As part of permit OSP251214 a waiver request for well to trench separation of 70' was granted and added to waiver OSV201062. Due to side wall sloughing the trench was installed closer to the well than originally intended and is now 67.5' away from the well. Box 196650 Anchorage, AK 99519-6650 Subject: New Septic System Permit & Waiver request — T13NR3W SEC33 LOT 6 W3 SE4 M&B On June 12, 2025 the home -owner of the subject lot contacted us in order to replace the failed septic crib in preparations for a COSA. On June 19 a test hole was excavated on the subject lot in order to verify soils and percolation rates. The proposed system will serve the existing 2-bedroom residence. The existing septic system was documented and a COSA was approved in April 2003. This system was a crib with unverified length and width and a total depth 4.83'. The new testhole showed no groundwater up to a depth of 12' from grade. Record documents show the existing septic tank is a 1000-gallon plastic tank and is located 57' from the existing well. Due to the layout, the size, and the location of the well on this lot, we propose decommissioning the existing crib in place, replacing it with a trench 32' long 5' Wide with an effective depth of 2.5'. Due to the potential overlap of this system we require that any portion of the existing drainfield within 6' of the new trench to be excavated and replaced with type II material. In addition we are also requesting a well to field waiver of 70' and a field to property line waiver of 1' from the west lot line. Topography of the property is mostly flat with no apparent flow paths for ground water. The well was probed and confirmed to be cased more than 40' below grade. Previous water samples were taken on 9 / 16 / 2020 and show ND for Nitrates. The adjacent lots north and west are owned and used as commercial properties and are connected to AWWU services. The properties south are multi -residential with public services. The lot east of this property has a private well but the property on Wright is public sewer. There is no public information for the well or septic on the lots east or south of this property. Box 196650 Anchorage, AK 99519-6650 Subject: Undocumented Well & Septic — T13N R3W Sec 33, Lot 6, W3, SE4 Gentlemen: The well and septic system of the above subject property were undocumented. The 2 - bedroom house was built pre 1960 and the well and septic conformed to the applicable separation requirements of that year. On March 10, 2003 we excavated one testhole within 20' of the existing septic field for verification of ground water and bedrock or impermeable barrier. No bedrock or impermeable barrier was discovered and ground water was encountered at 9'. Attached is the soils log of the testhole. The septic tank is approximately 1,000 gallons and outfalls into a crib with an approximate depth of 4.83 feet. As shown on the attached as -built survey, the on-site well serving the lot is located within the house. The six-inch casing extends 12" above grade and 40'+ below grade. The well was probed to a total depth of 65' TOC. The crib standpipe is 3.5' from property line and we are therefore requesting a lot -line waiver.
