1. The attached approved design.
2. All requirements specified in Anchorage Municipal code Chapters 15.55 and 15.65 and the State of Alaska
Wastewater Disposal Regulations (18AAC72) and Drinking Water Regulations (18AAC80)
3. The wastewater code requires inspections during the installation. The engineer shall notify the Development
Services Department per AMC 15.65. Provide notification by calling (907) 343-7904 (2417).
4. From October 15 to April 15, a subsurface soil absorption system under construction during freezing weather
shall be either:
a. Opened and Closed on the same day, or
b. Covered, sealed, and heated to prevent freezing
Special Provisions: Prior to final inspection report approval, slope stabilization of the drainfield mound and
adequate site drainage away from the house shall be confirmed by the department.

Subject: The Cascades Block 1 Lot 2 - Septic System Design
Dear On-Site Services Engineer:
The owner of the above lot called us because their maintenance report indicated water was flowing back
into the Advantex system from the trench. Our subsequent investigation revealed that the trench was
completely full. We dug a test hole and found, after monitoring the water, that groundwater had risen to
within 2.5’ of the original grade. This is a rise of nearly 6’ from our original monitored levels. After
discussions with the home builder and contractor, we are proposing to rebuild the trench in the same
spot, raising it to 3’ above the measured groundwater level, due to the September reading. The location
of the trench will not change, and while the ground slope approaches 25%, the areas immediately
downslope are also mostly under 25% and meet the requirements of section 15.65.210.B.1.e of the code.
The existing trench will be excavated, and Type II gravel will be filled in as ERS. Above that, 2’ of filter
sand will be placed under the bottom of the new 5-wide trench. With the new height, we are requesting
a waiver to allow for 2:1 side slopes for the mound. With the height of the fill slope being about 3.5’,
the 3:1 slopes will extend approximately 56’, while the house is only 20’ away. A 2:1 side slope will
extend 18.9’. The slopes will be covered with topsoil and seeded. The soils drain quickly and effluent
will migrate downward faster than it will spread laterally. An impermeable barrier such as Visqueen will
be installed to prevent lateral downhill movement and daylighting by the effluent.
Storm water drainage will not impact this septic system. The new trench will be constructed parallel to
the slope. The proposed well is shown, and adjacent properties are undeveloped. The new system will
be a minimum of 50’ from all private wells and surface water, and more than 10’ from the subsurface
drain recently constructed to control groundwater.
Please refer to the attached test hole log along with plan and profile sheets for the septic design. If this
design is followed, there will be no adverse impacts to adjacent properties.

Subject: The Cascades Block 1 Lot 2 - Septic System Design
Dear On-Site Services Engineer:
The owner of the above lot intends to build a 4-bedroom home on the property, so we are
submitting this permit application for the construction of a new well and septic system. The
attached site plan identifies the location of the home and the proposed well, septic location and
alternate site. No conflicts exist between this proposed system and any other well or septic system,
whether on this lot or adjacent lots.
The ground surface on the lot slopes moderately and evenly from the back of the lot toward the
road. Slopes range from 20-30%. There are very few spots where slopes approach 40-50%, just
small dips in the ground only a few feet long. Drainage arrows are shown on the site plan showing
the grade and direction of flow. Storm water drainage will not impact this septic system. The new
trench will be constructed parallel to the slope.
A Category III Advantex Treatment System will be constructed on the lot. This system allows for
reduction in both vertical and horizontal separation from groundwater and excessive slopes. The
trench will be built in an area where the slopes are less than 25%, along with the alternate site. The
areas immediately downslope are also mostly under 25% and meet the requirements of section
15.65.210.B.1.e of the code.
The percolation rate measured at TH3 was under 1 min/in. The laboratory analysis designated the
soils as GW-GM, and not GW. Additionally, soils tests and percolation tests on other lots in the
area demonstrate that this type of loose soil is not commonly found here. This appears to be a
relatively small pocket of this type of gravelly soil, and we are confident that, according to the
exception at 15.65.210.D.1.c, other soil layers exist that will protect the aquifer from
contamination. A subdrain recently excavated near the test hole did not indicate any areas of
subsurface material which would absorb water at less than 1 minute per inch. The secondary perk
test done approximately 50’ away showed the same soil type at approximately the same level. The
consistency over this area makes us confident that the alternate site is adequate, although it is not

