The application has been reviewed and the following comments have been generated. These are to be satisfactorily
addressed prior to MOA approval:
1. Inspection report and record drawings for the septic system have been submitted and reviewed, but house has not
been installed yet. Once the house is completed, the following will be needed:
a. Swing tie distances for all septic standpipes.
b. Final grade elevations for the septic tank and field.
c. Separations to well and foundation.
d. Final electrical approval from MOA.
e. Confirmation that berms are in place, as needed, to provide a minimum 50 ft path of travel for any surfacing effluent
before it reaches surface water.
f. Pump install log and water sample results for total coliform, nitrates and arsenic.
g. An updated as -built survey to show structure and well.
2. An updated AWWTS Maintenance Agreement for the Quanics system shall be submitted prior to final inspection report
approval or issuance of a COSA (whichever comes first).

Advanced Treatment System
Per the request of the contractor, he would like to switch to a CAT II Quanics Aerocell
system instead of an Orenco CAT III Advantex system. In short, we would be switching
from CAT III to CAT II and would need a total FT of 150 FT (instead of 125 FT 2) for an
application rating of 4 (<1 min/inch soil) for a 4 -bedroom home (600 FTz). We are
proposing to increase the width of the existing bed by 1 foot. In summary, the proposed
bed will be 6 feet wide instead of 5 feet wide. The contractor is proposing to use an
Anchorage Tank 1500 -gallon steel STEP tank with a polyurethane coating. Please let
me know if you have any questions or need additional information/design drawings.
We are unaware of any adverse impacts this installation or waivers would have on
adjacent wells or septic systems. If you have any questions, please contact us at 337-

Ref: Septic system design for Chugach Park View; Lot 29
To whom it may concern:
The existing lot is vacant. Per the request of the homeowner, he would like a 4-bedroom
septic design with his home located between the two creeks (see attached 100 scale).
As can be seen on the attached MOA wetlands map and the 100 Scale, there are "class
A" wetlands on the property, as well as two creeks (surveyed in by Fred Walatka, PLS),
limiting the location of the septic system. Due to these facts and the presence of high
groundwater (see attached soil log), we are proposing CAT III Advanced treatment
(Advantex). We are proposing to install a new 1500-gallon Advantex tank with Advantex
components, one AX-20 Pod and a mounded five-wide type trench with a 4.5-foot MOA
approved sand filter. Additionally, due to high groundwater, we are proposing to utilize
Engineered Receiving Soil. We are proposing to excavate down to 3.5 feet (into the GP-
GM material), import 4.5 feet of MOA approved sand, and construct the drainfield above
grade. It should also be noted that MOA onsite employee Tim Ecklund and GEG
employee David Garness met onsite on 6/13/2018 to discuss the proposed design. Tim
Ecklund was in agreement with the proposed location and felt as though it would not
pose a health/safety risk to install the trench as proposed. Comments regarding the
design are summarized as follows:
1. SOILS: When excavating the percolation bench/testhole the ground was frozen to
about 2.5+ feet and groundwater was at approximatley 3 feet. Due to the high
groundwater, and frozen soil, a percolation test could not be performed (see attached
soil log). However, a soil sample was pulled from the GP-GM material below the ML
layer and submitted to Alaska Test Labs for sieve analysis (attached). As can be seen
on the attached sieve analysis, the soil is classified as a GP-GM. It should be noted that
during the excavation of the testhole, water was pouring into the testhole at a depth of
approximately 3.5 feet, indicating a percolation rate of less than 1 minute/inch. Based
upon this fact, we are proposing an application rating of 5 GPD/FT2 for a CAT III
system. This application rating is used for percolation rates of up to 15 min/inch, which
is conservative and reasonable for the application of this design.

2. DRAINFIELD DESIGN: See attached design drawing for drainfield specifications.
3. SURFACE WATERS: As can be seen on the attached design drawings, there are
creeks to the west and east. See the "Waiver" section below and the design drawings
for more information.
4. TOPOGRAPHY: As can be seen on the attached design drawing, the topography
around the proposed drainfield is less than 10%.
5. WAIVER: We are requesting several waivers for the subject property. Below is a list
of the waiver request, followed by its justification:
• Drainfield, Tank, and AX-20 pod to surface water from 50 feet to 35 feet
• The use of Engineered Receiving Soil
• Waive the requirement for a percolation test
Justification for granting these waivers is as follows:
• Due to the abundance of surface water on the property and the presence of
Class A wetlands, there is minimal room to place an onsite septic system. We
feel as though the proposed location maximizes the separation distances given
the proposed home location.
• We are proposing the use of CAT III advanced treatment. The effluent quality will
be much greater than that of conventional treatment.
• We are proposing to use a 4.5 foot sand filter, after passing through the sand
filter, the effluent quality will be even greater than that of CAT III after passing
through the sand filter.
• The Advantex system will be remotely monitored. If the system were to go into
"high-level" alarm, the service provider would be notified, and corrective action
taken.
• The area between the proposed drainfield and surface water is vegetated.
• During the excavation of the testhole, the west creek did not appear to be
flowing.

