Your request for a waiver of the required 50 feet horizontal separation from the absorption field
to the excessive slope has been approved. The approved separation distance is 12.0 feet. See
engineer's waiver request letter and profile drawing for justifications. Although profile drawing
shows the effluent daylighting, the 25% hvp othetical line is overly conservative for the fast

perking soils near the field.
This waiver approval applies to the existing absorption field only. Any future upgrade to the on-
site wastewater disposal system will require all separation distances be met or another approval
from this department.

Absorption Trench to Slope Waiver
Dear Onsite Services Engineer:
The area to the east of the new absorption trench on the subject lot was excavated to
provide a parking area for the new shop/apartment building. This resulted in a cut
slope greater than 25%. It is located 12' from the edge of the new trench sidewall. The
slope drops 11.8' over a distance of 13' from this location. At the toe of the slope the
parking area is horizontal for a distance of 16' to the edge of the building. The attached
cross sectional drawing shows the 25% imaginary slope when extended from the
sidewall of the trench from 2.5' above the distribution flowline will daylight 19' from the
sidewall. It then falls below ground 22' from this point and does not daylight again.
Soils in the area are very good for the treatment and absorption of septic effluent. The
majority of absorption will be down and migration in any direction other than down will
be minimal. The face of the slope will be revegetated and maintained to insure no
erosion can occur. We therefore request a waiver be issued allowing the new
absorption trench to remain in its existing location at 12' from a slope greater than 25%.

Permit number is incorrect on inspection report (OSP141179 vs OSP141479). Please address.
Provide separation distances from lift station on inspection report.
For the field to slope >25% waiver, the 25% hypothetical line is started from a point 2.5 feet above the invert of the
distribution piping of the septic field (narrative and profile use 2 feet). Please address.
4. The slope referenced in the waiver is almost a 1 -to -1 slope. Address slope stabilization.
5. Provide statement from home owner that day -lighting of effluent has not occurred.

Septic System Design and Permit Application
Impacts to Adjacent Properties
Dear Onsite Services Engineer:
The owner of BLM Lot 113 intends to construct a new shop with a mother in law
apartment on the second floor. The apartment will have only one bedroom, but the
system is sized for three. The facility will be served by a separate septic system. We
are therefore requesting a permit be issued for the construction of a new septic system
to accommodate the shop. A well is currently in place on the lot and will be used to
serve the shop. The attached Site Plan and backup documentation identify the location
and configuration of the existing well and septic system along with the proposed system
and alternate site. Also identified on the plans are the existing wells in the area and
septic systems on surrounding lots. No conflicts exist between the proposed septic
system and well or septic systems and wells on adjacent lots. Drainage arrows are
shown on the site plan showing the grade and direction of flow on the lot. The drainage
pattern will not be changed as a result of this project.
The test hole placed on the lot in the area of the absorption trench and alternate site
indicate poorly graded gravel with some sand and silt (GP/GM). The percolation rate of
the soil was determined to be between 3 minutes per inch. No groundwater was found
during the placement of the test hole and none was noted during the monitoring period.
An application rate of 1.2 gallons per day per square foot was used in the design of the
absorption system. We are proposing to place a new 1,250 -gallon STEP tank and a
38' long by 5' wide by 4' effective depth absorption trench to treat and dispose of the
effluent generated in the shop. The elevation of the distribution line in the absorption
trench will be 2' below the surface and the total depth will be 6'. A minimum of 3' of fill
or 2" of insulation and 2' of fill will be placed over the absorption trench and 4' over the
septic tank.

than the shop and slopes to the east at an 8% grade with some cross sloping to the
southwest. The new trench will be constructed parallel with the contours of the slope as
much as possible in conformance with Municipal requirements. The new septic system
will be constructed a minimum of 100' from all wells and any surface water and 10' the
proposed and existing water services in the area.
If the system is constructed in accordance with our design the following statements
apply:
1. The system, if constructed as designed, will have no adverse impact on the wells
in the area or those to be constructed in the future.
2. The system, if constructed as designed, will have no adverse impact on existing
septic systems in the area or those to be constructed in the future.
3. The system, if constructed as designed, will have no adverse impact on reserve
space, either surface or subsurface, on any lots located in the area.
4. The system, if constructed as designed, will have no adverse impact on drainage
patterns in the area. The current drainage pattern will be maintained.

