarding the design are summarized as follows:
1. SOILS: See the attached logs which shows the soil classifications, groundwater monitoring,
and the percolation test results.
2. TRENCH DESIGN:
a. Percolation Rate: <1 minutes/inch
b. Allowable Application Rate: Proposing 1 0 gallons/day/ft2
c. Number of Bedrooms: 3
d. Design Flow: 450 gallons per day
e. Minimum Absorption Area: 450 ft2
f. Total Depth: 8.0 feet (max.)

short, there are no slope concerns. The trench is to be installed parallel to slope contours.

As certified by my seal affixed hereto and as of the validation date shown below, I verify that my investigation, based on procedures outlined
in the Certificate of On -Site Systems Approval Guidelines for this application, shows that the on-site water supply and/or wastewater
disposal system is (are) safe, functional and adequate for the number of bedrooms and type of structure indicated herein. I further verify that
based on the information obtained from the Municipality of Anchorage files and from my investigation and inspection, the on-site water supply
and/or wastewater disposal system is(are) in compliance with all applicable Municipal and State codes, ordinances, and regulations in effect at
the time of installation. - -
In conducting an adequacy test, 1 attempt to provide a thorough, conscientious engineering analysis of the system in accordance with.MoA COSA
guidelines and regulations. The reported results describe the performance of the system under the conditions encountered at the time of the test,
and separation distances measured to readily identifiable features. The operational life of all wells and septic systems depend on the local soil
condition, ground water levels that may fluctuate during the year, and the water usage of the family being served by the system. These conditions
are outside the control of the evaluator of this system. All systems eventually fail and satisfactory test results do not guarantee future
performance of the system, nor do they guarantee that there are no hidden defects or encroachments. Therefore we cannot provide any warranty
for future performance, nor can we estimate remaining life of the system. The content of this report is for the sole benefit of the owner listed
above.

Approval (HAA) based only upon the representations given in paragraph 4 by an independent professional civil
engineer registered in the State of Alaska. Certificates of Health Authority Approval are required for the transfer

water supply system. DSD also issues HAAs upon request to homeowners. Certificates of Health Authority
Approval are valid for 90 days from the date of issue for properties served by a private or Class C we!! and may
be reissued with new water samples. (Certificates may be reissued for a period of up to ane year with valid
water samples.) Certificates are valid for one year for properties served by Class A or B wells or a public water
system. The Municipality of Anchorage is not responsible for errors or omissions in the professional engineer's

Engineer's Comments:
In conducting this evaluation, AKWWC, lnc. attempted to provide a thorough,
conscientious engineering analysis of the system in accordance with ADEC and MOA
DSD Guidelines & Regulations. The reported results described the performance of the
system under the conditions encountered at the time of the test, and separation
distances measured to readily identifiable features. The operational life of all weds and
septic systems depend on the local soils condition, groundwater levels that may
fluctuate during the year, and the water usage of the family being served by the system.
These conditions are outside the control of the evaluator of the system. Satisfactory test
results do not guarantee future performance of the system, nor do they guarantee that
there are no hidden defects or encroachments. AKWWC, Inc. can therefore not provide
any warranty or future estimate of how long the system will continue to meet the
operational requirements of the ADEC or MOA DSD. The content of this report is for
the sole benefit of the owner listed above. Any reliance upon or use of this report by any
other person or party is not authorized, nor will it confer any legal right whatsoever.

ENCROACHMENT NON -OBJECTION AND AGREEMENT
THIS AGREEMENT, made and entered into this 2) day of march, 20 0 3 , by and
between MATANUSKA ELECTRIC ASSOCIATION, INC., an Alaskan non-profit member owned
electric cooperative, whose post office address is Box 2929, Palmer, Alaska, 99645, hereinafter
called "First Party", and Michael Quinn Construction, Inc. whose address is P.O. Box 772641,
Eagle River, AK 99577, hereinafter called "Second Party", WITNESSETH THAT:
A well is located within the 15' T + E easement Tying adjacent to and along Denaina Drive
In Lot 23, Block 1, Denaly Subdivision, according to Plat 2000-12, Anchorage Recording
District. T14N, R1W, S.M.
NOW, THEREFORE, for and in consideration of their mutual agreements and other good
and valuable consideration, the receipt of which is hereby acknowledged, First Party, for itself,
its successors and assigns, waives its objections to Second Party's encroachment on the said
easement for the purpose mentioned, subject to the following conditions:
1. Second Party will hold First Party harmless from liability for any injury or
damage to any person or property which may result from such encroachment.
2. This non -objection is granted with the full understanding and agreement by
all parties having an interest in the above property that this encroachment will in no way restrict
or limit the current or future ability of MEA to fully enjoy the benefits of the easement for any
and all utility purposes that it presently enjoys under the easement. This non -objection is
further conditioned by the stipulation that MEA will be held harmless from liability for any and all
damages to the encroachment that may result from the existing and future use of said
easement by MEA, its contractors, successors, agents, licensees, or assigns.
3. Any facilities or property of First Party which are damaged or destroyed as a
result of such encroachment will be repaired or replaced. The cost of repair or replacement will
be paid entirely by Second Party.
4. Second Party will comply with all applicable safety codes and regulations.
5. Any expenses which First Party may incur in the relocation or modification of
its facilities to accommodate the said encroachment will be paid entirely by Second Party.
By its execution of this Agreement, Second Party agrees, for itself, its
successors and assigns, that its encroachment upon the named easement as hereinabove
mentioned will be subject to the foregoing conditions.
IN WITNESS WHEREOF, the parties have caused this Agreement to be
executed by their duly authorized representatives and agents as of the day and year first above
mentioned.