within 30’ of the test hole. We request that this alternate site be approved for this purpose. If
required, a test hole can be dug at a future time to verify groundwater levels.
The subdivision file mentions a spring on the lot, draining along the northwest property line. A
site visit on Thursday, July 26 for the purpose of investigating this did not reveal evidence of a
spring or regular running surface water on the lot, and particular attention was shown to the
northwest property line. Additional groundwater levels were taken, to compare with subdivision
records and to check the effectiveness of the curtain drain. The test hole was now dry down to the
bottom.
The proposed well is shown, and adjacent properties are undeveloped. The new system will be a
minimum of 50’ from all private wells and surface water, and more than 10’ from the subsurface
drain recently constructed to control groundwater.
Please refer to the attached test hole logs, soils lab analysis, plan and profile sheets for the septic
design. If this design is followed, there will be no adverse impacts to adjacent properties.

Anchorage,herein the"MUNICIPALITY", in accordance with Anchorage Municipal Code
(AMC) 15.65.365. In consideration of the mutual covenants contained herein,the parties to this
Agreement agree as follows:
1. Advanced Wastewater Treatment Systems. The Municipality grants permission to the
Owner to utilize and operate an Advanced Wastewater Treatment System(AWWTS),
described as Advantex Treatment System

2. Maintenance.Repairs and Alterations.

Throughout the term of this Agreement,the Owner shall enter into a service agreement
with an AWWTS service and maintenance provider approved by the Municipality or the
manufacturer's representative. The AWWTS shall be maintained in a satisfactory
condition capable of performing as designed and producing.treated septic effluent in
accordance with the equipment's approval for operation in the Municipality.
. It shall be the responsibility of the Owner during the term of this Agreement to pay for all
repair(s),maintenance,adjustment(s),replacement costs,and inspection costs. This
//77 includes an annual maintenance fee(typically$400 to$600).
.)2jOwner agrees that only maintenance and repair personnel approved by the Municipality
or the manufacturer's representative will inspect andmake any necessary maintenance,
repairs or permitted alterations to the system.
PIP4P5Owner acknowledges that regular maintenance of an AWWTS reduces the potential
failure of the system,which could include sewage backup and costly repairs or drainfield
replacement.

ePi..2 Owner acknowledges that the Municipality may request records of maintenance and
repairs from the manufacturer's representative or maintenance provider.
,QV Owner acknowledges that the fine for failing to maintain and repair an AWWTS may be
assessed in accordance with AMC 14.60.030.
gbj2j>7 Owner agrees to grant the Municipality reasonable access to test and inspect the
AWWTS. The Municipality will give at least 24-hour notice.
i 27/3 Owner agrees that any sale or transfer of title of the property will not occur without a new
Certificate of On-Site Systems Approval.
)<J 'Owner agrees that the AWWTS installation and maintenance requirements as provided
by the AWWTS vendor/installer and approved by the Municipality are the governing
guidelines for the construction,maintenance and repair of the Owner's AWWTS.
2�Owner agrees to maintain remote monitoring of the AWWTS as required by the
AWWTS approval.
3. Term. The term of this Agreement shall begin on the date of approval by the
Municipality to operate the installed system,or upon transfer of title,and shall continue
while the AWWTS is operational or until title is transferred.
4. Nonwaiver. The failure of the Municipality at any time to enforce a provision of this
Agreement shall in no way constitute a waiver of the provisions,nor in any way affect
the validity of the Agreement or any part hereof,or the right of the Municipality
thereafter to enforce every provision hereof.
5. Amendment. This Agreement shall only be amended by authorized representatives of
the Owner and Municipality. Any attempt to amend this agreement by either an
unauthorized representative or unauthorized means shall be void.
6. Jurisdiction: Choice of Law. Any civil action arising from this Agreement shall be
brought in the Superior Court for the Third Judicial District of the State of Alaska at
Anchorage. The laws of the State of Alaska shall govern the rights and obligations of the
parties under this Agreement.
7. Severabilitv. Any provisions of this Agreement decreed invalid by a court of competent
jurisdiction shall not invalidate the remaining provisions of the Agreement.
