Dear Sirs:
Enclosed for your review is an updated As-Built Inspection Report for the septic system on Lot 12,
Block 2, Alpine Woods S/D, along with an application for a HAA certificate. The system bas installed in
1992, but the as-built inspection report was never finalized until now. The following paragraphs address the
points Susan Oswalt raised on her 11/30/92 pink sheet.
As should be evident from the design submittal, test holes//4 and #5 were shallow, hand-dug holes
that were dug solely for the purpose of confirming continuity of soils in the area where the drainfields were
constructed. While the exact elevations of the bottoms of these test holes are unknown, they xvere dug at a
location where the ground elevation was significantly higher than it was at test hole #I. Thus, the bottoms of
these 4'-5' deep postholes were more than 7 feet above the bottom oftest hole #1 that is shown on the
profile.
The final backfill work bas completed in accordance with the specifications.
As I recently discussed with Jim Cross, the original MOA electrical inspection approved all the house
electric without specifically addressing the lift station. Accordingly, the o~mer has arranged for the enclosed
inspect!on by a certified electrician attesting to the lift station compliance with electrical code.
During my recent adequacy test the soil absorption system the initial fluid depths were 7.5" and 5.5"
in the upper and lower drainfield monitor tubes, respectively. In the designed operation of the system the lift
station pumps effluent into the upper drainfield only, and the fluid depth in the upper drainfield gradually
rises to the point where effluent overflows into the lower drainfield. In this system, the upper drainfield only
begins to overflow to the lower drainfield when the fluid depth in the upper drainfield monitor tube is 15
inches. During the course of the test I added a total of 604 gallons of water into the system, which caused
the fluid level in the upper drainfield to stabilize at 15 inches and in the lower drainfield to rise to 8 3/8". At
this point fluid began to be visible in the inverts of the cleanouts at the ends of the lower drainfield,
indicating that the system was full. 166 minutes after the flow of water had stopped the fluid depths were'
12.5 inches in the upper drainfleld monitor tube and 8 1/8" in the lower drainfield monitor tube. These
changes in fluid levels correspond to a calculated absorption rate of 0.69 gpm which is in excess of 450
gallons per day. The majority of this absorption appears to be taking place in the upper drainfield.

Dear Sirs:
The purpose of this letter is to provide the required design narrative in support of our application for a
permit to construct water supply and wastewater disposal facilities on Lot 12, Block 2, Alpine Woods
S/D, located at 11811 Moose Road. Soils logs, perc test results, a site plan, design drawings and
specifications are enclosed for your review.
The proposed system will be constructed in the vicinity of test hole #1. As can be seen from the soil
log for test hole #1, the native material between 2' and 7' is a clean fine sand with a measured perc rate of
less than one minute per inch. Since this sand visually meets the M.O.A. spec for filter sand (sample
enclosed), the proposed design does not incorporate extra filter sand beyond what is needed for leveling.
Using the soil application rate of 1.0 gpd/sq, ft. specified in the wastewater ordinance for filter sand, this 3
bedroom residence requires a total absorption area of (3 x 150)/!.0 = 450 square feet. The proposed
design of two parallel 5' wide by 45' long soil absorption drainfields connected in series has a total
absorption area of 450 square feet. Shallow test holes #'s 4, 5, and 6 were dug to confirm continuity of
the receiving stratum in the proposed construction area. The measured perc rate in these confirming test
holes was between 1 and 5 minutes per inch. A lift station will be necessary to distribute septic tank
effluent to the drainfields.
Most of the lot (with the exception of the area proposed for the original and replacement wastewater
disposal sYstems) is poorly drained and boggy. A stream along the east boundary of the lot further limits
the useable area. The topography in the proposed construction area varies from 25% to 35%, with an
average of 27%. In achieving the required soil cover over the drainfields, the fill slope on the downhill
side will extend out to cover a portion of the boggy area at the toe of the slope. The lowest drainfield will
be constructed on the uphill side of test hole #1, and the maximum cut depth on the uphill side of each
drainfield will be 3', thereby ensuring that the required 4' vertical separation from groundwater is
achieved. Level bottoms of the drainfields will be achieved by placement of additional filter sand as
needed, prior to placement of the sewer gravel. Despite the fact that the slope in the construction area
slightly exceeds the normal maximum of 25%, the proposed drainfields should be able to function without
concern as to effluent daylighting out of the sides.
The proposed project will have no impact on present or future water supply and wastewater disposal
systems serving adjacent properties, nor will it have any impact on reserved space/surface and subsurface,
or on drainage.

