Septic system design
Dear On -Site Services Engineer:
The owner of the above lot has a septic tank that has reached its end of useful life, so we are
submitting this permit application for its replacement. The attached site plan identifies the location
of the home as well as the wells and septic location. No conflicts exist between this proposed
system and any other wells or septic system, whether on this lot or adjacent lots. The home has 3
bedrooms, and we are replacing the existing 1250 -gallon septic tank with a 1500 -gallon Step tank.
The new septic tank will be a minimum of 100' from all wells and surface water, and more than
5' away from the absorption field. Please refer to the attached plan for the septic design. If this
design is followed, there will be no adverse impacts to adjacent properties.

The subject permit #SW980425, issued by this office for a single family well and/or on-site
wastewater system, is due to expire as of 10/30/99.
If this is a well permit and you have drilled the well, a well log must be sent to this office
for documentation of the installation and to close the permit.
Ifthis is an on-site wastewater systm and a licensed Professional Engineer has inspected
the installation, the original as-built inspection report must be sent to this office for review,
approval and documentation. All inspection reports must be submitted within 30 days of
construction completion.
A neTM permit must be obtained from this office for a well and/or on-site wastewater
system NOT installed by the expiration date. However a new permit can be issued free of
charge for a second year if the application for the renewal is received on or before the
date of expiration of the original permit for which a fee was paid.
When applying for a new permit atter the original permit has expired or for more than a
second year, the fees are: $320.00 for an on-site wastewater permit and $120.00 for a well
permit.

Gentlemen:
On October 30, 1998, you issued a permit for a new well and septic. Subsequently
the owner has decide to move the house site to the east side of the lot--which
necessitates the movement of the well and requires a lift station to be put in to make
the septic system work. All other items on the previous request will remain the
same. Please find the attached plan-design modifications for your approval.

Subject: New sewer/well permit - BLM Lot 37, Section 7, T15N, R1W, S.M.
Gentlemen:
On September 30, 1998, we excavated two testholes for the subject property. The
results of these tests and water monitoring are attached.
We propose to install a 5' wide shallow trench. The depth to water is 7.5' therefore
we have designed the system in the upper top three feet of the gravel/sand layer.
Additional fill will be placed over the system to provide a minimum of 3' of cover
when complete. This lot slopes from southeast to northwest. The majority of the lot
has a 2-5% grade with a steep slope in the southeast corner that has grades in excess
of 25%. The proposed layout maintains slopes away from the proposed house and
any surrounding wells. Although the distance between the tank and the field is long
there is sufficient grade to maintain a gravity system.
We propose to drill the well north of the house and septic area. There are no public
or private wells within 100' of our proposed system location except as shown. There
is neither surface water within 100' nor any known curtain drains within 50'. We do
not expect that there will be any adverse effect on adjacent lots by the development
of this system.
