To whom it may concern:
Upon completion of the documentation of construction for the septic upgrade for the referenced
property, it came to our attention that the 10 -foot separation distance to west lot line was not
met with this installation. Due to the construction methods used by the owner who performed
the installation himself, it could not be verified during construction the separation distance to this
lot line. We are requesting that a 6 -foot lot line waiver be granted from the new drainfield to the
west lot line. We are unaware of any adverse effect on the adjacent property with the granting of
this waiver.

Hi Rebecca,
Here are some photos of the septic repair. Once the hole was dug, I had Alaska Quality Septic pump out the tank. I then did
a wipe down inside of the repair area, and applied the mastic coating. I put mastic on the lip of the lid cover before
replacing. If you have any further questions please let me know.

Jody,
During the installation of the septic system, the top of the septic tank was damaged by the tooth of the excavator.
I went to Anchorage Tank who I had purchased the system from and discussed the repair with them. Their shop foreman
gave me a round cut out that they have from cutting out the top hatches when they are manufacturing them. This is the
same gauge material of the tank itself. I asked what they recommended I use for waterproofing material. They
suggested any roofing mastic would work. I then cut the damaged area so that I had straight lines to weld to. I used a
MIG welder to make the repair with the material that I had picked up at Anchorage Tank. I then used a roof mastic to
waterproof the area of the repair. I am including photos of the damaged area and the repair.

Jim,
I went to your property on Tuesday and swung -tied all the new septic pipes and shot final grades. I
have completed the record drawing and inspection report for the new septic system that you installed.
In order to submit the documentation paperwork, we need the following items from you:
• We need an updated as -built survey from you registered and surveyor. This is a new
requirement that came into effect recently that requires a updated survey be done after all

installations. I have attached the preliminary record drawing which shows all the new septic
standpipes that were installed with the table that has all the swing -tie information from the
house corners. I have cc'd Buku with Fixed Heights, LLC so he has this information so he can
update his survey. Please contact him to have this work completed and have him email me a
signed PDF copy of the survey.
Since the tank was damaged during the installation, we need an email or letter from you
stating how you fixed it, that you consulted the tank manufacture about the repairs, and that
the tank is watertight and good working order.
We also need an email or letter stating that the old septic system was completely
decommissioned per Uniform Plumbing Code (UPC)
As you can see on the record drawing that is attached, the drainfield is slightly over 6 feet from the
west lot line. The separation distance requirement is 10 feet. We are requesting a 6 foot lot line waiver
with the record drawing. We do not foresee any issues with the granting of this waiver. Once we have
this information, we will invoice you for service to date and submit the documentation and record
drawings to the MOA Onsite Department

Reference: Septic System Upgrade for Peters Creek S/D; Block 2, Lot 5
To whom it may concern:
The existing 3 bedroom house on the referenced property is served by a AWWU public water and an
undocumented septic system. The septic system is possibly an old seepage pit, but it is unknown
since the standpipe only extends -4 feet below grade and is filled with dirt. The owner would like to
have a documented septic system to serve his house and has hired us to design the septic system.
We are proposing that a new 2-compartment 1000 gallon plastic septic tank and deep trench type
drainfield be installed. If after the line exiting the house is excavated and it is determined that the
maximum burial depth for the plastic depth cannot be achieved, a 1,000 gallon steel septic tank will
be installed instead. A test hole has been excavated in the area for the proposed upgrade and the
drainfield has been designed around the 30 foot radius of this test hole. Comments regarding the
design are summarized as follows:
1. SOILS: See the attached log which shows the soil classifications, groundwater monitoring, and
the percolation test results.
2. DRAINFIELD DESIGN: See attached design drawing for drainfield specifications.
3. SURFACE WATERS: No surface waters were observed within 100 feet of the proposed septic
system.
4. TOPOGRAPHY: Attached is the MOA contour map of the area. As can be seen on this contour
map, there does not appear to be any slope concerns with the proposed upgrade. We have also
shown some slope information on the design drawing for your reference.

The On-site Water and Wastewater Section (On-site) may issue an approval for a homeowner
to perform work on an on-site wastewater disposal system to serve that individual's owner-
occupied, single-family or duplex home if the homeowner meets and agrees to the following
requirements:
1. The property owner and excavation equipment operator may perform work on no more
than one owner-installation project in a 12-month period.
