I would certainly like to have a copy of both as you state the homeowner is responsible for such.
If this is asking too much from your office at least be considerate enough to provide a name and contact
information
for the engineer as again, you are expecting the homeowner to have records that go back almost four
years.
This will at least give me a place to start to attempt to work on your other request for as -built survey.

Mr. Cox,
Per Anchorage Municipal Code AMC 15.65.070D, it is the responsibility of the owner or his authorized representative to
submit a final inspection report, record drawings and current as -built survey within 90 days of the final inspection. There is
only so much the Onsite Water and Wastewater Section can do to get an engineer to close out permits since we do not
know the status or nature of the contract that they have with each homeowner. Prior to sending letters to homeowners, we
do send the engineers a list of open permits and request that they give us an update on each. In some cases, the engineer is
waiting on the homeowner to do something before they can prepare their final record drawings (such as hiring a land
surveyor to complete the as -built survey). In other cases, the homeowner has not paid the engineer for their services to
close out the permit. In your case, the engineer contacted us and said that he had been waiting on a current as -built survey,
but he was able to complete his inspection report and record drawings without the survey and he submitted those to us
after we had sent out your letter. AMC 15.65.070F requires a current as -built survey to be submitted prior to final
inspection report approval, so that is currently all that we would need to close out your permit. We can defer this
requirement to time of title transfer, when a Certificate of Onsite System Approval would be needed, or at time of any
needed drainfield upgrade. However, we encourage homeowners to have the survey done sooner in case the survey reveals
that the contractor did not install the tank and/or drainfield such that it meets code required separations.
As for the inspections, the Onsite Section does not have any required inspections. Code requires that we be notified at
specific times during the installation and we do our best to make all inspections. However, if we are unable to make an
inspection the contractor is allowed to proceed. The engineer's inspections are required, so it would be 90 days after the
final engineer inspection that the close-out paperwork should be submitted.
We apologize that our letter came across as threatening and harassment, but we have found that most homeowners are not
aware that the final paperwork has not been submitted and we want to let them know the possible consequences that
might result if a permit does not get closed out.

Interesting that after approximately four years you expect the homeowner to do your job. When installing the
replacement septic tank, I contracted with a reputable septic tank installation firm who insisted on utilizing an
engineering firm with whom they frequently worked to file for the permit. As I recall the septic firm installing the
replacement tank called for an inspection prior to covering the new tank. They were advised that no one was going
to show and given permission to cover the tank. I remember questioning this and was informed that it was the
current practice for no inspector to show for inspecting a direct tank replacement. I never was provided a copy of
the permit that was filed by the engineer nor any of the other documents you are requesting.
I do not appreciate being threatened with fines nor harassed when I was trying to be a responsible citizen by
replacing an aging rust riddled septic tank.
Your office should have on file all the paperwork needed to contact the engineer who filed the original paperwork
for the permit. This is sad commentary on how poorly the Development Services Department On -Site Water and
Wastewater Section functions requesting this from the homeowner after almost four years.

We are submitting an inspection report and record drawings for the subject property. We are requesting
the department consider approval of the inspection report without receipt of an asbuilt survey:
• Per 15.65.070, F, the department may determine that an asbuilt survey is unnecessary.
• An asbuilt will be required for the property when it next needs a COSA.
• There is little risk to the health and safety by accepting the inspection report and record drawings
without an asbuilt survey.
• There are not any known wells, surface water, or other surface features which are impacted by
this system. The location of nearby wells and the creek offset are well documented and shown on
the plans.
• The new septic tank standpipes were located relative to the house by swingtie.
• The new tank was placed in the same location as the old tank.

RE: Proposed Septic System Upgrade for Clearview lot 9
Dear Reviewer,
The above referenced property is currently served by a 4 bedroom septic system installed in 1995. The
1250 gallon tank has failed and is in need of immediate replacement.
We are proposing that the existing tank be replaced with a new 1250 or 1500 gallon tank constructed and
installed in accordance with MOA requirements.
The tank shall be covered with a minimum of 2” moa approved insulation and 3’ of cover.
The repair shall be performed by a moa certified installer in accordance with MOA requirements.
The engineer will inspect the tank before backfilling.
Repair of the proposed system will not negatively impact adjacent lots.
Upon completion of the installation, a record drawing will be submitted showing the location of the new
tank, leachfield, well, and other applicable features.

1. There are three replacements site identified for this property. The platted reserve sites have been
approved by DHSS. Technically the trench shown adjacent to the existing trench is not required.
2. & 3. Testhole location is shown. Soil was uniform for the total length of the trench, e/~
4. This system is located on a slope. The actual elevation of the testhole will be lower or higher than
the trench elevation, depending upon the location of the testhole, up slope or down slope of the trench. Elevation of
testhole is not available. Total testhole depth was 18 feet.
5. With 5 feet of soil rated at 1.2 gpdpsf and a trench length of 55 feet effective area is 550 sq.fi.
6. This location was accepted by Robbie Robinson and Jim Cross on a field inspection in the fa'st week
of September 1995. The original location of the septic system was at the plotted locations. The developer was told by
Robinson and Cross that the location on the slope was acceptable.

Gentlemen;
Per your verbal instructions additional percolation tests were performed at the location of testhole
#3 at this property. The purpose of the additional percolation test was to establish a design rate for
an absorption field in the vicinity of testhole #3. The slope at this location is steeper than 25%
within 50 feet of the proposed trench, however, the developer of the lot was told by DHSS
representative, Mr. Robinson, that a slope waiver to 35 would be granted.
Accordingly we request a slope waiver to 35 feet to install a trench not less than 35 feet from a 25%
slope. The revised design and the percolation test is submitted for your review.

Gentlemen;
We are requesting permit to install well and septic systems on lot 9 of Clearview S/D.
Soil logs and percolation tests performed in 1991 are on file with the Health Department. These files show that no
groundwater was observed on 8-26-91. The monitors are in place and no water was observed on May 26, 1995.
The percolation rates were confirmed on May 26, 1995, as shown by the attached forms. The silt layer between 5 and
6 feet was also tested and found to have a percolation rate of 56 minutes to the inch.. This layer is not included in the
design of the drainfield.
A third testhole was excavated down the slope. Sand was observed below 6 feet. The sand perc'ed at 3 minutes to the
inch. No ground water as observed. The slope at this location is steeper than 25%.
We propose to use the area along Alatna Avenue for the drainfield. A distance of 50 feet can be obtained to slopes
steeper than 24%. The well on lot 4 will not interfere with the septic system installation.
