1. A test hole is required prior to construction of the absorption field, in the location shown on the site plan, to
confirm minimum separation to impermeable soil and seasonal high groundwater, as well as percolation
rate. Construction may proceed at your own risk prior to 7-day groundwater monitoring. If results require
a design change, construction shall stop pending approval of a change order. Please submit results with
the inspection report (or change order, if required).
2. Confirm bedrock depth at each corner of bed to verify vertical separation.

RE: Proposed Septic System for Midway Industrial Park Lot 8
Dear Reviewer,
The above referenced property is currently being developed. The owner would like a permit to construct a
well and septic system for a 5 bedroom home. The lot is large and there are no wells or septics near the
proposed onsite systems.
Our review of available documentation and field investigation show that this project will not adversely
impact any nearby Wells, Wastewater disposal systems, replacement disposal sites, or drainage flowing
onto and off of the subject property.
As shown on the plan, the tank and field will be greater than 10’ from the house foundation.
The tank will be of MOA approved construction.
The tank and field shall be covered with a minimum of 2” moa approved insulation and 3’ of cover or a
minimum of 4’ of cover without insulation.
The design is based on the testhole logged by us in 2025. We found the soils to be free draining and
bedrock was encountered at a depth of 6 feet. A cross section is provided to illustrate the leachfield
details of construction.
Upon completion of the installation, a record drawing will be submitted showing the location of the new
tank, leach field, well, and other applicable features.
The homeowner has started construction of the septic system. The tank was set and the leachfield was
partially constructed. We were able to visit the site mid-construction and the homeowner agreed to stop
work until a permit was issued. We verified soil conditions for the bed were consistent with those
expected. The tank is bedded with D1. At this time sewer rock for the bed has been placed, but it has not
been backfilled. The sewer rock is a clean crushed rock product which appears to meet MOA spec and all
workmanship appears to be consistent with MOA requirements and industry standards.

Septic System Owner -installer Agreement
The On -site Water and Wastewater Section (On -site) may issue an approval for a homeowner
to perform work on an on -site wastewater disposal system to serve that individual's owner -
occupied, single-family or duplex home if the homeowner meets and agrees to the following
requirements:
1. The property owner and excavation equipment operator may perform work on no more
than one owner -installation project in a 12-month period.

Ref: Proposed Well and Septic System for Midway Industrial Park S/D; Lot 8
To whom it may concern:
The proposed 3 bedroom house is to be served by a private well and septic system. Two test
holes were excavated on the property. The primary drainfield and alternate site have been
designed around the 30 foot radius of the test hole #1 (see site plan). We are proposing that a
1000 gallon septic tank and a 5 -wide type drainfield be installed for this property. The proposed
well is to be drilled as shown on the attached site plan. Comments regarding the design are
summarized as follows:
1. SOILS: See the attached logs which shows the soil classifications, groundwater monitoring,
and the percolation test results. The percolation rate for TH#1 was 1.1 minutes/inch which would
require the use of a 1.2 gpd/sq.ft application rate.
2. DRAINFIELD DESIGN: See attached design drawing for drainfield specifications.
3. SURFACE WATERS: There are no surface water concerns.
4. TOPOGRAPHY AND SLOPE WAIVER: Based upon the topography information provided
by S4 Group, the topography in the area of the proposed drainfield averages 40% percent
running approximately Southeast to Northwest. We are proposing to use the Approved
Revisions to the wastewater code for this installation. We have attached a copy of the Steep
slope disposal fields section for you review (pages 29 & 30 of 62). Based upon this code, Deep
Trench and Wide Trench disposal field may be installed on slope greater than 25 percent, but
less than 46 percent. As can be seen on the profile drawing of page 3 of 3, our proposed design
is using a non -mounded Wide trench that is 12 feet deep on the up -gradient side of the disposal
field, with a effective depth of 4 feet, and a maximum trench width of 5 feet. This meets
requirement #1 and #2 of this section. There is natural vegetation 50 feet downgradient of the
proposed trench and it shall remain undisturbed. This meets requirement #3 of this section. The
distribution pipe invert will be greater than 36 inches below the top of the natural organic layer
measured on the downhill side. This meets requirement #4 of this section. Based upon these
justifications, it is our opinion that we have provided sufficient information to show that we have
meet the requirements of the revisions the wastewater code and we request this waiver be
granted.
