If this is a well permit and you have drilled the well, a well log must be sent to this office
for documentation of the installation and to close the permit.
If this is an on-site wastewater systm and a licensed Professional Engineer has inspected
the installation, the original as-built inspection report must be sent to this office for review,
approval and documentation. All inspection repons must be submitted within 30 days of
construction completion.
A new permit must be obtained from this office for a well and/or on-site wastewater
system NOT installed by the expiration date. However a new permit can be issued free of
charge for a second year ifthe application for the renewal is received on or before the
date of expiration ofthe original permit for which a fee was paid.
When applying for a new permit after the original permit has expired or for more than a
second year, the fees are: $320.00 for an on-site wastewater permit and $120.00 for a well
permit.

Absorption Trench Location
I have mbdified thc location of the absorption trench and reserve area for Lot 48 to be a minimum
of 70' from the east property line. This will allow more room for placement of wells and septic
systems on the adjacent subdivision if it is ever developed. A portion of the new location falls
outside the radius of the testhole. A new testhole will be dug during construction and a log
submitted with the as-built of the system.
We are hopeful the permit for Lot 48 can now be issued. Thank you.

Septic System Design
Impacts to Adjacent Properties
Dear Onsite Services Engineer:
The owner of Lot 48, Section 33, T12N, R3W intends to construct a five
bedroom home on the lot. We are therefore applying for a permit to
construct a well and septic system on the property. The attached site
plan and backup documentation identify the size and location of the new
well, septic tank and absorption trench to serve the home on the lot. We
have also attempted to identify the locations of wells and septic systems
on adjacent properties. No conflicts were found between the existing
systems and those to be constructed.
Testholes placed on the property revealed well graded gravels with
varying amounts of sand and minor amounts of silt. Soils percolate at a
rate approximating 3 minutes per inch. Groundwater was not encountered
during the excavation nor was any noted during the monitoring period. We
have therefore designed a 5' wide shallow absorption trench with 4' of
gravel beneath the distribution piping. The trench will be 63' long with a
total depth from original grade of 7.5'. The distribution piping will
entered mid trench.
The ground surface of the lot varies and the southern half has been
completed cleared and flattened. A small knoll exists at the location of
the proposed house and the ground surface slopes in all directions from
there. The slope at the location of the absorption trench is shallow and
less than 10%.

CONSTRUCTION AND MATERIAL SPECIFICATIONS
SUBJECT: LOT 48, SECTION 33, T12N, R3W
GENERAL:
1. The scope of this project includes the procurement and
placement of 1,500 gallon septic tank. It also includes the
construction of a new 63' long X 5' wide X 4' effective depth
absorption trench at the location shown on the site plan.
The distribution piping in the trench must be placed at 3.5'
below the original ground surface. Total depth of the trench
will be 7.5' from the original ground surface.
2. Construction shall be in accordance with the approved site
plan, design drawings, Municipal Permit with any special
provisions or conditions, and all applicable State and
Municipal Wastewater Disposal Regulations.
3. The Contractor shall be responsible for obtaining all
underground utility locates and for the layout of the septic
system and verification of the location of all lot lines.
4. Unless specifically agreed otherwise, the contractor shall
be responsible for final grading areas subsequently
depressed from soil settling. Property owner shall be
responsible for revegetation of affected areas unless
specifically agreed otherwise.
5. Contractors installing wastewater disposal systems must
be certified by the Municipal Department of Health and
Human Services for system installations. Owners installing
their own systems must receive prior approval from D.H.H.S.
before beginning system installation.

The Municipality of Anchorage Development Services Department (DSD) Issues Certificates of Health Authority
Approval (HAA) based only upon the representations given in paragraph 5 by an independent professional civil
engineer registered in the State of Alaska. Certificates of Health Authority Approval are required for the transfer of
title (except between spouses) for properties served by a single family on-site wastewater disposal and/or water
supply system. DSD also issues HAAs upon request to homeowners. Certificates of Health Authority Approval are
valid for 90 days from the date of issue for properties served by a private or Class C well and may be reissued with
new water sample results less than 30 days old. (Certificates may be reissued for a period of up to one year with
valid water samples.) Certificates are valid for one year for properties served by Class A or B wells or a public water
system. The Municipality of Anchorage is not responsible for errors or omissions in the professional engineer's
work.

Certificate of Health Authority Approval
Attached is a drawing showing the well recently placed on BLM Lot 48. The protective
radius of the well will impact the adjacent Lot 47G. The impact, however, will be
minimal once the protective radius for the well on Lot 47G is added. The drawing
shows two possible locations for the well on Lot 47G. These radii shadow much of the
protective radius from the well on BLM Lot 48.
An area approximating 2,400 square feet exclusive of the easement remains for septic
system development if the well serving Lot 47G is placed in the nodhwest corner of the
lot. The protective radius from the well on Lot 48 decreases the available area by only
144 square feet and the majodty of this area is within 10' of the lot line, which is
normally not usable for septic system development.
Please let me know if additional information is needed prior to issuance of the
Certificate of Health Authority Approval for BLM Lot 48.