Specifications
1.0 General:
1.1 The scope of the project consists of installation of a 1250 gallon STEP tank followed by two 5'
wide by 45' long soil absorption drainfields connected in series. The contractor is advised that boggy
conditions prevail over much of the lot, and that the slopes in the proposed drainfield area range from 25%
to 35%.
1.2 Construction shall be as depicted on the approved site plan and design drawings. Minor
deviations from these drawings may be allowed or required by the engineer conducting the inspections.
All construction procedures and material specifications shall conform with Municipal and State
requirements. All separation distances shall be in conformance with Municipal requirements, unless
specifically waived.
1.3 The contractor shall be responsible to obtain any necessary utility locates, and to work around any
buried utilities.
1.4 The contractor shall provide adequate cover material and rough grading over all 'system
components to ensure that proper drainage is achieved after settlement and that there are no residual
depressions. Insofar as possible the contractor shall minimize damage to trees.
1.5 Unless specifically agreed otherwise, the homeowner shall be responsible for finish grading after
the soil is compacted, as well as placement of topsoil and reseeding all areas disturbed by the construction.
2.0 Septic Tank:
2.1 The septic tank shall be a Municipally approved 1250 gallon STEP tank with two compartments,
and shall be set level on undisturbed soil. Each compartment shall be equipped with a watertight manhole
cover and a 4" cleanout. The tank shall be buried a minimum of 4 feet below finish grade and shall be
insulated with 2 inches of approved burial type, rigid insulation.
2.2 All pipe connections to the tank shall be equipped with waterproof mechanical couplings. Since
the tank will be buried in an area subject to groundwater, particular cm-e shall be taken to ensure that all
pipe joints are watertight, and that all pipe is properly bedded to avoid displacement. The waste line from
the residence to the septic tank shall have a minimum slope of 1/4'" per foot. A cleanout shall be installed
within 5 feet of the building foundation.
3.0 Lift station:
3.1 The lift station shall be installed in the second compartment of the septic tank. The lift station shall
be an "Orenco" package system as supplied by Anchorage Tank.
3.2 The contractor shall be responsible to obtain the necessary Municipal electric permit and
inspection, and to provide a copy of the electrical inspector's report to the engineer overseeing the
construction of the wastewater disposal system.

3.3 Thc on, off, and alarm floats shall be set at the elevations specified by thc lift station supplier, and
their proper operations shall be verified by the contractor.
4.0 Soil absorption system:
4.1 The soil absorption dralnfields shall be constructed by carefully stripping off the overlying peat
and sandy loam strata to expose the top of a sand stratum at a depth of approximately 2 feet below ground
surface.
4.2 The longitudinal axis of each drainfield shall follow the contour of the top of the sand stratum.
Because of the irregular topography, the drainfields will be curved instead of straight. Level bottoms of
the drainfields shall be created by excavating no deeper than 3' on the uphill sides. Particular care will be
necessary to avoid over-excavation of the sand. Additional sand as needed for leveling the drainfield
bottoms shah be coarse pit mn sand complying with Municipal specifications for f'flter sand.
4.3 A total of 1.0 feet of approved sewer gravel shall be placed in the bottom of the excavation with
the perforated distribution pipes laid level such that the pipe inverts are no less than 6 inches above the
bottom of the sewer gravel. Sewer gravel shall be 0.5" - 2.5" screened gravel, with less than 3% passing
the #200 sieve.
4.4 The effluent from the STEP tank shall be transported by 1.25" diameter schedule 40 PVC for
pressure distribution into the upper drainfield only. The distribution lateral shall be 1.25" diameter
schedule 40 PVC with 3/16" holes drilled in the top on 2' centers. The end of the distribution lateral shall
be capped, and all PVCjoints in the pressure distribution system shall be glu.,e~l,. A separate 4" dia
perforated pipe shall be bedded level in the upper trench with the invert also 6 above the bottom of the
gravel, This pipe shall serve to collect excess effluent in the upper trench and shah be connected to a
similar 4" diameter non-pressurized distribution pipe in the lower drainfield by means of a non-perforated
4" pipe incorporating a 1" high "bump" as it exits the upper drainfield.
4.5 Monitor tubes and cleanout pipes shall be of 4" diameter and installed in the locations shown on
the design drawings. The portion of the monitor tube extending through the sewer gravel shall be
perforated.
4.6 Approved filter fabric shall be placed over the entire top surface of the sewer gravel. A minimum
of 2 feet of soil cover is to be placed over the filter fabric. If the soil cover thickness is less than 3 feet,
two inches of rigid, burial type insulation is to be placed over the entire top surface of the gravel, in
addition to the filter fabric.
4.7 The top surface of the cover material shall be raised a minimum of 6 inches higher than the
surrounding terrain to allow for subsequent settlement, and shall be graded to smooth contours. Fill
slopes shall be no steeper than 3:1.
4.8 All areas disturbed by the construction including side slopes shall be covered with 4 inches of
topsoil and seeded with an approved grass seed to promote rapid revegetation. This work shall be the
responsibility of the owner.
5.0 Inspections:
5.1 A minimum of 5 engineering inspections will be required during the course of the project: (1)
initial stakeout with the contractor to establish the location of the system and to discuss 'the plans,
specifications and construction procedures, (2) after the native material has been excavated to expose the
infiltrative surface to ensure that it is level and at the right elevation, and conforms with the soil test
information, (3) after the sand leveling material has been placed, (4) after the sewer gravel is in place and
the distribution pipes have been laid and connected up to the septic tank, but prior to placement of
insulation or filter fabric, and (5) after final backfill and grading is complete. The septic tank requires one
· inspection after it is set level and the piping connected, but prior to backfill. This inspection may be
incorporated with any of the above inspections.
5.2 The installer shall coordinate the timing of the inspections with the engineer sufficiently far in
advance to ensure the availability of the engineer.

A permit issued, by this Department for an individual well
and/or on-site sewer system has expired as of December 31,
.1888.
Permits are issued on a calendar year basis by authority of
Municipal Ordinance. A new permit must be obtained from
this Department for any well and/or on-site sewer system
not installed by the expiration date.
If you have drilled the well, a well log needs to be sent to
this Department for documentation of the installation and to
close the permit.
If a private engineer inspected the installation of the
on-site sewer system, the orginal as-built inspection report
(three-part form) must be sent tO this office for review and
approval, and for documentation.
When applying for a new permit, the fees are: $90.00 for
an on-site sewer permit; $50.00 for a well permit; $140.00
for a combined sewer and well permit.
