I have reviewed the Class C Public Water System Registration form and your waiver request for an existing community sewer line and manhole. Waiver Review It appear that the community wastewater collection line and a manhole were installed in 1962 and the well was drilled in 1964. Your request to obtain approval for a Class C public water system has required a waiver review of the site conditions. The waiver requests are as follows: • A 24 foot waiver from the existing Class C public water system source well on Lot 8, Block 2, Calais Subdivision to the community sewer line. • A 95 foot waiver from the existing Class C public water system source well on Lot 8, Block 2, Calais Subdivision to the community manhole. I have completed my review of the files on this property and the information that you have submitted on this project. Based on the existing soils conditions, the submitted test results are satisfactory for total coliform bacteria and nitrate (as nitrogen), it appears the two waivers are reasonable. Therefore, in accordance with the State Drinking Water Regulations (18 AAC 80.020(c) & (d)), the following waivers are granted: • The separation distance between the Class C public water system source well (PWSID # 218648) located on Lot 8, Block 2, Calais Subdivision to the existing community sewer line has been waived from 100 feet to 24 feet. • The separation distance between the new Class C public water system source well (PWSID # 218648) located on Lot 8, Block 2, Calais Subdivision to the existing community wastewater manhole has been waived from 150 feet to 95 feet. ' The distances noted in the waivers are from your submittal. It appears that these distances are from the nearest edge of the existing well casing to the nearest edge of either the existing community sewer manhole or the wastewater collection line. If either the existing community sewer manhole or the wastewater collection line fails or if contamination can be traced back to either of them, the waivers will be invalid. Public Water System It appears that'the drinking water system has been designed in accordance with State Drinking Water Regulations and guidance. Therefore, in accordance with State Drinking Water Regulations (18 AAC 80.200(a)), Class C Public Water System registration is granted. The signed registration form constituting this approval, is enclosed. ' This approval does not imply the granting of additional authorizations, nor obligate any state, federal or local regulatory body to grant required authorizations. STATEMENT OF INSPECTION BY ENGINEER As certified by my seal affixed hereto and as of the validation date shown below, I verify that my investigation, based on procedures outlined in the Certificate of On-Site Systems Approval Guidelines for this application, shows that the on-site water supply and/or wastewater disposal system is (are) safe, functional and adequate for the number of bedrooms and type of structure indicated herein. I further verify that based on the information obtained from the Municipality of Anchorage files and from my investigation and inspection, the on-site water supply and/or wastewater disposal system is(are) in compliance with all applicable Municipal and State codes, ordinances,and regulations in effect at the time of installation. In conducting an adequacy test, I attempt to provide a thorough, conscientious engineering analysis of the system in accordance with MoA COSA guidelines and regulations.The reported results describe the performance of the system under the conditions encountered at the time of the test, and separation distances measured to readily identifiable features. The operational life of all wells and septic systems depend on the local soil condition,ground water levels that may fluctuate during the year,and the water usage of the family being served by the system.These conditions are outside the control of the evaluator of this system. All systems eventually fail and satisfactory test results do not guarantee future performance of the system, nor do they guarantee that there are no hidden defects or encroachments.Therefore we cannot provide any warranty for future performance, nor can we estimate remaining life of the system. Box 196650 Anchorage, AK 99519-6650 www.muni.org/onsite (907) 343-7904 Arsenic Advisory Certificate of On-Site Systems Approval # OSC 171442 A Certificate of On-Site Systems Approval inspection and test of potable water was recently conducted on the well water supply on Block 2, Lot 8 of Calais Subdivision. This inspection revealed an arsenic concentration of 14.1 micrograms per liter (ug/L) for the property's well water sample. The Environmental Protection Agency (EPA) has established a maximum contaminant level (MCL) of 10.0 ug/L for public drinking water systems. While private wells are not subject to this regulation, EPA standards are based on existing health information and can therefore be used to gauge the relative quality of water from private wells. Information on arsenic is available from the On-Site Water and Wastewater Program website (www.muni.org/onsite) or at 343-7904. This advisory must be attached to all copies of the subject Certificate of On- Site Systems Approval.
