5. STATEMENT OF INSPECTION BY ENGINEER
As certified-by my seal affixed hereto and as of the validation date shown below, I verify that my investigation, based on procedures outlined
in the Certificate of On-Site Systems Approval Guidelines for this application, shows that the on-site water supply and/or wastewater
disposal system is (are) safe, functional and adequate for the number of bedrooms and type of structure indicated herein. I further verify that
based on the information obtained from the Municipality of Anchorage files and from my investigation and inspection, the on-site water supply
and/or wastewater disposal system is(are) in compliance with all applicable Municipal and State codes, ordinances,and regulations in effect at
the time of installation.
In conducting an adequacy test,I attempt to provide a thorough,conscientious engineering analysis of the system in accordance with MoA COSA
guidelines and regulations.The reported results describe the performance of the system under the conditions encountered at the time of the test,
and separation distances measured to readily identifiable features. The operational life of all wells and septic systems depend on the local soil
condition,ground water levels that may fluctuate during the year,and the water usage of the family being served by the system.These conditions
are outside the control of the evaluator of this system. All systems eventually fail and satisfactory test results do not guarantee future
performance of the system,nor do they guarantee that there are no hidden defects or encroachments.Therefore we cannot provide any warranty
for future performance, nor can we estimate remaining life of the system.The content of this report is for the sole benefit of the owner listed
above.

Drain Field Separation Waiver
Ladies and Gentlemen:
1. Drain Field Separation Waiver: I request a waiver to the code requiring a minimum drain field
separation from the foundation of ten feet(10') to a distance of four feet(4'). The existing drain field was
installed in 1988 and is operating adequately. The system was installed under ADEC permitting and
approval. A garage addition in 1999 constructed a footing too close to the existing drain field. The garage
is constructed on a shallow footing with rigid insulation extending outward from the foundation and over
the drain field end. It is highly unlikely, verging on impossible, that effluent will daylight in the garage
footing or effect the stability of the shallow foundation.
The existing installation does not affect the future development of the surrounding or existing lots, nor
does it pose a health threat to this property. If you have any questions or concerns,please contact me at

Attached please find the asbuilt plans for the 4 bedroom, duplex
dwelling sewage system installed as permitted.
The system, based on a 150 soil rating, has an absorption area of
700 S .F. The attached plans include swing ties for trench location.
The septic tank incorporates a manhole to allow owner inspection of
the tank's second compartment.

We have reviewed the plans and specifications for the subject project
and are hereby issuing this approval letter which constitutes the permit
required by A.S. 46.03.720(a) for construction of the sewerage system.
It should be remembered that final approval will necessitate the submis-
sion
ubmitsion of Engineer As-Built Plans and owners name, address and telephone
number. This will consequently mandate that a professional engineer
conduct basic inspection of this project so as to be able to sign off on
ass--built plans. Arrangement for this inspection work is the responsi-
bility of the developer. Any future expansion of subject project will
require additional approval from this office.