Septic System Design and Permit Application
Impacts to Adjacent Properties
Dear Onsite Services Engineer:
The owner of BLM Lot 113 intends to construct a new four-bedroom home on the lot.
We are therefore requesting a permit be issued for the construction of a new septic
system to accommodate the home. A well was placed on the lot at some point in the
past and will be used to serve the new home. The attached Site Plan and backup
documentation identify the location and configuration of the existing well and the
proposed septic system along with the alternate site. Also identified on the plans are
the existing well on the lot and in the area and septic systems on surrounding lots. No
conflicts exist between the proposed septic system and well or septic systems and wells
on adjacent lots. Drainage arrows are shown on the site plan showing the grade and
direction of flow on the lot. The drainage pattern will not be changed as a result of this
project.
The test holes placed on the lot in the area of the absorption beds and alternate site
indicate silty gravel with some sand (GM). The percolation rate of the soil was
determined to be between 31 and 60 minutes per inch. Groundwater was found during
the placement of the test holes at 16' below the surface and stabilized during the
monitoring period at 12' below the surface in Test Hole No. 2 and 8.5' in Test Hole No.
1. An application rate of .3 gallons per day per square foot was used in the design of
the absorption system. The absorption fields do not fall entirely within the 30' radius of
Test Hole No. 2. We are requesting that additional test holes and percolation tests be
completed during construction to verify the quality of the soils at the location of the
absorption beds. The test hole requirement may be a contingency to issuing the
construction permit.
We are proposing to place a new 1,250 -gallon septic tank and 2-67' long by 15' wide by
.5' effective depth absorption beds to treat and dispose of the effluent generated in the
house. The elevation of the distribution line in the absorption beds will be 3.5' below

the surface and the total depth will be 4'. A flow splitter valve will be placed to insure
even flow to each bed. A minimum of 3' of fill or 2" of insulation and 2' of fill will be
placed over the absorption trench and 4' over the septic tank.
The ground surface on the lot in the area of the septic system slopes to the southeast
at a shallow grade with some cross sloping to the southwest. The new beds will be
constructed parallel with the contours of the slope as much as possible in conformance
with Municipal requirements. The new septic system will be constructed a minimum of
100' from all wells and any surface water and 10' the proposed and existing water
services in the area.
If the system is constructed in accordance with our design the following statements
apply:
1. The system, if constructed as designed, will have no adverse impact on the wells
in the area or those to be constructed in the future.
2. The system, if constructed as designed, will have no adverse impact on existing
septic systems in the area or those to be constructed in the future.
3. The system, if constructed as designed, will have no adverse impact on reserve
space, either surface or subsurface, on any lots located in the area.
4. The system, if constructed as designed, will have no adverse impact on drainage
patterns in the area. The current drainage pattern will be maintained.

disposal system, a new permit must be obtained from this office. A new permit for a second year may
be issued for a fee of $100.00 if the renewal application is received on or before the expiration date of
the original permit.
When applying for a new permit, the fees are: $400.00 for a wastewater permit and $150.00 for a well
permit.

engineer registered in the State of Alaska. Certificates of On -Site Systems Approval are required for the transfer of
title (except between spouses) for properties served by a single-family on-site wastewater disposal and/or water
supply system. DSD also issues COSAs upon request to homeowners. Certificates of On -Site Systems Approval
are valid for 90 days from the date of issue for properties served by a private or Class C well and may be reissued
with new water sample results. (Certificates may be reissued for a period of up to one year with valid water
samples.) Certificates are valid for one year for properties served by Class A or B wells or a public water system,

Water Well Advisory
Certificate of On -Site Systems Approval (COSA) # 101158
During a recent COSA on-site inspection and test of the potable water
supply well on Block , Lot 113 of T15N R2W Section 25 subdivision,
the well's productivity was determined to be 0.7 gallons per minute. The
minimum well productivity required by this Department (AMC 15.55) for a
4 -bedroom residence is 0.4 gallons per minute. Although the subject well
currently exceeds this minimum requirement, all parties concerned are
advised that the production capacity of the well may fluctuate. Restriction
of non-critical water uses such as washing cars and watering lawns and
gardens may be required.
This advisory must be attached to all copies of the subject Certificate of On -
Site Systems Approval.
