intervals pf 800 feet in non-commercial areas to reduce inconvenience and sanitary hazards during
repairs.
Based on'the information provided, construction approval is granted for the project. The enclosed
Construction and Operation Certificate; with the Approval to Construct section completed,
constitutes a written approval required under Drinking Water Regulations 18 AAC 80. Please note
that deviations from approved plans which affect capacity, flow, pressure, operation, compliance,
materials' of major system components (particularly any components which do not meet National
Sanitation Foundation Standards 61), must be approved by DEC prior to their construction or

Septic System Feasibility Study
Dear Dan:
Test holes were placed at several home site locations in the second phase of Campbell
Canyon Subdivision. The tract is proposed for development under the Site Plan concept
and is not composed of individual lots. Each individual home site however is more than
1 acre is surface area as shown on the attached drawing. The purpose of the test
holes was to assess the underlying soil conditions at each home site and to determine
whether septic systems could be placed at the sites to serve single-family homes. The
test holes were placed to depths of 19' utilizing a tracked backhoe. An experienced
soils technician under my supervision was onsite during the excavation to log the soils
encountered and to perform percolation tests to determine the absorption
characteristics of the materials encountered. A monitor tube was placed in each hole to
determine the groundwater elevation over a period of time. No groundwater was found
in any of the holes during excavation and none has been noted to date.
The surface of the proposed site development slopes from south to north at varying
grades. In some areas the surface grades in the subdivision slightly exceed 25%, but in
most cases are usually much flatter. The property is currently heavily treed with both
spruce and birch and some areas are thick with underbrush.
A total of 30 test holes were placed throughout the tract. Soils encountered were
similar in all the holes with the upper 1' to 3' composed of organics with some peat and
organic silt. This layer was underlain by poorly graded to well graded gravel with
varying degrees of sand and silt. The majority of the material was found to be clean
and fairly dry. Percolation rates were usually less than 2 minutes per inch. These
findings compare closely with soils found in Campbell Canyon and Near Point Knoll
Subdivisions immediately to the west. This type of material is excellent for the
treatment and disposal of septic effluent. As mentioned earlier, no groundwater was
found in any of the holes during excavation and none is expected to accumulate during

the ensuing heavy runoff period. Monitoring completed during mid October indicated
no groundwater in the test holes.
The Municipal ordinance governing septic systems requires either a reserve area on
each lot for septic systems or a minimum of three septic system sites designated on the
final plat. Based on our field exploration it appears a minimum of 10,000 square feet of
reserve area can be found on each lot for septic system development. Since the lots
will be served by a community water system no wells with their respective 100'
protective radii will be required for each lot. The home sites will be developed without
lot lines, but easements will be created for the septic system sites.
A 10,000 square foot reserve area is shown on each of the home sites on the attached
plan. Test holes were not placed on 5 of the sites, but soils encountered were
consistent throughout the development. We are confident similar soil conditions would
be found on each of the 5 sites yet to be explored.
Copies of the test hole logs and percolation tests along with a Test Hole Location Plan
and septic system reserve sites are attached for your review.

Dear Ron:
The owner of Tract 1 B, Campbell Canyon Subdivision has spent substantial time and effort to
locate an additional groundwater source to serve his public water system at Campbell Canyon.
He has enlisted the services of Terrasat, Inc. to scientifically identify a location where drilling for
water has the most optimum chance of success. The attached plan shows the location of the
proposed well site and the access alignment necessary to transport the well drilling rig.
The Corps of Engineers has issued a permit which allows the drilling rig to cross the identified
wetlands during March of 2007 while sufficient snow pack exists to protect the wetlands from
damage. A minor amount of clearing and excavation will be required outside the wetlands to
provide access for the track mounted Nodwell drilling rig. All clearing will be completed by
hand and the debris from the clearing will be placed along centerline of the alignment.
Approximately 50 cubic yards of excavation is anticipated to remove knolls along the alignment.
Material from the excavation will be pushed forward to fill low spots in front of the knolls.
Grading is not allowed in areas zoned R-IOSL at this time of year except in cases of emergency.
We are hereby requesting your approval to transport the drill rig and complete the necessary
grading and excavation while the ground is still in a frozen state and snow cover is of sufficient
depth to protect both the wetlands and the surrounding area. All streams in the area, which must
be crossed are currently frozen and covered with nearly 3' of crusted snow. This is the only time
of year that a drilling rig can cross this fragile ground without damage and could be considered
an emergency to prevent damage to the environment.
Clearing and grading work will be completed using a privately owned D-6 Dozer operated under
the supervision of Terrasat, Inc. A Terrasat representative will be on site the entire time work in
Tract 1B, Near Point Knoll Subdivision

