5. STATEMENT OF INSPECTION BY ENGINEER
As certified by my seal affixed hereto and as of the validation date shown below, I verify that my investigation,
based on procedures outlined in the Certificate of On -Site Systems Approval Guidelines for this application,
shows that the on-site water supply and/or wastewater disposal system is (are) safe, functional and adequate
for the number of bedrooms and type of structure indicated herein. I further verify that based on the information
obtained from the Municipality of Anchorage files and from my investigation and inspection, the on-site water
supply and/or wastewater disposal system is(are) in compliance with all applicable Municipal and State codes,
ordinances, and regulations in effect at the time of installation.

In conducting this evaluation, GEG provided an engineering evaluation 'of the well and/or septic system
in accordance with the guidelines and regulations established by the Municipality of Anchorage and
industry practices. The reported results describe the condition of the system/s on the date/s of the
evaluation. Separation distances were measured to readily identifiable features. Hidden defects or
encroachments may exist that were not identified during the evaluation. The operational life of all wells
and septic systems depend upon a variety of variables, including but not limited to, soil conditions,
groundwater levels (that may fluctuate during the year), quality of construction (materials and
workmanship), and the water usage of the family utilizing the system/s. These conditions can vary, and
are outside the control of GEG. Satisfactory test results do not guarantee future performance of the
systemis; therefore, GEG makes no warranty (express or implied) regarding the future performance of
the well or septic system. GEG makes no representation whether an alternative well or septic system
can be installed on the property in the event either of the current systems fail to perform adequately in
the future. The content of this report is for the sole benefit of the person/party that retained GEG to
perform the evaluation. Reliance upon the information provided in this report by any other person or
party (including subsequent property purchasers) is not authorized, nor will it confer any legal right
whatsoever.

Reference: Kwik Log, Subdivision, B4, L12 — Waiver — Well to Public Sewer Mains
Ms. Wockenfuss:
Per MOA records, the home on the subject property was built in 1968. According to AW WU
records, it appears that the home on the subject lot existed prior to the sewer mains being
installed to the north and east of this property in around 1970. It is reasonable to believe that the
well on the subject property existing at the time the sewer mains were designed and installed
because the structure is shown on the record drawings, For some reason, the well is not shown
on the AW WU (GAAB) design or record drawings. Per our evaluation, it appears that the sewer
main to the east may be as close as 44 feet from the subject well. Ultimately AWWU is
responsible for the waiver fee and preparation of the waiver request. Given the fact that the sale
of this home is due to close very soon, we do not have time to pursue this issue with AWWU.
As a result, GEG is requesting the waiver on behalf of the property owner. If there is a waiver
fee, please coordinate payment with AWWU.
We are requesting that your department waive the required separation distance of 50 feet
between the well on the subject property and the AWWU sewer main to the east to 40 feet.
Justifications for this waiver are as follows:
1. The required separation distance at the time of construction was 50 feet, so we are only
asking for a 20% variance to the code requirement of that time.
2. The encroachment has existed for approximately 50 years.
3. Recent water samples indicated non-detectable Nitrate levels and no bacteria. In short,
water quality does not appear to have been impaired by the encroachment.

4. We were unable to find a well log for the property; however, we did find well logs for a
number of nearby properties (they were submitted with the COSA package). The logs
generally show layers of silt, clay, gravelly -silt, sandy -clay, hardpan, and other soil types
that would help impede the flow of contaminates from the sewer main (if it were to leak)
to the aquifer. All of the well/drilling logs found indicate the wells in the general vicinity
are 100 feet deep or greater and, given the static water levels, appear to be tapped into a
confined aquifer.
5. The well on the subject lot is artesian, which indicates it is tapped into an aquifer that is
under pressure.
Based upon the aforementioned information, we are requesting that your department waive the
subject separation distance to 40 feet. Please do not hesitate to call me at 337-6179 if you
