To whom it may concern:
The existing lot is vacant. Per conversations with the owner, he would like to construct a 1-bedroom home. We are
proposing to install a two compartment IM-1060 Infiltrator followed by a single compartment IM-540 infiltrator septic
tank (to be used as a treatment tank) with an ATS-6 Aerocell pod, a pump control panel and shallow trench/5-wide
type drainfield. Four (4) "testholes"were excavated and found to have shallow bedrock was encountered at a depth
of about 1.5-2 feet in all of the testholes. Due to this fact, perforated pipe and a percolation tests were not
performed/installed. The 5th testhole was excavated to the west of the lot (labeled T.H.#1 on attached drawings)
where bedrock was encountered at approximately 5.5 feet below grade. A percolation test and 7-day groundwater
monitoring were performed. Based upon the excavations performed, it appears that bedrock depth is "dropping" to
the west of the lot. Due to this fact,we feel it is reasonable to assume there is at a minimum of 2 feet of receiving soil
at the proposed location of the drainfield. Comments regarding the design are summarized as follows:
1. SOILS: See attached 2019 testhole by Garness Engineering Group, Ltd. which shows, soil classifications,
percolation test and groundwater monitoring. It should be noted that soil logs were not completed for the shallow
bedrock"testholes", but the location and depth of bedrock are noted on the design drawings.
2. DRAINFIELD DESIGN: See attached design drawing for drainfield specifications.
3. SURFACE WATERS: There are no surface water concerns.
4. TOPOGRAPHY: The slopes for the lot were approximately 25-30%.We are proposing to meet the"steep slope"
provision with the"exception"for shallow trenches with 6 inches of effective. Our proposed trench meets the following
requirements:
• We are proposing to install a"shallow trench"with 6 inches of effective depth.
• The trench shall not exceed 60 inches in width.
• We are proposing to install 3 feet of sand below OG on downgradient side(as required by the provision).
• The downgradient area is undisturbed and vegetated for a minimum of 50 feet downgradient of the proposed
drainfield

MAINTENANCE AND REPAIR AGREEMENT
THIS MAINTENANCE AND REPAIR AGREEMENT, herein the "AGREEMENT" made and

Anchorage, herein the "MUNICIPALITY'', in accordance with Anchorage Municipal Code
(AMC) 15.65.365. In consideration of the mutual covenants contained herein, the parties to this
Agreement agree as follows:
1. Advanced Wastewater Treatment Systems. The Municipality grants permission to the
Owner to utilize and
operate an Advanced Wastewater Treatment System (AWWTS),

2. Maintenance,Repairs and Alterations.
(Owner is required to read, understand and initial each section)
41. Throughout the term of this Agreement, the Owner shall enter into a service agreement
with an AWWTS service and maintenance provider approved by the Municipality or the
manufacturer's representative. The AWWTS shall be maintained in a satisfactory
condition capable of performing as designed and producing treated septic effluent in
accordance with the equipment's approval for operation in the Municipality.
It shall be the responsibility of the Owner during the term of this Agreement to pay for all
repair(s), maintenance, adjustment(s), replacement costs, and inspection costs. This
includes an annual maintenance fee (typically $400 to $600).
Owner agrees that only maintenance and repair personnel approved by the Municipality
or the manufacturer's representative will inspect and make any necessary maintenance,
repairs or permitted alterations to the system.
Owner acknowledges that regular maintenance of an AWWTS reduces the potential
failure of the system, which could include sewage backup and costly repairs or drainfield
replacement.
