Their septic system was designed and built in 1995. At the time, the home was operating as a duplex, and therefore under the jurisdiction of the DEC. Unfortunately, it seems that due to some miscommunication, as-builts were not completed at the time- there is no official record that the system was built as designed and approved to operate. The water test and well & septic adequacy tests all indicate that the system is working and adequate for the residence. We are presenting the information that we have found in our investigations as well as our field measurements to show that the system was built adequately. According to the approved permit, the trench was designed to be 14' total depth, with 10' effective depth of gravel below the pipe. It was designed to be 32' long. Field measurements in the monitoring tube confirm a depth of 14.6'. While we obviously cannot confirm the exact depth of the gravel and the pipe, the confirmation of the measurement gives confidence that the cross-section of the trench is correct. As stated, the permit was approved for construction. While we do not have the record of the test hole and perk test, we can assume a minimum perk result from this design. The trench as designed provided 640SF of effective absorption area, and the home is a 4-bedroom home. The soils rating must have been at least 160 SF/bdrm in order to get approval. The original test holes must have also been deep enough to show the required 4' separation from groundwater and 6' from bedrock, as the design mentions this requirement. Additionally, a letter from the engineer (1-I. Wilson, stamped and dated Sept 24, 1995) mentions that a monitor tube was checked and installation was observed, confirming `water table and impermeable strata separation'. This is supported by soils data from adjacent Lot 4, showing no bedrock or groundwater to 16' in two test holes and perk rates under 5min/in; test holes from Block C Lot17 across the road, showing similar perk rates and no bedrock or water down to 18'; and records from Lot 31 behind, which show approved seepage pits at 12' deep. The design specifies a 1500-gallon septic tank. This is adequate for the size of the system, and matches the measurements taken of the vents. Of course, without digging it up we cannot verify the material or where the tank came from, but there isn't reason to suspect in this case that it was not installed as specified. This same letter also mentions that the trench was installed as 46' long 'rather than the required 32 feet'. This is confirmed by field measurements, and indicates that the final installation was actually conservative. Since the house was a duplex, according to regulations at the time the well was classified as a Class C well. That is no longer a requirement, and the well can be considered a simple private well according to current regulations. There is no well drilling log, but there is a pump test report dated 9/18/95 which gives some pertinent information- depth, casing depth and size, static water level, and well yield. All of the result of the investigation taken with the measurements in the field indicate that the system was built as designed and is functioning properly. Attached are stamped drawings showing the verified design. We are confident that this is sufficient for the records and for a COSA to be issued for the home. It appears, from the as-builted locations of the cleanouts, that a portion of the trench falls closer than 10' to the front lot line, probably around 8'. This will not affect the neighboring lots, since it is in the front next to the ROW, and there is sufficient distance from the actual road surface. This trench will not cause any problems to the ROW. Therefore, we would like to request that a waiver be issued for the distance from the trench to the front lot line. This was requested because the duplex is now in the process of being sold. In researching your files I found a 4/15/95 engineer's report requesting construction approval and a 5/9/95 letter from Michael Lu granting the construction approval. I also found a 9/24/95 letter from Mr. Windeler's engineer, Henry Wilson, providing a post construction response to conditions and questions raised in the 5/9/95 letter. I did not find any formal as-built drawings of the septic system or operational certificates issued by D.E.C. Further inquiry led to the discovery of a draft DEC response dated 10/12/95 that was prepared for Mr. Lu's signature, but apparently was then overlooked with the result that it never got sent out to Mr. Wilson. Thus, Mr. Windeler was never made aware that your department required further information before issuing formal operational approval. This submittal provides the desired additional information. I understand that your office will not require payment of the $230 additional waiver fee referenced in the Mr. Lu's draft letter. There appears to have been some confusion regarding the proper legal description of the property. Mr. Wilson's submittals refer to the property as being Lot 2, Block B, Sunset Hills S/D, whereas the actual legal description should be Lot 3, Block B, Sunset Hills S/D. Because all the adjoining lots are of the same size and shape with no clearly distinguishing features, and because the subdivision has been truncated by the construction of the Rabbit Creek interchange on the Seward Highway, it is easy to mistake one lot for the next. I have prepared the enclosed drawing showing the as-built configuration of the water supply and wastewater disposal system components. • On June 5, 1998 I re-inspected both systems. Mr. Wilson provided complete documentation of the construction of the water supply well and its yield in his 9/24/95 submittal. I subsequently inspected the components of the water distribution system visible inside the duplex. The water supply line from the well is 1" diameter copper, and the interior plumbing is '/." and '/:' copper. The plumbing appears to be in good condition and there is no indication it has been modified in recent years. The water storage tank is 20" diameter x 36" high, bladder type pressure tank with a calculated total volume of 50 gallons. The water heater is a gas fired Rheem unit with an external diameter of 18" and a height of 45". The lab test results for water samples that I collected on 5/26/98 were satisfactory, showing 0 coliform or other bacteria per 100 ml., and 0.261 mg/I of nitrate-N. The as-built locations of the septic system components with respect to the water supply well and the building corners are shown on the accompanying drawing. This documents the fact that the as-built configuration conforms to the approved 100' horizontal separation distances of the proposed design. Mr. Wilson's signed and sealed letter dated 9/24/95 provides assurance that the system as installed maintains the required 4-foot vertical separation from groundwater and 6 feet from impermeable material. I trust that this submittal adequately addresses DEC's remaining concerns regarding these systems. If so, please issue the applicable Operation Certificates. A copy of the test data is attached for your review and files. The test showed a yield of 9.8 gpm with very little drawdown and an immediate recovery to-the initial static level. I interpret this to mean that the 9.8 gpm is limited by the capacity of the pump rather than the well. Wastewater Disposal System I. The writer monitored the installation of the wastewater disposal system. The system was installed in compliance with the approved design, except that the trench length installed was 46 feet rather than the required 32.feet. 2. Based on the nearby soil monitor tube installed in the test hole and conditions observed during the installation, soils, water table and impermeable strata separation were verified. 3. Effective depth of the trench was measured from the bottom of the distribution lateral. Please advise if any other information is needed.