Article 5.4 Sizing the Absorption Field
After selecting the most appropriate type of absorption system for a given site,the infiltrative
surface is sized based on soil conditions. Recognizing soil conditions and accurately rating the
absorption capacity of the soils is vital to the design of a good system. At least two prospective areas
should be investigated.
When conditions are suitable, a deep trench soil absorption system is recommended and should be
used because they tend to out-perform other system types.
The perimeter of each proposed absorption area should be temporarily marked using stakes and
string line. Every effort should be made to keep heavy equipment and vehicular traffic outside of the
marked areas to prevent compaction effects on the soil absorption characteristics.
The following sizing information shows the specific data needed to find the recommended soil
loading or application rate,in square feet of infiltrative area per bedroom.
• GW (Well Graded Gravel with percolation rate faster than 1): A two (2) foot thick sand liner
beneath the sewer rock is required. Use an application rate of 150 square feet per bedroom.
• GW (Well Graded Gravel with percolation rate of 1 or slower):A sand liner beneath the sewer
rock is not required. Use an application rate of 125 square feet per bedroom.
• GP (Poorly Graded Gravel with percolation rate faster than 1): A two (2) foot thick sand liner
beneath the sewer rock is required. Use an application rate of 150 square feet per bedroom.
• GP (Poorly Graded Gravel with percolation rate of 1 or slower):A sand liner beneath the sewer
rock is not required. Use an application rate of 125 square feet per bedroom.
• SW (Well Graded Sand): Use an application rate of 150 square feet per bedroom.
• SP (Poorly Graded Sand): Use an application rate of 150 square feet per bedroom.
• GM (Silty Gravel),SM (Silty Sand),or ML(silt): Requires an engineer's soil log and percolation

MAINTENANCE AND REPAIR AGREEMENT
THIS MAINTENANCE AND REPAIR AGREEMENT, herein the "AGREEMENT" made and

(AMC) 15.65.365. In consideration of the mutual covenants contained herein, the parties to this
Agreement agree as follows:
1. Advanced Wastewater Treatment Systems. The Municipality grants permission to the
Owner to utilize and operate an Advanced Wastewater Treatment System (AWWTS),

2. Maintenance,Repairs and Alterations.

TA/ Throughout the term of this Agreement, the Owner shall enter into a service agreement
with an AWWTS service and maintenance provider approved by the Municipality or the
manufacturer's representative. The AWWTS shall be maintained in a satisfactory
condition capable of performing as designed and producing treated septic effluent in
accordance with the equipment's approval for operation in the Municipality.
It shall be the responsibility of the Owner during the term of this Agreement to pay for all
repair(s), maintenance, adjustment(s),replacement costs, and inspection costs. This
(^/ includes an annual maintenance fee (typically $400 to $600).
IV " Owner agrees that only maintenance and repair personnel approved by the Municipality
or the manufacturer's representative will inspect and make any necessary maintenance,
/ repairs or permitted alterations to the system.

Owner acknowledges that regular maintenance of an AWWTS reduces the potential
failure of the system,which could include sewage backup and costly repairs or drainfield
replacement.

repairs from the manufacturer's representative or maintenance provider.
Owner acknowledges that the fine for failing to maintain and repair an AWWTS may be

Owner agrees to grant the Municipality reasonable access to test and inspect the
AWWTS. The Municipality will give at least 24-hour notice.
TV") Owner agrees that any sale or transfer of title of the property will not occur without a new
Certificate of On-Site Systems Approval.
TO Owner agrees that the AWWTS installation and maintenance requirements as provided
by the AWWTS vendor/installer and approved by the Municipality are the governing
, guidelines for the construction, maintenance and repair of the Owner's AWWTS.
Owner agrees to maintain remote monitoring of the AWWTS as required by the
AWWTS approval.
3. Term. The term of this Agreement shall begin on the date of approval by the
Municipality to operate the installed system, or upon transfer of title, and shall continue
while the AWWTS is operational or until title is transferred.
4. Nonwaiver. The failure of the Municipality at any time to enforce a provision of this
Agreement shall in no way constitute a waiver of the provisions, nor in any way affect
the validity of the Agreement or any part hereof, or the right of the:Municipality •
thereafter to enforce every provision hereof.
5. Amendment. This Agreement shall only be amended by authorized representatives of
the Owner and Municipality. Any attempt to amend this agreement by either an

6. Jurisdiction: Choice of Law. Any civil action arising from this Agreement shall be
brought in the Superior Court for the Third Judicial District of the State of Alaska at
Anchorage. The laws of the State of Alaska shall govern the rights and obligations of the
parties under this Agreement.
7. Severability. Any provisions of this Agreement decreed invalid by a court of competent
jurisdiction shall not invalidate the remaining provisions of the Agreement.

THIS MAINTENANCE AND REPAIR AGREEMENT, herein the "AGREEMENT" made and

2.Maintenance, Repairs and Alterations.
(Owner is required to read, understand and initial each section)
RB Throughout the term of this Agreement, the Owner shall enter into a service agreement
with an A WWTS service and maintenance provider approved by the Municipality or the
manufacturer's representative. The A WWTS shall be maintained in a satisfactory
condition capable of performing as designed and producing treated septic effluent in
accordance with the equipment's approval for operation in the Municipality.

includes an annual maintenance fee (typically $400 to $600).

repairs or permitted alterations to the system.
Owner acknowledges that regular maintenance of an A WWTS reduces the potential

replacement.