under way and will direct all facets of the operation. The Nodwell drill rig is owned and
operated by M -W Drilling Co., one of the most experienced drilling companies in Alaska. M -W
is similarly experienced in' transporting the drill rig across fragile frozen ground. ,The alignment
has been staked by LanTech, Inc. and al wetlands boundaries have been flagged. Work can begin
immediately and scheduled to start on Monday, March 26, 2007.
The access alignment will be reconstructed as much as possible to pre access conditions once all
well drilling operations are completed and the well rig has left the site. The alignment will be
monitored regularly until signs of spring thaw and runoff begin. Silt fence and other erosion
control measures will be placed at that time if found to be necessary. No further work will be
completed in the area once well drilling is completed without further permitting from both the
City and the Corps of Engineers.
A drawing is attached which identifies the access alignment, areas of excavation, drill pad site
and other pertinent information. We have also enclosed a copy of the Corps Permit allowing the
crossing of the wetlands and a letter from Tom Nelson regarding work in the R-1 OSL area at this
time of year. We hereby request your approval to complete the access alignment as outlined
above and as shown on the attached drawing.

This .is in response to your February 1, 2007, application for a Department
of the Army (DA) permit on behalf of Mr. Joseph Henri, President of South -
Central Timber Development, Inc., to cross and drill in previously unmapped
wetlands during frozen ground conditions for the purpose of determining
possible locations for high producing water wells for the proposed Campbell
Canyon/Villas Subdivision. It has been assigned

community at the end of Shavalik Drive; in Anchorage, Alaska.
DA permit authorization is necessary because your project would involve
placement of fill material into waters of the U.S. under our regulatory
jurisdiction.

to you. Regional Conditions A-E and L-0 apply to your project. You must
comply with all terms and c onditi.ons associated with NWP No. 6, as well as
with the special conditions listed beloia:
1. Project boundaries shall be clearly identified in the field (e.g.,
staking, flagging, silt fencing, etc.) prior to site clearing and
construction to ensure avoidance of impacts to waters of the U.S.,
including wetlan6s, beyond the project footprint. Tn no case may
—disturbance extend beyond the identified project footprint.
2.. The applicant shall implement and maintain appropriate and effective
erosion and sediment controls before, during, and after construction
to prevent erosion and the introduction of sediments and/or
contaminants into waters of the U.S., including wetlands. Prior to
initiating construction activities, a silt fence and/or other

sediment containmen'- structures shall be properly installed to
contain sediment anal turbidity at the work site (a) parallel to and
within 10 feet of t''ie toe of.any fill, or soil exposed within 25
feet of a standing •)r flowing waterbody, if the fill site has a
downslope or surfaca connection to the waterbody; and, (b) adjacent
to any fill placed or soil exposed within a standing or flowing
waterbody. These structures shall remain in place until all slopes
or other disturbed areas subject to potential erosion have been
permanently stabilized.
3. Equipment shall not, be fueled, serviced, or stored in wetlands or
below the ordinary high water (OHW) mark; fuel, lubricants, or other
hazardous substances shall not be stored in wetlands or below OWH.
Fueling and service vehicles shall be equipped with adequate
materials (such as sorbent pads, booms, etc.) to immediately
contain, and commerce clean up of spilled fuels and other petroleum
products.
4. Previously vegetate°d areas disturbed during well drilling activity
and any exposed slopes created by the activity shall be restored to
their original coni -our, as close as possible, and revegetated with
native plant specios. Revegetation shall be accomplished such that
percent live cover of native vegetation similar to adjacent areas is
restored to a mini;aum of 75% by October 5, 2007.
5. To ensure complian::e with this permit, all builders, contracLar,
subcontractors, eq.zipment operators, and laborers shall be advised
of the permit terms and conditions.
To ensure compliance wi-=h this permit, you are.eneouraged to provide us
with photos taken during the well drilling activities which document the
drilling site(s), access point(s), and access route(s). This documentation
will assist cooperating agencies in the.ongoing effort to resolve recent
violations that occurred on site.
Further, please note General Condition 14 requires that you submit a
signed certification to us once any work and required mitigation are
completed. Enclosure 1 is the form for you to complete and return to us.
This verification is valid until the NWP is modified, reissued, or
revoked. All of the exist:..ng NWPs are scheduled to be modified, reissued, or
.revoked prior to March 18, 2007. It is incumbent upon you to remain informed
of changes to the NWPs. We will issue a public notice when the NWPs are
reissued. Furthermore, if you commence or are under contract to commence
this activity before the date that the relevant nationwide permit is modified
or revoked, you will have twelve (12) months from the date of the
modification or revocation of the NWP to complete the activity under the
present terms and conditions of this nationwide permit.
Nothing in this letter excuses you from compliance with other Federal,
State, or local statutes, ordinances, or regulations.

Winter Access Across Wetlands
Dear Mr. Young:
This is in response to your letter dated March 12, 2007, in regards to
winter access across wetlands for Canyon Villas Subdivision.
Your letter requests the ability to cross wetlands on the eastern portion
of the subdivision with a water well drill rig during March in order to
cross while a sufficient snow pack exists to protect wetlands from vehicle
damage. The Department approves the limited intrusion as outlined in
your letter and attachcd map. This approval expires on April 15, 2007.
