During the process of requesting a COSA, it was documented that the liquid level in the septic tank was five (5) inches out of level between the two compartments. This is likely due to the tank being installed in a high groundwater area and while being pumped the tank was damaged. We are proposing to decommission the existing 1300 -gallon HDPE tank and install an IM -1530 Infiltrator tank (two-compartment). The existing pump vault will remain in place. This includes an annual maintenance fee (typically $400 to $600). Owner agrees that only maintenance and repair personnel approved by the Municipality or the manufacturer's representative will inspect and make any necessary maintenance, repairs or permitted alterations to the system.
